Roth's syndrome is one of the most often found options of tunnel syndromes. It is caused by a nerve compression under an inguinal sheaf or over a front upper ileal awn.
Etiologies of Roth:
The compression neuropathy can arise in connection with the following topografo-anatomic features of its arrangement.
The compression and tension of a lateral cutaneous nerve of a hip can come at the exit under an inguinal sheaf from a pelvic cavity where it does a bend, pro-butting an ileal fascia or when passing via the channel which is formed by bunches of an inguinal sheaf.
There can be a squeezing of a nerve in the tunnel which is formed by a wide fascia of a hip. At a nerve arrangement at once behind a front upper ileal awn it can be exposed to friction and be squeezed at a trunk inclination forward and at the movements in a hip joint.
The nerve compression at the level of an inguinal sheaf is the frequent reason of damage of a nerve at pregnant women. During pregnancy increase a lumbar lordosis, extension in a hip joint, a basin tilt angle that conducts to a tension of an inguinal sheaf and squeezing of a nerve in case it passes through a duplikatura of an inguinal sheaf.
Also the tension of a nerve can result from excess adjournment of hypodermic cellulose in hips and in a lower part of a front abdominal wall. In more exceptional cases there can be a squeezing of a nerve at the level of an ileal or lumbar muscle at a tumor, a retroperitoneal hematoma, operations and inflammatory diseases in an abdominal cavity, etc. Can lead wearing close underwear, hard belt or a corset to development of this neuropathy.
Most often diseases are subject middle-aged persons, and men, than women get sick three times more often. Family cases of this disease can sometimes meet.
Injury of nerves at Roth's disease
Symptoms of a disease of Roth:
At Roth's disease the feeling of numbness, cold, burning on the anterolateral surface of a hip, paresthesia like pricking and crawling of goosebumps, easy trophic disturbances of skin most often meets. More rare burning intolerable pain is felt.
The disease has the name a parestetichesky meralgiya (Greek meros - a hip). At later stages of a course of a disease there is an emergence of an analgesia in an innervation zone. Often the disease has type of the alternating lameness – developing of paresthesia and pain only during walking. Percussion and a palpation of a trunk of a nerve in the place located medialny a front upper ileal awn can cause local pain with irradiation in the field of an innervation. Introduction at the level of a neurothlipsia of local anesthetic stops painful feelings that is confirmation of the diagnosis.
Roth's disease does not inflict usually serious suffering on patients. Often there are long remissions. However in certain cases, the stated above symptom complex is the reason of heavy pains, and therefore demands surgical intervention.
Treatment of a disease of Roth:
Treatment has to be the symptomatic, directed to elimination of the main reason diseases and can include:
reflexotherapy (acupuncture, electroreflexotherapy)
analgetics
darsonvalization, hydrosulphuric, mud, radonic baths
the allowed physiotherapy
massage
special exercises
for maintenance of function of a nerve - an injection of vitamins: a thiamine bromide (B1 vitamin) intramusculary 6% solution of 25 injections on 2 ml, cyanocobalamine (B12 vitamin) intramusculary 20 injections on 200 mkg
In the cases continuing the long period the roentgenotherapy is shown.
Reflexotherapy.
The classical reflexotherapy for treatment of a disease of Roth recommends processing of the following points: E31, E32, E44, F2, RP10, RP 11, V21, V22, V23, VB31, VB37. During one session it is possible to apply no more than 4-5 points. The course of treatment makes 10-12 sessions which are held daily. In case of need the course of treatment can be repeated in 2 weeks.
