General characteristics. Structure:

Active ingredient: 5 g of pitch guar in 1 package.

Excipients: no.

Medicine for control and a degrowth of a body. Reduces feeling of hunger, accelerates saturation that leads to reduction of amount of the consumed food and a body degrowth. As GUAREM is cellulose, it has additional properties (normalization of a chair, intestinal microflora, binding of toxins).




Pharmacological properties:

Pharmacodynamics. Pitch guar is food cellulose which is received from an endosperm of seeds of Cyamopsis tetragonolobus. Pitch guar is the polysaccharide forming viscous gel with water.

Pitch guar considerably prolongs gastric emptying, and also absorption of carbohydrates in a small intestine. Thus, it reduces a hyperglycemia and concentration of insulin. At prolonged use pitch guar reduces the initial level of glucose in blood approximately by 1−2 mmol.

It is considered that the effect of decrease in level of cholesterol in serum is caused by the lowered cholesterol absorption, hypersecretion of bile acids, and also the increased metabolism of cholesterol in a liver. Thus, the effect of pitch guar is similar to effect of the pitches connecting bile acids. At monotherapy by pitch guar levels of the general cholesterol and lipoproteids of low density in serum approximately decrease by 10−15%.

Pharmacokinetics. Pitch guar is not absorbed from a digestive tract, partially decays bacteria in a digestive tract and approximately increases the volume of fecal masses by 20%.


Indications to use:

Excess weight.

The diabetes mellitus of adults at which obesity is observed and the carried-out dietotherapy does not yield desirable results.

Diabetes mellitus of adults at which primary reception of peroral glucose-lowering drugs did not allow to reach sufficient therapeutic effect and in that case when transition to therapy by a combination sulfanylurea-guanyl guanidine derivatives is not possible.

Badly controlled diabetes mellitus of adults at which, despite the maximum peroral therapy glucose-lowering drugs, do not reach even the minimum effect and when transfer into an insulin therapy is undesirable.

Juvenile diabetes mellitus and diabetes mellitus of adults when it is necessary to prevent very high hyperglycemia after meal.

Hypercholesterolemia at patients, cholesterol level at whom does not decrease to required, despite a dietotherapy.


Route of administration and doses:

Adults use drug inside in the following doses:
- excess weight: on 1 bag (5 g) or on 1 measured spoon of granules (5 g) 3 times a day during food;
- diabetes mellitus: on 1 bag (5 g) or on 1 measured spoon of granules (5 g) 3 times a day during food;
- hypercholesterolemia: on 1 bag (5 g) or on 1 measured spoon of granules (5 g) of 2−5 times a day during food.

With pitch guar 2,5 g (a half of a bag or a half of a measured spoon) as thus it is possible to avoid the undesirable effects from digestive tract caused by drug are preferable to begin therapy with a low dose, for example.

Guar, granules, it is always necessary to accept or having mixed them with food, or with suitable liquid (water, juice, milk), together with food. It is necessary to swallow of mix immediately. Dry granules Guar, per se, it is not necessary to use. On one dose of granules it is necessary to drink not less than one glass of liquid.


Features of use:

Patients with dysfunctions of a gullet or digestive tract in the anamnesis have an increased risk of development of impassability of digestive tract. At the beginning of therapy by pitch guar at the patients having diabetes and accepting insulin or sulphonylurea the increased tendency to a hypoglycemia can be observed.

Use at pregnancy and during feeding by a breast. As pitch guar is not absorbed, there are no obstacles for its inclusion in time of pregnancy and during feeding by a breast.

Children. Data on use of drug for children are absent. Influence on ability to manage motor transport and to work with mechanisms.

Pitch guar, per se, does not influence ability to manage motor transport or to work with mechanisms. However the hypoglycemia caused by pitch guar can negatively affect ability to manage motor transport or to work with mechanisms, and also on performance of the tasks requiring attention. Drug does not influence ability to manage motor transport or to work with mechanisms at patients with good control of glucose level.


Side effects:

Side effects of pitch guar preferential easy and short-term. Mainly, they are connected with a digestive tract. The most widespread is the meteorism which arises approximately at 50% of patients. Besides, there can be an abdominal pain, nausea or diarrhea. Impassability of a gullet or small intestine arise very seldom, approximately in 50% of such cases at patients dysfunctions of a gullet or intestines in the anamnesis were observed. Inhalation of dust of pitch guar can cause allergic rhinitis or an apnoea during sleep.


Interaction with other medicines:

In addition to nutrients, pitch guar can prolong absorption in digestive tract of some other drugs. However essential changes of bioavailability of other drugs do not happen. Ability of pitch guar was noted to prolong absorption of such drugs as paracetamol, digoxin and буметадин. It is considered that such interaction has the small clinical importance though there are messages that pitch guar reduces bioavailability of phenoxymethylpenicillin, Metforminum, Glibenclamidum and oral contraceptives. Therefore, it is necessary to avoid a concomitant use of pitch guar with these drugs.


Contraindications:

Hypersensitivity to pitch guar or to any of drug excipients. The patients suffering from narrowing or impassability of digestive tract should avoid use of pitch guar.


Overdose:

About cases of overdose of pitch guar it was not reported. Possibly, high doses can affect a condition of digestive tract therefore it is necessary to consider risk of emergence of impassability of intestines, mainly.


Storage conditions:

To store at the room temperature (15–25 °C) in the place, dry, unavailable to children.
