Duphaston – gestagenny drug.

Form of release and structure

Dosage form – tablets, coated: white color, a round biconvex form with slanted edges and dividing risky on one of the parties, on both sides from risks there is an engraving "155" (on 20 pieces in blisters, in a cardboard pack 1 blister).

Active agent of Duphaston – дидрогестерон, in 1 tablet – 10 mg.

Auxiliary components: gipromelloza, lactoses monohydrate, starch corn, magnesium stearate, silicon dioxide colloid.

Structure of a cover: опадрай white Y-1-7000 (polyethyleneglycol 400, gipromelloz, titanium dioxide (E171)).

Indications to use

Use of Duphaston is shown to women at progesteronovy insufficiency. Characteristic symptoms of deficit of progesterone are:

• The infertility caused by lyuteinovy insufficiency;
• Endometriosis;
• The abortion menacing or usual (against the background of insufficiency of progesterone);
• Secondary amenorrhea (in complex therapy with estrogen);
• Dysfunctional uterine bleeding;
• Premenstrual syndrome;
• Irregular periods, dysmenorrhea.

Besides, drug is used in the form of replacement hormonal therapy for neutralization of proliferative effect of estrogen on an endometria at patients with the frustration caused by a surgical or natural menopause at an intact uterus.

Contraindications

• Insufficiency of lactase, intolerance of a galactose and glucose, sprue;
• Breastfeeding period;
• Hypersensitivity to drug components.

It is necessary to appoint use of Duphaston with care if in the anamnesis of the patient the skin itch at the previous pregnancy is specified.

Route of administration and dosage

Pill is taken inside.

Single dose of drug – 10 mg, frequency rate of reception and the period of treatment depend on clinical indications:

• Endometriosis: 2-3 times a day continuously or from the 5th to the 25th day of a menstrual cycle;
• The infertility caused by lyuteinovy insufficiency: 1 time a day from the 14th to the 25th day of a cycle, continuous therapy lasts not less than 6 following cycles. Treatment should be continued within the first months of pregnancy;
• The menacing abortion: once – 40 mg, then on 10 mg accept every 8 hours before stabilization of a state;
• Usual abortion: 2 times a day up to 20 weeks of pregnancy, then a dose gradually it is necessary to reduce;
• Premenstrual syndrome: 2 times a day from the 11th to the 25th day of a cycle;
• Dysmenorrhea: 2 times a day from the 5th to the 25th day of a cycle;
• Irregular periods: 2 times a day from the 11th to the 25th day of a cycle;
• Amenorrhea: in a combination with estrogen (they are appointed 1 time a day from 1st to the 25th day of a cycle) Duphaston accept 10 mg 2 times a day from the 11th to the 25th day of a cycle;
• Dysfunctional uterine bleeding: 2 times a day, a therapy course – 5-7 days;
• Prevention of dysfunctional uterine bleeding: 2 times a day from the 11th to the 25th day of a cycle;
• Replacement hormonal therapy: at continuous reception of estrogen drug is appointed on 10 mg of 1 times a day within 14 days (at a menstrual cycle of 28 days), and at the cyclic scheme of estrogenic therapy – within 12-14 last days of use of estrogen. At inadequate reaction to the progestagenny therapy confirmed with ultrasonography or a biopsy, about 20 mg are recommended to lift a daily dose.

Side effects

Use of Duphaston can cause side effects:

• From gepatobiliarny system: seldom – the insignificant functional disturbances of a liver which are followed by an indisposition, weakness, sometimes pain in a stomach or jaundice;
• From system of a hemopoiesis: in isolated cases – hemolitic anemia;
• From a nervous system: headache, migraine;
• From immune system: very seldom – hypersensitivity reactions;
• From skin and hypodermic cellulose: allergic reactions, in the form of a skin itch, rash, a small tortoiseshell; very seldom – a Quincke's edema;
• From a reproductive system: seldom – hypersensitivity of mammary glands, breakthrough uterine bleedings which it is possible to warn increase in a dose;
• Others: very seldom – peripheral hypostases.

Special instructions

In case of development of breakthrough bleedings it is necessary to increase a drug dose.

At replacement hormonal therapy and other cases of the combined reception of Duphaston with oestrogenic drugs it is necessary to consider the contraindications and recommendations connected with their use.

Performing replacement hormonal therapy is recommended to be begun only after collecting the full anamnesis, in the course of treatment it is necessary to control individual portability periodically. The patient has to be informed about what symptoms of changes in mammary glands it is necessary to report to the doctor. Mammography and other researches are conducted according to the standard screening of patients.

Exact assessment of advantage and risk of replacement hormonal therapy can be defined over time.

In case of developing of breakthrough uterine bleedings in process or after a therapy course, it is necessary to carry out an endometria biopsy to exclude malignant new growths.

Carrying out careful clinical inspection of patients about progesterone - a dependent tumor (meningioma) in the anamnesis, and also in case of its progressing during pregnancy is required or at the previous hormonal therapy.

Drug does not exert impact on ability to control of vehicles and mechanisms.

Medicinal interaction

Simultaneous use with rifampicin, phenobarbital and other inductors of microsomal enzymes of a liver can promote acceleration of processes of metabolism of a didrogesteron and reduce efficiency of drug.

Clinically significant cases of incompatibility of Duphaston with other medicines it is not established.
