Description:


The Benign Paroxysmal Position Dizziness (BPPD) meets often. About 2,4% of people test DPPG during this or that period of the life. Behind the long obscure name "benign paroxysmal position dizziness" the everyday situations happening hide if not with you, then with your immediate family. The person sits at a table, lifts and turns the head to look at the book shelf, and suddenly tests a dizziness attack. The person lies on a bed, begins to turn with one side on another and "departs, being turned". These are only some of considerable quantity of cases which cornerstone DPPG usually is. Dizziness episodes at this disease are short-term and usually last no more than a minute.

The word "high-quality" in the name of a disease means that this dizziness is not followed by any serious disorders of the central nervous system. The term "position" says that  benign paroxysmal position dizziness  is caused by a certain position of the head. "Paroxysmal" means sudden bystry emergence of an attack.


Reasons of benign position dizziness:


  "the stone disease of the channel" is the cornerstone of a phenomenon of DPPG so-called каналитиаз, that is. Kanalitiaz is that can get to one of semicircular channels of an inner ear ("to be poured") fragments of otoliths from a threshold of an inner ear. Otoliths are the "stones" which are a part of "sensors" of gravitation of a threshold of an inner ear. In the semicircular channels which are responsible for other aspects of control of balance they should not be. It is considered that hit by gravity of particles of otoliths to the semicircular canal, (for example when the person turns over in a bed) can cause a dizziness attack.

If to try to simplify as much as possible the previous paragraph, then the DPPG mechanism can be described so:  the small kristallik necessary for the correct work of a vestibular mechanism are poured from one part of an inner ear into another where they should not be that provokes dizziness.

Symptoms of benign position dizziness:


The diagnosis "benign paroxysmal position dizziness" is considered confirmed if two conditions match:

- the patient complains of the repeating dizziness attacks which arise at change of position of the head in space;

- during inspection of the patient when carrying out reception Dix-Hollpayka is found in the patient special type of nystagmus and there is dizziness.

The nystagmus is spontaneous oscillating motions of eyeglobes. Reception (test, maneuver) Dix-Hollpayka is, in fact, provocation of dizziness. The patient takes seat on a bed, his head turns by 45 degrees aside. Keeping turn of the head, the patient by means of the doctor quickly lays down horizontally, slightly unbending a neck behind edge of a bed back.  At positive test Dix-Hollpayka the doctor observes the characteristic  movements of eyeglobes, and the patient feels short-term dizziness.


Treatment of benign position dizziness:


Treatment at DPPG is required not always, attacks of dizziness can disappear independently in several weeks or months. To accelerate recovery the special receptions aiming to remove (or it is possible to tell "to vytrusit") particles of otoliths from the semicircular channel back in a threshold are used. The most known "setting" reception is Epli's maneuver. The epigraph to article from "Romeo and Juliette" remarkably conveys the general meaning of the procedure.  This reception consists in consecutive change of several provisions of the head therefore fragments of otoliths leave the semicircular channel. Efficiency of maneuver of Epli is considered high, and the risk of deterioration at its performance is minimum
