Oksamp-natry

General characteristics. Structure:

Description: the powder for preparation of solution for introduction in oil (or porous weight) color, white with a yellowish shade, forming transparent, almost colourless solution at dissolution.

Active ingredient:  1 фл. ampicillin of sodium of 333.33 mg; Oxacillinum of sodium of 166.67 mg;

Pharmacological properties:

The combined antibacterial drug.

Oxacillinum - an antibiotic from group of semi-synthetic penicillin. Works bakteritsidno, suppressing synthesis of a cell wall of bacteria. It is steady against action of a penicillinase. It is active concerning gram-positive bacteria: Staphylococcus spp. (including producing a penicillinase), Streptococcus spp. (including Streptococcus pneumoniae), Corynebacterium diphtheriae, Bacillus anthracis; gram-negative bacteria: Neisseria gonorrhoeae, Neisseria meningitidis; anaerobic bacteria: spore-forming sticks, Actinomyces spp. The majority of gram-negative bacteria, rickettsiae, viruses, protozoa, mushrooms are steady against Oxacillinum.

It is active concerning gram-positive bacteria: Staphylococcus spp. (except for the strains producing a penicillinase), Streptococcus spp. (including Enterococcus spp.), asporous bacteria (including Listeria monocytogenes); gram-negative aerobic bacteria: Neisseria gonorrhoeae, Neisseria meningitidis, Escherichia coli, Shigella spp., Salmonella spp., Bordetella pertussis, some strains of Haemophilus influenzae.

Ampicillin collapses under the influence of a penicillinase.

The ampicillin combination to Oxacillinum is rational since the last, oppressing activity of a penicillinase, reduces thereby ampicillin destruction. The action spectrum at such combination becomes wider.

 

Pharmacokinetics. Absorption

Cmax of active agents is reached in 0.5-1 h after introduction in oil. At in introduction to blood the high concentration exceeding those at introduction in oil are quickly created.

Distribution and removal

At repeated introductions active agents do not collect in an organism that gives the chance to use drug is long. Are removed with urine.

Indications to use:

— respiratory infections (including bronchitis, pneumonia, bronchoectatic disease);

— diseases of biliary tract (including cholangitis, cholecystitis);

— infections of urinary tract and kidneys (including pyelitis, pyelonephritis, cystitis);

— infections of skin and soft tissues (including contaminated wounds);

— burn disease;

— quinsy.

Prevention of postoperative complications at surgical interventions.

Prevention and treatment of infections at newborns.

Route of administration and doses:

Intramusculary and intravenously (struyno or kapelno). 

The drug doses provided below are the sum of dosages of ampicillin and Oxacillinum (sodium salts) in their fixed ratio 2:1 (i.e. in the dosages of 0,5 g of drug provided below it is equal to the sum of 333,5 mg of ampicillin + 166,5 mg of Oxacillinum).

The daily dose for adults and children is more senior than 14 years - 3-6 g; for newborns, premature and children till 1 year - 100-200 mg/kg/days; 1-6 years - 100 mg/kg/days; 7-14 years - 50 mg/kg/days. The daily dose is entered into 3-4 introductions, with an interval of 6-8 h. If necessary the specified doses can be increased by 1,5-2 times.

Duration of treatment depends on disease severity (from 5-10 days to 2-3 weeks, and at chronic processes - within several months).

For intramuscular introduction bottle contents (133,4 mg + 66,6 mg or 333,5 mg + 166,5 mg) are dissolved, respectively, in 2 ml or 5 ml of water for injections.

For intravenous jet administration (within 2-3 min.) the single dose is dissolved in 10-15 ml of water for injections or 0,9% of solution of sodium of chloride.

For intravenous drop administration by the adult a single dose dissolve 0,9% of solution of sodium of chloride or 5% of solution of a dextrose (glucose) in 100-200 ml and enter with a speed of 60-80 of caps/mines; to children as solvent use 30-100 ml of 5-10% of solution of a dextrose (glucose). Intravenously the drug is administered by 5-7 days, with the subsequent transition to intramuscular introduction.

Solutions use right after preparation.

Features of use:

At emergence of allergic reactions it is necessary to stop administration of drug and to carry out the desensibilizing therapy.

At development of an acute anaphylaxis introduction of Epinephrinum, GKS (a hydrocortisone or Prednisolonum), bronchial spasmolytics, blockers histamine H1 receptors is shown; if necessary carry out IVL.

Use in pediatrics

With care children should appoint drug if in the anamnesis of mother there is an instruction on hypersensitivity to penicillin. In such cases in need of purpose of the drug Oksamp-natry in the presence of associated diseases (bronchial asthma, hay fever and other allergic diseases) simultaneous use of the desensibilizing drugs is shown.

Side effects:

Allergic reactions: skin rash, small tortoiseshell, Quincke's edema; seldom – an acute anaphylaxis.

Local reactions: at in introduction development of phlebitis and periphlebites is possible; at introduction in oil – morbidity on site injections.

Interaction with other medicines:

Synergism of antibacterial action at a drug combination with bactericidal antibiotics is observed - aminoglycosides, cephalosporins, Vancomycinum, rifampicin.

Contraindications:

— hypersensitivity to components of drug and other antibiotics from group of penicillin.

 

Use of the drug OKSAMP®-NATRY at pregnancy and feeding by a breast
Use of the drug Oksamp-natry at pregnancy according to indications is possible. It is allocated with breast milk in low concentration.

Overdose:

Now about cases of overdose of the drug Oksamp-natry it was not reported.

Storage conditions:

List B. Drug should be stored in the dry, protected from light place, at the room temperature. A period of validity - 2 years.
