General characteristics. Structure:

Active agent: betamethasone Dipropionas - 0,640 mg (0,064%) in terms of betamethasone - 0,500 mg and Gentamycini sulfas in terms of gentamycin the basis - 1,000 mg (0,1%);

Excipients: chlorocresol, dihydrophosphate sodium monohydrate, phosphoric acid, white soft paraffin, liquid paraffin, macrogoal цетостеарат, cetostearyl alcohol [cetyl alcohol of 60%, stearyl alcohol of 40%], sodium hydroxide, water.

Pharmacological properties:

Pharmacodynamics. Белогент® - anti-inflammatory, antiallergic, antipruritic, antibacterial agent.
Betamethasone Dipropionas - a synthetic glucocorticosteroid (GKS) rendering antiinflammatory, antiallergic, antipruritic and vasoconstrictive action. When drawing on the surface of skin narrows vessels, removes an itch, reduces allocation of mediators of an inflammation (from eosinophils and mast cells), interleykin 1 and 2, gamma interferon (from lymphocytes and macrophages), brakes activity of hyaluronidase and lowers permeability of a vascular wall. Interacts with specific receptors in cell cytoplasm, stimulates synthesis of MRNK inducing formation of proteins including the lipokortin mediating cellular effects. Lipokortin oppresses A2 phospholipase, blocks release of arachidonic acid and biosynthesis of the endoperoxides, prostaglandins, leukotrienes (promoting development of an inflammation, an allergy and other pathological processes).
Gentamycin - an antibiotic widely an action spectrum of group of aminoglycosides. Has bactericidal effect and provides highly effective topical treatment of primary and consecutive bacterial infections of skin. It is active concerning gram-negative bacteria: Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli, Proteus vulgaris, Klebsiella pneumonia; and gram-positive bacteria: Streptococcus spp. (sensitive strains of ß-and a α-hemolitic streptococcus of group A), Staphylococcus aureus (the koagulazopolozhitelny, koagulazootritsatelny strains and some strains producing a penicillinase).
It is inactive concerning anaerobe bacterias, mushrooms and viruses.

Pharmacokinetics. At external use of drug in therapeutic doses transdermalny absorption of active ingredients in blood very insignificant. Use of occlusive bandages increases absorption of betamethasone and gentamycin that can lead to increase in risk of development of side effects.

Indications to use:

Dermatosis, allergic and inflammatory diseases of skin with accession of a consecutive bacterial infection or probability of its development: eczema, atopic dermatitis, neurodermatitis, contact dermatitis (including professional), allergic dermatitis, psoriasis, red flat deprive, a phlebotodermia, an anogenitalny itch.
The infected dermatitis which developed owing to stings of insects, itch, having combed, grazes, thermal damages of skin and an intertrigo.
Pyodermas of various localization (strepto-staphylodermas, impetigo, sycosis, ecthyma). Belogent® cream is applied preferential to therapy of acute and subacute stages of the inflammatory diseases of skin including which are followed by exudation.

Route of administration and doses:

Only for external use.
Белогент® external use cream is applied on an affected area with a thin layer in the quantity necessary for a covering of the struck surface, twice a day, slightly rubbing. On sites with more dense skin (for example, elbows, palms and feet), and also Belogent® can put places from which drug is easily erased to a thicket, but no more than 4 times a day. Duration of a continuous course of treatment usually makes no more than 4 weeks. For prevention of a recurrence at treatment of chronic diseases therapy it is necessary to continue still some time after disappearance of all symptoms. Within a year perhaps numerous repetition of therapy.

Features of use:

External use of the drug Belogent® on face skin as development of a rozatse, perioral dermatitis and acne is possible is not recommended long (more than 4 weeks).
Белогент® it is not necessary to apply in eyes in connection with probability of hit of drug on a mucous membrane that can promote development of a cataract, glaucoma, fungal infections of an eye and an aggravation of a herpes infection.
Some body parts, such as axillary hollows, inguinal folds where there is some kind of natural occlusion, are more subject to risk of emergence of striya therefore continuous use of cream for external use on these sites of skin has to be short (no more than 4 weeks).
In cases of development of fungal microflora on skin, additional use of antifungal means is necessary.

Influence on ability of control of vehicles and mechanisms
Data on an adverse effect of the drug Belogent® of cream for external use on ability to manage vehicles and mechanisms are absent.

Side effects:

At use of drug in therapeutic doses by-effects are, as a rule, poorly expressed. Seldom, as well as at use of other glucocorticosteroids, hypersensitivity reactions (an itch, burning, redness), irritation, a xeroderma, aknepodobny changes, hypopigmentation, striya, a skin atrophy, a folliculitis, a hypertrichosis, teleangiectasias can develop. At long uncontrolled treatment and drawing on a big surface development of the system side effects characteristic of GKS is possible.
In case of reaction of hypersensitivity or by-effects therapy it is necessary to cancel and see a doctor.

Interaction with other medicines:

Interactions of the drug Belogent® with other medicines are unknown.

Contraindications:

Hypersensitivity at patients to betamethasone, gentamycin or to any of auxiliary components of drug; viral infections of skin, skin postvaccinal reactions, open wounds, trophic ulcers, vulgar eels, rozatsea, chicken pox, skin displays of syphilis, tuberculosis cutis.

Use at pregnancy and during breastfeeding
Topical administration of the drug Belogent® of cream for external use at pregnant women and nursing mothers is allowed under control of the attending physician. In such cases use of drug has to be short and, whenever possible, be limited to small sites of integuments. Drug cannot be applied on skin of a mammary gland before feeding.

Use in pediatrics
Children can appoint Belogent® cream since 1 with care and to the shortest term. It is not necessary to use drug to babies under bandages and, especially, under the plasticized diapers as it strengthens absorption of drug and increases risk of development of side reactions.

Overdose:

At long continuous use of drug, especially at children, on the extensive surfaces of skin, drawing on skin with the broken integrity or when using under an occlusive bandage, development of the side effects connected with the increased absorption of betamethasone or gentamycin is possible. It is necessary to emphasize that the probability of development of such side effects is exclusive it is small.
Symptoms: development of system side effects of GKS.
Treatment: it is recommended to cancel drug and if necessary to carry out a symptomatic treatment.

On 15 g and 30 g in an aluminum tuba. The opening of a tuba is protected by a membrane. The tuba is closed a polyethylene lid with cutting and a punch. Each tuba together with the application instruction is placed in a cardboard pack.
