Pharm group: Plasma substituting and perfused solutions

Release form: Liquid dosage forms. Solution for intra coronary perfusions.

Indications to use: Cardioplegia.

General characteristics. Structure:

Active ingredients: 1,19 g of potassium of chloride, 0,51 g of calcium of a gluconate, 3,94 g of magnesium of sulfate of heptahydrate, 0,84 g of Natrii hydrocarbonas, 0,27 g of lidocaine of a hydrochloride, 0,268 g of inosine.

Excipient: solution of Polyglucinum of 6% for инфузмй.

The combined drug for coronary perfusions.

Pharmacological properties:

Pharmacodynamics. Perfusion of heart polyionic buffered solution Konsol causes the termination of mechanical and electric activity of heart and prevents damage of cells from total ischemia and the subsequent reperfusion at recovery of a cordial blood-groove. Polyglucinum which is a part of solution, with its ability to connect water and to improve microcirculation, prevents sharp swelling of mitochondrions and intracellular hypostasis. The used concentration of cardioplegic agents cause reversible stabilization of excitable membranes of cardiomyocytes and other cells that does not lead to their irreversible damages to a vrezultata of long total ischemia and the subsequent reperfusion of body at operation.

The pharmacokinetics was not carried out.

Indications to use:

Cardioplegia (the operation demanding a cardiac standstill in the conditions of artificial circulation).

Route of administration and doses:

Use for a cardioplegic cardiac standstill by two techniques.

The first technique - for a cold cardiac standstill use solution with a temperature of 4 °C. Initial volume - in number of 600-800 ml, with simultaneous intra pericardiac cooling. Further - each 20-30 min. on 200-400 ml at a temperature of 4 °C, depending on the heart sizes.

This technique is recommended when carrying out a cardioplegia via the cardiopulmonary bypass.

The second technique - originally apply solution of room temperature (18-26 °C) of 400 ml, then - 400 ml of solution with a temperature of 4 °C. At the same time carry out intra pericardiac cooling of heart. Further - each 20-30 min. on 200-400 ml of solution with a temperature of 4 °C.

At antegrade delivery through a root of an aorta perfuse under pressure of 50-60 mm hg.

Intra pericardiac cooling in all cases is carried out by 0.9% by chloride sodium solution in the form of ice gruel.

Are not revealed.
