General characteristics. Structure:

Active ingredient: 150 AE immunoglobulins of specific protein fraction of blood serum of horses, giperimmunizirovanny poison of a viper ordinary.

Pharmacological properties:

Pharmacodynamics. Drug represents immunoglobin fraction of blood serum of horses, immunizirovanny viper poison, cleared and concentrated by method of peptic digestion and salt fractionation, containing specific antibodies.

The antibodies which are contained in serum neutralize poison of a viper ordinary.

Indications to use:

— treatment of the people bitten by a viper.

Route of administration and doses:

Serum is entered subcutaneously, intramusculary or intravenously into the earliest terms after a snake bite.

First-aid treatment bitten on site. The victim is stacked in a shadow and give plentiful drink: tea, coffee, milk, broth, water (the use of alcoholic beverages is forbidden) and enter 1 medical dose of serum irrespective of the body weight of bitten. In order to avoid an acute anaphylaxis or other allergic complications, before administration of serum the victim is given inside 1-2 tablets of any antihistaminic drug (Dimedrol, Pipolphenum, tavegil, etc.). Serum is entered subcutaneously fractionally (on Bezredk) into any body part of the victim: in the beginning 0.1 ml, in the absence of reaction in 10-15 min. enter 0.25 ml and then in 15 min. in the absence of side reactions all remained serum. The opened ampoule should be covered with sterile bandage or a cotton plug.

After first-aid treatment it is necessary to provide an urgent gosshchggalization of the victim in the nearest medical institution where will continue treatment. Transport the patient in lying situation. The bitten leg is bandaged to healthy, giving slightly sublime situation. At a sting in a hand it is fixed in the bent situation with the help of the fabric thrown through a neck.

Help in medical institution. The general entered dose of serum is established by the doctor depending on intoxication degree: at easy degree intramusculary 1-2 doses, at heavy 4-5 doses, taking into account the dose entered before hospitalization.

In case of especially heavy intoxication serum is recommended to be entered intravenously slowly kapelno after cultivation (1/5-1/10) by the sterile, warmed-up up to the temperature (37±1) of °C of 0.9% chloride sodium solution for injections. Rate of administering: in the beginning 1 ml within 5 min., then on 1 ml in min. In exceptional cases, at impossibility of implementation of drop infusion, slow jet introduction of a medical dose of serum without cultivation is allowed by the syringe. In order to avoid possible allergic reactions prior to intravenous injection of serum to the patient struyno enter 60-90 mg of Prednisolonum. Intravenous administration of serum is allowed only to medical personnel!

At children the same principle of dosing, as at adults, irrespective of the weight and age is used.

Features of use:

Use for children. At children the same principle of dosing, as at adults, irrespective of the weight and age is used.

Special instructions. Precautionary measures at use. Ampoules with serum before opening are carefully looked through. Drug is not subject to use at:

- change of physical properties (muddy contents of an ampoule, existence of not developing flakes);

- any damage of integrity of ampoules (crack, etc.).

The drug for first-aid treatment stored in conditions of expeditions with disturbance of temperature condition upon termination of an expedition is not suitable for use.

Influence on ability to driving of motor transport and to control of mechanisms. No.

Side effects:

Administration of serum against poison of a viper can be followed by development of various allergic reactions: immediate, coming right after introduction or in several hours and shown symptoms of an acute anaphylaxis, early - for the 2nd days after introduction and remote - for 5-10 days. The last are followed by a symptom complex of a serum disease (temperature increase, emergence of an itch and an enanthesis, joint pains, etc.). At emergence of anaphylactic reaction administration of serum is stopped. Subcutaneously enter adrenaline (Epinephrinum) (0.3-1 ml), Cordiaminum (1.5-2 ml), Prednisolonum of 25 ml or a hydrocortisone (50-100 ml), strophanthin (0.5 ml of 0.04%) with glucose (20 ml of 40% of solution). Considering possibility of shock at administration of serum behind everyone vaccinated it is necessary to provide medical observation within 1 hour after the end of administration of drug.

Administration of serum has to be registered in the established registration forms with the obligatory indication of date, a way and time of introduction, reaction of the patient, the manufacturing enterprise and number of a series of drug.

Interaction with other medicines:

It is necessary to consider compatibility with other medicines containing a foreign protein in the structure.

Contraindications:

Contraindication to continuation of use of serum is only development of an acute anaphylaxis at introduction of 0.1-0.25 ml of serum.
