Bromocriptinum

Bromocriptinum concerns to group of inhibitors of secretion of prolactin, is protivoparkinsonichesky drug.

Form of release and structure

Bromocriptinum is issued in the form of tablets of 2,5 mg (on 10, 20, 30, 40, 50, 60 or 100 pieces in blister strip packagings or polymeric containers).

Active ingredient of drug is Bromocriptinum.

Indications to use

Use of drug is reasonable at female infertility and disturbances of a menstrual cycle:

• Prolaktinzavisimy states and diseases which are followed or are not followed by a giperprolaktinemiya (an oligomenorrhea; the secondary giperprolaktinemiya induced by medicines; the amenorrhea which is followed and not followed by a galactorrhoea; insufficiency of a lyuteinovy phase);
• Prolaktinnezavisimy female infertility (a syndrome of a polycystosis of ovaries, anovulatory cycles – as additional treatment to anti-estrogen).

Also indications to purpose of Bromocriptinum are:

• Giperprolaktinemiya at men: prolaktinzavisimy hypogonadism (loss libido, oligospermatism, impotence);
• Benign diseases of mammary glands: high-quality cystous and/or nodal changes, especially fibrous and cystous mastopathy; a mastalgiya in the isolated look or in combination with high-quality cystous or nodal changes or with a premenstrual syndrome;
• Premenstrual syndrome: the hypostases connected with a cycle phase; morbidity of mammary glands; meteorism; disturbances of mood;
• Prolaktinoma: as conservative treatment prolaktinsekretiruyushchy micro and macroadenomas of a hypophysis; for the purpose of reduction of the size of a tumor and simplification of its removal (during the preoperative period of therapy); as postoperative therapy if the level of prolactin remains raised;
• Parkinson's disease, parkinsonism syndrome, including after encephalitis (as monotherapy, or as a part of the combined treatment with other protivoparkinsonichesky drugs).

Bromocriptinum can be shown to use for the purpose of the termination or prevention of a puerperal lactation on medical indications (a puerperal nagrubaniye of mammary glands, prevention of a lactation after abortion, the beginning lactational mastitis).

Contraindications

Contraindications to use of Bromocriptinum are:

• Chorea of Gentington;
• Arterial hypotension / hypertensia;
• Essential and family tremor;
• Severe forms of cardiovascular diseases;
• Endogenous psychoses;
• Pregnancy toxicosis;
• Hypersensitivity of the patient to ergot alkaloids.

Route of administration and dosage

The exact dosage and duration of therapy are defined individually.

Bromocriptinum is accepted inside on 1,25-2,5 mg 2-3 times a day.

The daily dose of drug can gradually raise:

• At a prolaktinoma, a men's hypogonadism, an acromegalia, benign diseases of mammary glands – to 5-20 mg;
• At parkinsonism – to 10-40 mg.

If in the course of selection of a dose side reactions are shown, it is necessary to reduce a daily dose of Bromocriptinum and within a week to support it at such level. At disappearance of collateral manifestations it is possible to increase a dose again.

Side effects

In certain cases use of Bromocriptinum can cause the following side effects:

• It is from outside central a nervous system: headaches, dizziness (as a rule, arises on the first week of therapy), psychomotor arousing, decrease in visual acuity, drowsiness, dyskinesia, hallucinations;
• From the alimentary system: diarrhea, nausea and vomiting, a lock, abnormal liver functions, feeling of dryness in a mouth;
• From cardiovascular system: arrhythmia, orthostatic hypotension; at prolonged use, in some cases – Reynaud's syndrome (especially at the patients having predisposition);
• Allergic reactions: skin rash;
• Others: spasms in gastrocnemius muscles, a nose congestion.

Special instructions

Bromocriptinum is appointed with care at simultaneous anti-hypertensive therapy, and also in the presence at the patient:

• Parkinsonism with symptoms of dementia;
• Instructions in the anamnesis on gastrointestinal bleeding;
• Digestive tract erosive cankers.

In the course of therapy it is regularly necessary to control a state and arterial pressure of the patient in view of an opportunity (especially in the first days of treatment) emergence of hypotensive reactions.

At a dose decline the majority of side effects of Bromocriptinum, as a rule, decreases. At the initial stages of treatment it is recommended to appoint an antiemetic for an hour before administration of drug.

At prolonged use of Bromocriptinum by women with a regular monthly cycle it is necessary to undergo once a year gynecologic inspection, it is desirable with a cytologic research of tissues of neck of uterus and an endometria. At approach of a menopause within half a year diagnosis is carried out every month.

Therapy by Bromocriptinum can normalize reproductive function. For prevention of undesirable pregnancy it is necessary to resort to use of reliable contraceptives (except for hormonal contraceptives).

At the adenoma of a hypophysis which is followed by significant increase in the sizes of the Turkish saddle and disturbance of fields of vision, purpose of drug is possible only in cases of inefficiency of surgical treatment and radiation therapy.

In case of purpose of Bromocriptinum at a mastopathy existence of a malignant tumor has to be completely excluded.

The combination therapy Bromocriptinum and a levodopa at Parkinson's disease and a syndrome of parkinsonism can promote increase in clinical effect that allows to lower a dose of a levodopa or to cancel it completely. It is especially important at treatment of patients at whom therapy by a levodopa does not result in stable therapeutic effect, or causes development of motor disturbances.

The patients accepting Bromocriptinum are recommended to refrain from implementation of potentially dangerous types of activity demanding speed of psychomotor reactions and the increased concentration of attention.

Analogs of Bromocriptinum are: Abergin, Parlodelum, Bromergon, Bromokriptin-KV, Bromocriptinum Richter.
