Analginum

General characteristics. Structure:

Active agent - metamizole sodium of 500 mg.
Excipient: starch, talc, calcium stearate, aerosil.

Pharmacological properties:

Pharmacokinetics. After intake metamizole sodium is quickly hydrolyzed in digestive tract with formation of an active metabolite of 4-methyl-amino-antipyrine which after absorption is metabolized in 4-formyl-amino-antipyrine and other metabolites.

Metabolites of metamizole sodium do not contact proteins of a blood plasma. The greatest part of a dose is removed with urine in the form of metabolites. Metabolites are allocated with breast milk.

Pharmacodynamics. Analgetic antipyretic. Is derivative pyrazyl ketone. Possesses the expressed analgetic, antiinflammatory and febrifugal action which mechanism is connected with oppression of synthesis of prostaglandins.

Indications to use:

Pains of various origin (headache, tooth, posttraumatic, burn, postoperative, альгодисменорея, neuralgia, mialgiya, renal and hepatic colic).

Route of administration and doses:

Drug is accepted after food on 250 – 500 mg by 2 - 3 times a day.

The highest doses for adults: one-time - 1 g, daily - 3 g.

To children drug is appointed inside on 5 - 10 mg/kg of body weight by 3-4 times a day.

Drug is intended for short-term use.

Features of use:

At prolonged use it is necessary to control a pattern of peripheral blood.

At use of analginum coloring of urine in red color is possible.

Considering side effects of medicine, it is necessary to be careful when driving or potentially dangerous mechanisms.

Side effects:

- nausea, vomiting, anorexia;

- pains in epigastric area, a meteorism, a lock, diarrhea;

- headache, dizziness, excitement, irritability.

Seldom:

- paresthesias, sonitus, vision disorder, tremor, depression;

- skin rash, Quincke's edema, acute anaphylaxis;

- a leukopenia, an agranulocytosis, anemia, thrombocytopenia (at long 

use).

Interaction with other medicines:

Simultaneous use of analginum with other analgetics antipyretics, non-steroidal anti-inflammatory drugs can lead to mutual strengthening of toxic effects.

At simultaneous use with inductors of microsomal enzymes of a liver reduction of efficiency of analginum is possible.

At simultaneous use activity of indirect anticoagulants, peroral gipoklikemichesky drugs, glucocorticosteroids, indometacin amplifies.

At simultaneous use with proizvody the fenotiazina is possible the expressed hyperthermia; with sedatives, anxiolytics - analgeziruyushchy effect of analginum amplifies; with tricyclic antidepressants, oral contraceptives, Allopyrinolum - are broken metabolism of analginum and its toxicity increases; with caffeine – effect of analginum amplifies; with cyclosporine – concentration of cyclosporine in a blood plasma decreases.

At use of analginum in a combination from a pitofenon mutual strengthening of their pharmacological action happens a hydrochloride (has direct myotropic effect on smooth muscles of internals and causes its relaxation) and with a fenpiveriniya bromide (m – a holinoblokator) that is followed by reduction of a pain syndrome, relaxation of unstriated muscles and decrease in the increased body temperature.

Barbiturates, codeine, анаприлин, blockers of H2-histamine receptors strengthen effect of analginum; the last increases hypoglycemic effect of peroral antidiabetic drugs.

Contraindications:

- the expressed disturbances of functions of a liver and kidneys;
 
- deficit glyukozo-6-fosfatdegidrogenazy;
 
- hemopoiesis disturbance;
 
- hypersensitivity to pyrazyl ketone derivatives;
 
- children's age up to 6 years;
 
- existence in the anamnesis of a peptic ulcer of a stomach and duodenum;
 
- bronchospasm;
 
- pregnancy, lactation period.

Overdose:

Symptoms - strengthening of side effects of drug.

Treatment – a gastric lavage, purpose of salt laxatives, absorbent carbon and carrying out an artificial diuresis, blood alkalization, the symptomatic therapy directed to maintenance of the vital functions.

Storage conditions:

To store in the dry, protected from light place, at a temperature from 2 to 30 °C. To store in the place, unavailable to children!
