General characteristics. Structure:

Active agent: a klobetazola propionate – 0,05g.

Excipients: paraffin (paraffin soft, white), liquid paraffin, цетомакрогол, sodium phosphate monohydrate, sodium phosphate a dihydrate, butyl hydroxytoluene, methylparahydroxybenzoate, пропилпарагидроксибензоат, propylene glycol, the water purified.




Pharmacological properties:

Pharmacodynamics. Glyukokorstikosteroid for external use. Prevents regional accumulation of neutrophils, reduces inflammatory exudation and products of lymphokines, slows down migration of macrophages, reduces intensity of processes of infiltration and granulation, renders local antiinflammatory, antipruritic, antiallergic and anti-exudative effects.

Pharmacokinetics. Average maximum concentration of a klobetazol of propionate is reached in plasma in 13 h after the first application and in 8 hours after repeated drawing on healthy skin of 30 g of a klobetazol of propionate in the form of 0,05% of ointment and makes 0.63 ng/ml. In 10 hours after application of the second dose (30 g) of a klobetazol of propionate in the form of cream its average maximum concentration in plasma slightly exceeds that ointments. In 3 hours after single use of 25 g of 0.05% of ointment of a klobetazol of propionate for patients with psoriasis and eczema average maximum concentration of drug in plasma makes 2.3 ng/ml and 4.6 ng/ml respectively.
System metabolism of a klobetazol of propionate after transdermal absorption was not studied.


Indications to use:

- psoriasis;
- eczema (various forms);
- red flat deprive;
- diskoidny lupus erythematosus;
- the skin diseases steady against therapy of less active glucocorticosteroids for topical administration.


Route of administration and doses:

Cream is applied with a thin layer on the affected skin one – twice a day before obtaining effect. Treatment it is impossible to spend more than 4 weeks without assessment of need of its continuation. For treatment of exacerbations of skin diseases it is possible to conduct repeated short courses Pauerkort. If therapy continuation - glucocorticosteroids is necessary, it is necessary to use less active drugs.
In especially resistant cases, especially in the presence of a hyperkeratosis, antiinflammatory effect of cream Pauerkort can be strengthened by means of imposing for the night on a bandage drug scope from a polyethylene film that usually is followed by positive effect. Further the reached effect manages to be supported without use of an occlusive bandage.


Features of use:

It is necessary to avoid purpose of glucocorticosteroids for topical administration for an appreciable length of time, especially at treatment of children of younger age as at the same time there can be a suppression of function of adrenal glands.
Pauerkort's use for children demands observation of the doctor at least once a week.
On a face more often than on other body parts, atrophic changes of skin as a result of prolonged use of local glucocorticosteroids can appear.
When putting cream on eyelids it is necessary to watch that drug did not get into eyes because it can cause glaucoma.
Treatment of psoriasis glucocorticosteroids for topical administration can be followed by development of a recurrence of a disease, tolerance to drug, a generalized pustular form of a disease, local or system toxic reactions because of disturbance of barrier function of skin therefore attentive observation of the patient is especially important.
At accession of consecutive infection it is necessary to carry out the corresponding antibacterial therapy. At any signs of generalization of an infection it is necessary to stop topical administration of glucocorticosteroids and to carry out the corresponding treatment by antibacterial drugs.
The warm wet conditions created when imposing an occlusive bandage promote developing of a bacterial infection therefore it is necessary to clear surely skin before imposing of a new bandage. Cream is not recommended to be dissolved.


Influence on ability to manage cars/mechanisms
It is improbable that the klobetazola propionate influences this ability.


Side effects:

Long purpose of glucocorticosteroids for topical administration in high doses or drawing them on extensive sites of skin can be followed by system absorption with the subsequent development of symptoms of a hypercorticoidism. The risk of development of this complication is increased at children, especially at use of an occlusive bandage, at the same time the diaper can carry out a role of such bandage.
On condition of Pauerkort's appointment to adult patients in a dosage no more than 50 g a week, suppression of function of a hypophysis and adrenal glands has tranzitorny character with their bystry recovery right after the termination of a course of treatment.
Prolonged treatment by highly active glucocorticosteroids for topical administration in high doses can cause atrophic changes of skin, in particular, its thinning, emergence of atrophic strips (striya), dilatation of superficial blood vessels, especially when imposing occlusive bandages and when putting drug on sites of skin with folds. The xanthopathy and a hypertrichosis can develop. In rare instances treatment of psoriasis glucocorticosteroids for topical administration (or their cancellation) provokes development of pustular psoriasis.
Pauerkort can sometimes cause an aggravation of symptoms of a disease.


Interaction with other medicines:

There are no data.


Contraindications:

- pink eels;
- vulgar eels;
- carcinoma cutaneum;
- nodal prurigo of the Guide;
- perioral dermatitis;
- perianal and genital itch;
- bacterial, viral and fungus diseases of skin (herpes simplex, chicken pox, tuberculosis cutis, actinomycosis and др);
- widespread blyashechny and pustular psoriasis;
- hypersensitivity to active and to excipients of drug;
- age till 1 year;
- lactation period.

Pregnancy and lactation
Glucocorticosteroids it is not necessary to apply to topical administration at pregnancy a long time in high doses.
Safety of use of a klobetazol of propionate in the period of a lactation is not established.


Overdose:

Development of acute overdose is improbable. However, in case of chronic overdose or the wrong use hypercorticoidism symptoms can develop that demands gradual drug withdrawal under observation of the doctor.
