Diclofenac

General characteristics. Structure:

Gel for external use of 1% of color, white or white with a yellowish or creamy shade, homogeneous, with a specific smell.

Active ingredient:    1 g
diclofenac of sodium           of 10 mg

Excipients: the ethanol rectified propylene glycol, carbomer (карбопол), methylparahydroxybenzoate (нипагин), trietanolamine (троламин), oil lavender, the water purified.

Pharmacological properties:

NPVP for external use.

Diclofenac has the expressed analgeziruyushchy and antiinflammatory properties. Not selectively oppressing TsOG-1 and TsOG-2 enzymes, breaks metabolism of arachidonic acid and synthesis of prostaglandins which are the main link in development of an inflammation.

It is used for elimination of a pain syndrome and reduction of the puffiness connected with inflammatory process. At topical administration causes easing or disappearance of joint pains at rest and at the movement. Reduces morning constraint and a swelling of joints, promotes increase in volume of movements.

 

Pharmacokinetics. At the recommended way of putting drug no more than 6% of diclofenac are absorbed.

Indications to use:

— a posttraumatic inflammation of soft tissues and joints owing to stretchings, retension and bruises;

— rheumatic diseases of soft tissues (including tendovaginitis, bursitis, defeat of periartikulyarny fabrics);

— the pain syndrome and puffiness connected with diseases of muscles and joints (including a pseudorheumatism, an osteoarthrosis, radiculitises, a lumbago, a sciatica);

— muscular pains of a rheumatic and not rheumatic origin (for gel of 5%).

Route of administration and doses:

Outwardly. The amount of drug depends on the size of a painful zone. The single dose of gel of 1% makes 2-4 g (on volume it is comparable to the size of cherry or walnut). A single dose of gel of 5% - to 2 g (about 4 cm at completely open mouth).

To adults and children 12 years are aged more senior drug should be applied on skin 3-4 and to rub slightly.

Children aged from 6 up to 12 years drug in the form of gel of 1% should apply on skin no more than 2 g (for one drawing) to 2 times/; drug in the form of gel of 5% - no more than 1 g (about 2 cm at completely open mouth) for one drawing to 2 times /

After putting drug of a hand it is necessary to wash up.

Duration of treatment depends on indications and noted effect. After 2 weeks of use of drug it is necessary to consult with the doctor.

Features of use:

Gel should be applied only on the unimpaired skin, avoiding hit on open wounds.

After drawing it is not necessary to apply an occlusive bandage.

It is not necessary to allow hits of drug on eyes and mucous membranes.

At long use of large amounts of drug it is impossible to exclude emergence of the system side effects characteristic of NPVP.

Side effects:

Local reactions: eczema, a photosensitization, contact dermatitis (an itch, reddening, puffiness of the processed site of skin, a papule, a vesicle, a peeling).

System reactions: generalized skin rash, allergic reactions (small tortoiseshell, Quincke's disease, bronkhospastichesky reactions), photosensitization.

Interaction with other medicines:

Drug can strengthen effect of the drugs causing a photosensitization.

Clinically significant interaction with other medicines is not described.

Contraindications:

— disturbance of integrity of integuments;

— the instruction in the anamnesis on attacks of bronchial asthma after use of NPVP and salicylates (including acetylsalicylic acid);

— III trimester of pregnancy;

— lactation period;

— children's age up to 6 years;

— hypersensitivity to diclofenac or other components of drug;

— hypersensitivity to salicylates (including to acetylsalicylic acid) or other NPVP.

With care it is necessary to use drug at a hepatic porphyria (aggravation), GIT erosive cankers, heavy abnormal liver functions and kidneys, chronic heart failure, bronchial asthma, in I and II trimesters of pregnancy, at patients of advanced age.

In addition for drug in the form of gel of 5%: with care it is necessary to apply at disturbances of coagulability of blood (including hemophilia, lengthening of a bleeding time, tendency to bleedings).

Use of drug in the III trimester of pregnancy is contraindicated. Use in I and II trimesters is possible only after consultation with the doctor.

Experience of use of drug in the period of a lactation is not available.

 

Use at abnormal liver functions
With care it is necessary to use drug at a hepatic porphyria, heavy abnormal liver functions.

 

Use at renal failures
With care it is necessary to use drug at heavy renal failures.

Overdose:

Extremely low system absorption of active components of drug at external use makes overdose almost impossible.

Symptoms: at an accidental proglatyvaniye of large amounts of gel (more than 20 g) emergence of the system undesirable reactions characteristic of NPVP is possible.

Treatment: it is necessary to wash out a stomach, to accept absorbent carbon.

Storage conditions:

Drug should be stored in the place, dry, unavailable to children, at a temperature from 15 to 25 °C; not to freeze. A period of validity - 2 years.
