General characteristics. Structure:

International and chemical names: Thiamazolum; 1-methyl-2-mercaptoimidazole.
main physical and chemical properties: tablets of color, white or white with a yellowish shade, with a flat surface, with a facet; structure: mercazolil - 0,005 g; excipients: sugar, potato starch, calcium stearate, talc.

Pharmacological properties:

The mechanism of effect of Mercazolil is caused by braking of synthesis of thyroxine in a thyroid gland that leads to normalization of metabolic processes in it at hyperfunction. Drug lowers standard metabolism. The effect of Mercazolil is shown in acceleration of elimination from a thyroid gland of iodides and oppression of activity of the fermental systems oxidizing iodides to iodine. Braking of iodination of thyreoglobulin and transformation of diiodotyrosine into thyroxine is as a result observed.

Indications to use:

Mercazolil is appointed at a diffusion toxic craw of easy, average and heavy forms, training of patients for operation for a thyrotoxicosis.

Route of administration and doses:

Mercazolil is appointed inside after food. At a slight and average thyrotoxicosis adults accept drug on 0,005 g 3-4 times a day, at a severe form - on 0,01 g 3-4 times a day. After remission approach (usually in 3-6 weeks) the daily dose is lowered every 5-10 days on 0,005-0,01 g, gradually selecting the minimum maintenance doses: 0,005 g once a day; 0,005 g every other day; 0,005 g of 1 times in 3 days. Accept before obtaining lasting effect. At a thyrotoxicosis recurrence the drug dose is increased again, adhering to the specified scheme of therapy. Duration of reception is caused by a form and disease severity, the reached effect. The highest doses for adults inside: one-time - 0,01 g, daily - 0,04 g. At a thyrotoxicosis at children the initial dose makes 0,4 mg/kg of body weight a day disposable, or is divided into two receptions; the maintenance dose – 0,2 mg/kg of body weight of the child a day disposable, or is divided into two receptions.

Features of use:

At the early termination of treatment by Mercazolil the disease recurrence is possible.
There are data on expediency of a combination of Mercazolil from lithium a carbonate.
Mercazolil needs to be applied under observation of the doctor and once a week to investigate a condition of peripheral blood. At a preparation for surgery, as a result of drug use, strengthening of bleeding of a thyroid gland is not excluded. In order to avoid that (at achievement of remission or considerable improvement of a state) Mercazolil is cancelled and appoint iodine drugs. Operation is performed in 2-3 weeks.

Side effects:

Agranulocytosis, leukopenia, thrombocytopenia, disturbances from digestive tract, damages of a liver, dizziness, hypostases, skin rash, paresthesias, a joint pain, baldness, a vision disorder, a medicamentous hypothyroidism, goitrogenic effect. At emergence collateral action the dose of drug is reduced or the last is cancelled.

Interaction with other medicines:

Mercazolil cannot be combined with the drugs causing a leukopenia: pyramidon and its analogs, streptocides.

Contraindications:

Pregnancy, the feeding period a breast, the expressed leukopenia and a granulocytopenia, nodal forms of a craw (an exception - the heavy, progressing current temporarily postponing operation).

Overdose:

Clinical symptoms. Changes of a menstrual cycle, xeroderma, drowsiness, headache, muscular weakness. The specific antidote is unknown.

Storage conditions:

To store in the dry, protected from light place.
Period of validity – 5 years.
