Biprol

Biprol – the medicine with antiarrhytmic and hypotensive action applied at treatment of arterial hypertension and stenocardia.

Form of release and structure

Biprol release in the form of coated tablets (on 10 pieces in a blister strip packaging, on 3,5,10 packagings in a cardboard pack; on 100 pieces in polymeric banks, after 1 bank in a cardboard pack).

Active agent: the bisoprolola fumarates – 5 or 10 mg.

Excipients: MKTs, лудипресс LTsE, starch corn, silicon dioxide colloid, кросповидон, magnesium stearate.

Indications to use

• Arterial hypertension;
• Coronary heart disease (for prevention of attacks of stenocardia).

Contraindications

• Acute heart failure;
• Shock (including cardiogenic);
• Fluid lungs;
• Collapse;
• AV blockade of II and III degrees;
• Chronic heart failure in a decompensation stage;
• Sick sinus syndrome;
• Sinuatrial blockade;
• Metabolic acidosis;
• Bronchial asthma and chronic obstructive pulmonary disease in the anamnesis;
• Printsmetal's stenocardia;
• The expressed bradycardia;
• Arterial hypotension (systolic pressure is less than 100 mm of mercury., especially at a myocardial infarction);
• Cardiomegaly (in the absence of symptoms of heart failure);
• Hypersensitivity to the components which are a part of tablets, and also to other beta adrenoblockers.

Also Biprol do not appoint:

• Along with MAO inhibitors (except MAO inhibitors of B type);
• At late stages of disturbances of peripheric circulation;
• At a pheochromocytoma (without simultaneous use of alpha adrenoblockers);
• To children 18 years are younger.

Biprol elderly people should take with caution, and also in the presence of the following diseases:

• Psoriasis;
• Thyrotoxicosis;
• Liver failure;
• Myasthenia;
• Chronic renal failure;
• AV blockade of the I degree;
• Depression (including in the anamnesis);
• Diabetes mellitus.

Route of administration and dosage

Biprol is appointed 1 time a day on 5 mg. Drug is accepted in the morning on an empty stomach. In case of need the daily dose can be increased to 10 mg. The most admissible daily dosage – 20 mg.

Side effects

Biprol seldom causes serious side effects. At administration of drug can arise:

• From a nervous system: fatigue, weakness, dizziness, frustration of a dream, concern, depression, adynamy;
• From sense bodys: vision disorders, xerophthalmus, conjunctivitis;
• From the alimentary system: nausea, vomiting, abdominal pains, disturbances of a chair, taste change, hepatic dysfunction (cholestasia);
• From cardiovascular system: bradycardia, disturbance of conductivity of a myocardium, AV blockade, development of arrhythmia, strengthening of manifestations of cardiovascular insufficiency, orthostatic hypotension; also during Biprol's use development of a vasomotor spasm (disturbance of peripheric circulation, a cold snap of extremities, aggravation of symptoms of a Raynaud's disease) is possible;
• From respiratory system: breath difficulty (when using high doses of Biprol), with predisposition – a bronchospasm.

Also during therapy development of allergic reactions is possible: rash, urticaria, itch.

Patients with a diabetes mellitus who receive insulin can have a hypoglycemia.

Symptoms of overdose of Biprol are: a faint, spasms, bradycardia, premature ventricular contraction, arrhythmia, the expressed hypotension, breath difficulty, a bronchospasm. Treatment consists in a gastric lavage and purpose of the adsorbing means. In case of development of AV blockade administration of atropine intravenously is shown. At hypotonia intravenous administration of plasma substituting solutions is shown, in case of inefficiency the dopamine, Dobutaminum, Epinephrinum are appointed. At development of heart failure cardiac glycosides, diuretics are used. In case of developing of spasms it is necessary to enter diazepam intravenously.

Special instructions

At intravenous administration of iodinated X-ray contrast agents against the background of Biprol's reception and analogs the risk of development of anaphylactic reactions is increased.

Drug together with cardiac glycosides and blockers of calcium channels (for example, verapamil) and other antiarrhytmic means strengthens risk of developing of bradycardia, heart failure and AV blockade.

Biprol changes effect of insulin and the tableted hypoglycemic drugs.

Drug masks hypoglycemia symptoms.

Non-steroidal anti-inflammatory drugs, glucocorticoids and estrogen at simultaneous use with Biprol weaken its hypotensive effect.

In combination with antidepressants and antipsychotic drugs Biprol strengthens oppression of the central nervous system, with nifedipine and diuretics – leads to a considerable lowering of arterial pressure.

During therapy daily control of arterial pressure and an ECG is necessary. At patients with a diabetes mellitus determination of level of glucose of blood is recommended (at least 1 time in 1-2 months). In case of development of bradycardia (less than 50 beats per minute) it is necessary to see a doctor immediately.

At patients with diseases of lungs before Biprol's reception the research of external function of lungs is recommended.

Biprol is not effective at 20% of patients with stenocardia, the expressed atherosclerosis of coronary vessels is the main reason for it.

At chronic smokers the effect of administration of drug is reduced.

If the patient uses contact lenses, it is necessary to consider the fact of decrease in products of the lacrimal liquid.

At development in the patient of a depression at Biprol's reception drug should be cancelled.
