Nitsergolin

General characteristics. Structure:

Active agent: ницерголин 10 mg.

Pharmacological properties:

Alpha adrenoblocker. Improves cerebral circulation, activates metabolism of a brain. Reduces brain vascular resistance, increases an arterial blood stream and consumption of oxygen and glucose brain tissues. Reduces resistance of pulmonary vessels. Increases blood-groove speed in vessels of upper and lower extremities, especially at blood circulation disturbance. Reduces aggregation of thrombocytes and improves haemo rheological indicators. With arterial hypertension can cause gradual decrease in the ABP in patients.

Pharmacokinetics. After intake ницерголин quickly and almost completely (90-100%) it is absorbed from a GIT. Cmax of a nitsergolin in a blood plasma is reached in 1-1.5 h.
It is metabolized by hydrolysis, dimethylation and a glyukuronization. 3 metabolites of a nitsergolin are known, is basic of which 6-метил-8β-оксиметил-10α-метоксиэрголин.
70-80% of a nitsergolin and its metabolites are removed by kidneys during 70-100 h after intake. 20% of a nitsergolin are removed with a stake.
The elimination half-life of a nitsergolin at healthy volunteers makes 2.5 h, the main metabolite - 12-17 h.

Indications to use:

Acute and chronic insufficiency of cerebral circulation (including atherosclerosis of vessels of a brain, dynamic disturbances of cerebral circulation, fibrinferment and a vascular embolism of a brain), migraine, obliterating diseases of vessels of extremities, a Raynaud's disease, arterial hypertension and hypertensive crisis (as supportive application).

Route of administration and doses:

Establish individually. Inside - on 5-10 mg of 3 times/days, a course of treatment - 2-3 months and more.
In oil - on 2-4 mg of 2 times/days. The single dose at in drop introduction makes 4-8 mg, at in / and - 4 mg.
In certain cases in an initiation of treatment enter in oil or in/in, then - inside as a maintenance therapy.

Features of use:

More than 2 mg/dl are required to patients with a renal failure and the patient of advanced age at the level of serumal creatinine correction of the mode of dosing.

After in oil or in/in introductions of a nitsergolin the patient needs to be in horizontal position within several minutes.

At long therapy of patients it is necessary to inspect at least, than 1 time in 6 months, for the purpose of a possible dose decline or cancellation of a nitsergolin.

Side effects:

From cardiovascular system: orthostatic hypotension and dizziness (especially later in oil or in/in introductions), faints, erubescence of the person.
From TsNS: sleep disorders, excitement, drowsiness, sleeplessness, concern, sweating strengthening.
From the alimentary system: nausea, diarrhea, increase in acidity of a gastric juice, abdominal pain, loss of appetite.
Allergic reactions: erythema, small tortoiseshell.

Interaction with other medicines:

At simultaneous use with anti-hypertensive means anti-hypertensive action amplifies.
At simultaneous use effect of indirect anticoagulants and antiagregant amplifies.

Contraindications:

Acute bleeding, recently postponed myocardial infarction, arterial hypotension, the expressed bradycardia, concomitant use alpha and beta adrenoblockers, pregnancy, a lactation, hypersensitivity to a nitsergolin.

Use at pregnancy and feeding by a breast
Nitsergolin is contraindicated to use at pregnancy and in the period of a lactation (breastfeeding).

Use at renal failures
More than 2 mg/dl are required to patients with a renal failure at the level of serumal creatinine correction of the mode of dosing.

Use for elderly patients
More than 2 mg/dl are required to patients of advanced age at the level of serumal creatinine correction of the mode of dosing.

Overdose:

It is not described.

Storage conditions:

Nitsergolin is stored at the room temperature. The place of storage has to be dry and unavailable to children.
