General characteristics. Structure:

Acting substances: Domperidon - 30 mg; Omeprazol - 20 mg

Other ingredients: seeds a nonpareil, magnesium a carbonate easy, a gipromelloza, ethyl cellulose, the copolymer of methacrylic acid, a macrogoal, triacetin, talc purified sodium hydroxide, silicon hydroxide colloid, titanium dioxide, ferrous oxide red (Е 172), ferrous oxide yellow.

Pharmacological properties:

Pharmacodynamics. The combined drug which effect is caused by the components which are its part.
Omeprazol treats antiulcerous means which oppress basal and stimulated secretion of hydrochloric acid in obkladochny cells of a stomach owing to specific blocking of N+/K+-Atfazy (the proton pump). The anti-secretory effect of an omeprazol develops for the first hour after reception and remains for days. Omeprazol thanks to the high lipophilicity easily gets into covering cells of a stomach, concentrates in them and has cytoprotective effect. The inhibiting effect increases in the first 4 days of reception. Omeprazol does not influence motility of a GIT.
Domperidon blocks peripheral dopamine receptors, eliminates the inhibiting influence of a dopamine on the GIT motor function, in particular stimulates motor эвакуаторную activity of a stomach. Eliminates nausea and a hiccups, has antiemetic effect. Badly gets through GEB, practically does not influence dopamine receptors of a brain.

Pharmacokinetics. it was not studied.

Indications to use:

Treatment of a round ulcer of a stomach and duodenum in the absence of H. pylori, at the slowed-down evacuation of contents of a stomach, a gastropareza, a reflux esophagitis, functional dyspepsia.

Route of administration and doses:

Capsules accept inside whole, without opening and without chewing. The recommended dose and duration of a course of treatment depend on the course of a disease and are established individually. The average recommended dose for adults and children is aged more senior than 12 years — 1 capsule of 1 times a day in 10–15 min. prior to food (to wash down with a glass of water).
At patients of advanced age and patients with a renal failure individual approach in selection of a dose is necessary, however in many cases of dose adjustment it is not required.

Features of use:

Before the beginning and after completion of therapy omeprazoly it is necessary to conduct an endoscopic research for an exception of cases of not diagnosed malignant process as treatment omeprazoly can disguise symptoms and delay the correct diagnosis.
It is necessary for patients with chronic diseases of a liver constantly (at least 1 time in 2 weeks) to conduct a laboratory blood analysis for definition of activity of liver enzymes. In case of changes of these indicators administration of drug should be stopped.
During treatment by drug it is necessary to be careful at control of vehicles or work with difficult mechanisms.

Side effects:

Most of patients well transfer drug. At prolonged use can note skin reactions (rash, an itch, urticaria, a multiformny erythema); dryness in a mouth, diarrhea, a lock, intestinal colic, a headache, a photosensitization, an alopecia, a Quincke's edema, dizziness, paresthesias, drowsiness, sleeplessness, a sight illegibility, change of flavoring feelings, excitement, a depression, a leukopenia, thrombocytopenia, a pancytopenia, an agranulocytosis, a mialgiya, an arthralgia, a bronchospasm, intersticial nephrite, it is very rare — a galactorrhoea, a gynecomastia. Domperidon can cause development of extrapyramidal frustration, especially at patients of advanced age.

Interaction with other medicines:

Antiacid drugs slow down and reduce Limzer's absorption therefore they should be applied not earlier than in 2 h after his reception.
As metabolism of an omeprazol is carried out in a liver by means of system of P450 cytochrome, it is necessary to take with caution it together with diazepam, Phenytoinum, warfarin, propranolol, Prednisolonum, theophylline (at the same time омепразол can reduce their elimination that demands decrease in doses of these drugs). At simultaneous use of an omeprazol and klaritromitsin their concentration in a blood plasma increases.
Cimetidinum and Natrii hydrocarbonas reduce bioavailability of a domperidon.
Anticholinergic drugs neutralize action of a domperidon.

Contraindications:

Hypersensitivity to drug components, gastrointestinal bleedings, mechanical intestinal impassability, perforation of a stomach or intestines, a prolaktinom, the period of pregnancy and feeding by a breast, age up to 12 years.

Overdose:

Symptoms: drowsiness, disorientation, vomiting, tachycardia, extrapyramidal disturbances.
Treatment: use of absorbent carbon, symptomatic therapy.
