Provirsan

General characteristics. Structure:

Active agents: acyclovir of 200 mg;

Other components: lactoses monohydrate, cellulose microcrystallic, carboxymethylstarch sodium salt, коповидон, magnesium stearate, polysorbate 80.

Pharmacological properties:

Pharmacodynamics. Provirsan - antiviral drug which is a synthetic nukleozidny analog of a deoksiguanozin with the selection action on herpes viruses. Drug selectively concentrates in the cell infected with a virus, interacts with a virus thymidinekinase and is consistently phosphorylated in ацикловиртрифосфат. Atsiklovirtrifosfat blocks synthesis of virus DNA - a polymerase, joins instead of a deoksiguanozin in DNA of viruses and suppresses replication. Provirsan effective for treatment and prevention of the infections caused by a virus of herpes (Herpes simplex) І and the II types, viruses that cause chicken pox (Varicella), and shingles (Herpes zoster).

Pharmacokinetics. Provirsan's absorption at reception through a mouth of 15-30%. The maximum concentration in a blood plasma is observed in 1, 5-2 hours. In blood an insignificant part of drug (9-33%) communicates proteins of plasma. Period of half removal 2-3 hours. Strong permeability in all bodies and fabrics, including skin and a brain is characteristic of drug (in cerebrospinal fluid - 50% of plasmatic concentration of an acyclovir). Concentration of drug in kidneys is ten times higher than at a blood plasma. The acyclovir gets through a placental barrier. Collects in maternal milk.
Therapeutic range of concentration of Provirsan in plasma makes 2, 0-0, 5mg/l.
The most part of a dose of Provirsan is removed by kidneys by glomerular filtering and canalicular secretion in steady-state condition (84%) and in the form of a metabolite 9-karboksimetoksimetilguanin (14%) which is not pharmacological active. About 2% of drug are removed with a stake. Against the background of a chronic renal failure the elimination half-life increases till 20 o'clock.

Indications to use:

The acyclovir is used for treatment and prevention of primary and recurrent herpetic damages of skin and mucous membranes (including genital herpes), herpetic defeats of patients with an immunodeficiency (AIDS, a state after transplantation of marrow), shingles, chicken pox, herpetic to a keratitis.

Route of administration and doses:

It is necessary to appoint treatment as soon as possible, that is right after emergence of the first symptoms of a disease. Establishes a dose and frequency rate of reception and the doctor individually adjusts. At infection with the Herpes simplex virus with the adult and to children aged from 2 years appoint 200 mg of 5 times a day in 4 hours with constant cancellation of one night dose within 5 days. At a disease Provirsan appoint 800 mg (4 tablets) of 5 times a day within 7-10 days to shingles of adults. At a disease to chicken pox drug is appointed by adult 800 mg (4 tablets) 4 times a day every 6 hours with constant cancellation of one night dose within 7-10 days. To children age of 2 years the dose makes 20 mg/kg 4 times a day. To children with body weight more than 40 kg appoint such dose, as well as the adult.

For prevention appoint 200 mg 4 times a day every 6 hours. At patients with a renal failure the dose can be lowered. Drug is accepted at reception of food, or right after reception of food, washing down with enough liquid.

Features of use:

At patients with a renal failure the dose can be lowered to 1 tablet (200 mg) twice a day with 12-hour intervals.
Safety and efficiency of an acyclovir at children are not revealed by age up to 2 years.
If necessary drug use Provirsan women who nurse need to stop chest feeding for treatment. At treatment of a herpes infection of generative organs it is worth abstaining from sexual contacts.
At reception of high doses of drug it is recommended to use a large amount of liquid.

Side effects:

Usually drug is well transferred by the patient. Sometimes symptoms of transitional character take place: skin rashes, pain in a stomach, nausea, vomiting, a diarrhea, trembling, a condition of confusion, dizziness, drowsiness, fatigue, it is very rare - hallucinations.

Interaction with other medicines:

Probenetsid prolongs the period of elimination of Provirsan. Simultaneous use with an acyclovir of nephrotoxic drugs increases risk of damage of function of kidneys.

Contraindications:

Hypersensitivity to an acyclovir or other component of drug. During pregnancy the acyclovir is appointed in a case existence of especially significant reasons. During chest feeding drug cannot be accepted. Children's age up to 2 years.

Overdose:

Cases overdose during oral administration of an acyclovir are not registered. At emergence of signs overdose the symptomatic treatment is necessary.

Storage conditions:

To store in the place, dry, dark, unavailable to children, at a temperature of 15-25 °C.
