Gentamycin

General characteristics. Structure:

Active ingredient: 3 mg of gentamycin (Gentamycini sulfas) in terms of gentamycin basis.

Excipients: dinatrium эдетат (Disodium salt ethylene diamine tetraacetic acid), sodium chloride, Polyglucinum - dextran solution with a molecular weight from 50000 to 70000 and chloride sodium, water for injections.

Pharmacological properties:

Pharmacodynamics. A bactericidal antibiotic of a broad spectrum of activity from group of aminoglycosides. Communicates with 30S in subunit of ribosomes and breaks protein synthesis, interfering with formation of a complex of transport and information RNA, at the same time there is a wrong reading of RNA and formation of nonfunctional proteins; polyribosomes are split and lose ability to synthesize protein (бактериостаз). Unlike other antibiotics aminoglycosides possess bactericidal action - in big concentration reduce barrier functions of cytoplasmic membranes and cause death of microorganisms.

It is effective concerning Staphylococcus spp. (koagulazopolozhitelny and koagulazootritsa-telny), Pseudomonas aeruginosa, Proteus spp. (including indolpositive and Indo-лотрицательных strains), Escherichia coli, Klebsiella pneumoniae, Haemophilus influenzae, Haemophilus aegyptius, Enterobacter aerogenes (Aerobacter aerogenes), Moraxella lacunata, Neisseria spp. (including Neisseria gonorrhoeae).

Pharmacokinetics. At topical administration it is soaked up in insignificant quantities.

Indications to use:

Drug is used for treatment of bacterial infections of eyes (the blepharitis, blefarokon-yunktivit, conjunctivitis, a keratoconjunctivitis, a keratitis, a dacryocystitis, meybomiit). Prevention of infectious complications after injuries and eye operations (for this purpose drug is appointed before and after an operative measure).

Route of administration and doses:

Locally. Dig in in a conjunctival sac of the affected eye. At infectious damages of moderately severe eyes appoint on 1 drop every the 4th hour; at severe defeats - on 1 drop each 1 hour. Duration of use is no more than 14 days.

Features of use:

Pregnancy and lactation. Pregnant women should appoint drug only if the estimated advantage exceeds risk of development of by-effects in a fruit. Use of drug in the period of a lactation is possible according to the recommendation of the doctor.

Before use of drug it is necessary to take out soft contact lenses. They can be put repeatedly not earlier than in 30 minutes after a drug instillation.

Against the background of treatment insensitive microorganisms can develop. In similar cases it is necessary to cancel treatment and to begin performing the corresponding therapy.

Side effects:

After an instillation of drug dacryagogue, reddening of eyes, pain, burning, a photophobia, a vision disorder within several minutes after instillation is possible.

Development of allergic reactions is possible.

At prolonged use developing of a fungal infection of eyes or development of the bacteria steady against gentamycin is possible.

Interaction with other medicines:

Combined use with erythromycin and chloramphenicol owing to pharmaceutical incompatibility is not recommended.

In case of a heavy bacterial infection of eyes topical treatment needs to be added with the general use of antibiotics, however it is not necessary to combine use of eye drops Gentamycin with other antibiotics rendering from - and nephrotoxic action.

Contraindications:

Gentamycin is contraindicated at hypersensitivity to all components of drug.

With care. At heavy renal failures, neuritis of an acoustical nerve.

Overdose:

There are no data concerning overdose of gentamycin at its topical administration.
