The dermatophytosis known also under the name of a dermatofitiya — a persistent fungal infection with damage of skin and its derivatives (nails, hair).
Disease severity varies depending on localization of an infection and a type of a fungus. Infectious defeat of feet of stop of the athlete (tinea pedis) is followed by emergence of cracks of skin folds of the I finger of foot, a peeling of bottom surfaces, emergence of bubble rash around the I finger and on edge of soles. Interdigital defeats are characterized by an itch or, in case of bacterial superinfection, morbidity. Damage of palms meets less often and is similar to defeat of feet. The dermatophytosis of a pilar part of the head (tinea capitis) is characterized by emergence of sites of an alopecia and peeling. In cases of a so-called endotrikhozny infection of hair breaks in the field of a root with education on a pilar part of the head of the sites speckled by black points. At some forms of defeat of a pilar part of the head the expressed suppuration called keriony is observed. The dermatophytosis of smooth skin (tinea corporis) is characterized by limited defeats of extremely various form. Sites of a peeling, vesicle, pustule can meet. Inflammatory process can be minimum on expressiveness or intensive. On sites with smaller inflammatory changes observe the centers of the central healing. The dermatophytosis of mental area is characterized by a pustular folliculitis (tinea barbae). At an onychomycosis (tinea unguium) nails take the whitish, decoloured form, become thinner, crumble, under edge of a nail plate — a horn detritis.
Carry the following types of fungi to dermatophytes: Trichophyton, Microsporium and Epidermophyton. Strike the keratosic fabrics. For other mycoses, such as candidiasis, multi-colored deprive also a chernotochechny trichophytosis, the invasion of the structures containing a keratin is also characteristic, but by tradition they are not called dermatophytoses.
Treatment of the Dermatophytosis:
If defeat easy and moderate severity, and affected areas are localized on a trunk, in the field of a scrotum and on the lower extremities, reasonablly topical administration of the imidazole or any drug which was already mentioned in this section. Such treatment is less effective in case of damages of palms and a foot with the expressed hyperkeratosis. Patients with the dermatomycoses of a moderate and heavy current which are not giving in to local influence and also with defeat of a pilar part of the head, nails or mental area are subject to system therapy. Drug of the choice is the griseofulvin appointed in a dose of 500 mg of a microcrystallic form or 375 mg of an ultra microcrystallic form in day in 1 — 2 intake during food. For treatment of patients with refractory forms of mycosis the dose doubles. Treatment continues before total disappearance of the infected corneous layer. Removal of the infected indumentum, epilation of nails and sanitation of interdigital intervals accelerate recovery. At a consecutive bacterial infection of feet antibiotics and the drying means are recommended. A recurrence of an infection of feet, caused by dermatophytes, it is possible to prevent, watching purity and dryness of integuments. In cases of the infection caused by a fungus form, resistant to griseofulvin, appoint кетоконазол, orally, in a dose 200 — 400 mg a day.
