Preparation of a burn surface for an autodermoplastika

Now there are two main ways of preparation of deep burn wounds to an autodermoplastika: a chemical necretomy from autodermoplastiky delayed autodermoplastiky and a surgical necretomy with single-step or delayed. Tactics of topical treatment with use of a chemical necretomy is quite justified at extensive deep burns more than 40% of a body surface on condition of extremely serious general condition of patients. Especially at treatment of patients of advanced and senile age at whom the heavy accompanying pathology makes operative measures in early terms impossible.
In these cases from first days after an injury topical treatment has to be directed to bystry formation of a dry burn scab, prevention of infection and deepening of burn wounds. For this purpose in the period of burn shock apply wadded gauze bandages with ointments on a water-soluble basis. It promotes reduction of loss of liquid from a burn surface, to warming of the patient, formation of a dry scab, does not demand daily bandagings.
In the next days formation of a dry burn scab is reached by use of the wet drying bandages. Optimum drugs during this period are also 1% the solutions of a yodopiron or yodovidon providing drying of a scab and possessing a wide range of antimicrobic and antifungal action. Perhaps also use of wadded gauze bandages with ointments on a water-soluble basis. Use of ointment on a fatty basis is contraindicated.
Considerably treatment of the patient in the conditions of the abacterial environment accelerates formation of a dry scab. In this case the open method of treatment with processing of burn wounds two-three times in day of 1% solution of a yodopiron, yodovidon or the drug "Naksol" and use of abacterial insulators or beds of Klinitron is used. Drying of a scab is promoted by infrared radiation of wounds. Formation of a dry scab reduces losses of protein from a burn surface, promotes reduction of intoxication, improvement of the general condition of the patient.
