Biyenosilim

General characteristics. Structure:

Active agents: 55 mg of a thistle spotty fruits of extract dry (in terms of the sum of flavoligpan in terms of Silibininum), 30 mg of a biyen (in recalculation of a pas ethyl ethers of fatty acids).

Excipients: K-25 povidone, calcium stearate, lactoses monohydrate.

Structure of a gelatinous solid capsule: gelatin, glycerin, the water purified titanium dioxide, sodium lauryl sulfate.

Pharmacological properties:

Pharmacodynamics. The combined medicine containing 2 (two) active agents: Silymarinum and биен. Silymarinum is mix of 4 isomers of flavonolignan: Silibininum, iso-Silibininum, silidianin and silikristin. The flavonoid which is a part of drug Silibininum has gepatoprotektivny and anti-toxic effect, stabilizes membranes of cells of a liver, entering with them biochemical interaction, prevents loss of compound components of a cell (including transaminases). At the expense of phenolic structure it is capable to connect free radicals and to stop process of peroxide oxidation of lipids (and by that further destruction of cellular structures). Slows down formation of low-new dial and the raised oxygen absorption. Due to blocking of places of communication of toxins or transport systems interferes with their penetration into a hepatocyte. Stimulating RNA - a polymerase, increases synthesis of structural and functional proteins and phospholipids, accelerates regeneration of the injured hepatocytes. At alcoholic damages of a liver hepatoprotective action is caused by blocking of production of acetic aldehyde and binding of free radicals. The last promotes also preservation of a reserve of the glutathione playing an important role in a hepatic detoxication of xenobiotics. Slows down penetration into a cell of some hepatotoxic substances (poison of a mushroom of a death angel).

Biyen - a complex of ethyl ethers of the unsaturated fatty acids received from lipids of a micellar mushroom of Entomophthora virulenta. Renders hypolipidemic, gipokholesterinemichesky, reparantny, cytoprotective action. The unsaturated fatty acids which are a part of a biyen, including essential (linoleic, α-linolenic, arachidonic) are important components of biomembranes and sources of autokoid - bioregulators of functional activity of cells and tissues of a liver. Essential fatty acids are predecessors of synthesis of karbonilsoderzhashchy prostaglandins, natural endogenous gepatoprotektor. Biyenosilim's reception improves the general condition of patients with liver diseases, reduces subjective complaints, normalizes laboratory indicators (activity of transaminases, gamma глутамилтрансферазы, alkaline phosphotazy, bilirubin level).

Pharmacokinetics. Silymarinum. At intake it is slowly adsorbed from digestive tract, the elimination half-life of Silymarinum of-6 h is metabolized in a liver by conjugation. The highest concentration of Silymarinum are found in a liver and absolutely insignificant quantities - in kidneys, lungs, heart and other bodies. It is removed preferential (80%) with bile in the form of glucuronides and sulfates and in insignificant degree (about 5%) with urine. Does not kumulirut in an organism. After multiple dose inside, on 2 capsules of Biyenosilim (140 mg in terms of Silymarinum) 3 times a day, are reached the stable concentration which is removed with bile. Biyen. The arachidonic acid which is a part of a biyen is a predecessor of biologically active compounds - eicosanoids: diene prostanoid (prostaglandins; prostacyclins, thromboxanes), tetrayenovy leukotrienes, tetrayenovy lipotriyen, lipoksin, gidroperoksi-and hydroxyeicosahalf-yen acids. Дигомо-γ-линоленовая acid through Δ5-десатуразу turns into arachidonic acid, or itself directly participates in synthesis of eicosanoids. α-linolenic acid acid in the course of biotransformation forms eykozapentayenovy acid and dokozageksayenovy acid, can take part in formation of prostaglandins and leukotrienes through a number of intermediate stages. From α-linolenic acid triyenovy prostanoida are formed (prostaglandins; prostacyclins, thromboxanes), pentayenovy leukotrienes, pentayenovy lipotriyena, lipoksina, gidroperoksi-and hydroxyeicosahalf-yen acids. Linolic acid is a predecessor of biosynthesis of monoyenovy prostanoid (prostaglandins; prostacyclins, thromboxanes), and also triyenovy leukotrienes, triyenovy lipotriyen, lipoksin.

Indications to use:

Toxic injuries of a liver (alcoholism; intoxication galogensoderzhashchy hydrocarbons, compounds of heavy metals; medicinal damages of a liver) and their prevention.

As a part of a combination therapy: chronic hepatitis, cirrhosis, states after infectious and toxic hepatitises, dystrophy and fatty infiltration of a liver.

Route of administration and doses:

Inside in 30 min. prior to food without chewing, washing down with a small amount of water. For treatment of heavy toxic injuries of a liver on 1 - 2 capsules 3 times a day within 25-45 days. A maintenance dose at treatment of chronic diseases of a liver on 1 capsule 2 times a day. Maximum duration of a course of a maintenance therapy 3 months.

Features of use:

Because of potentiality of development of the expressed by-effects when using high doses of drug, Biyenosilim with care appoint at patients with chronic pancreatitis, calculous cholecystitis, gout, arthropathies of various genesis, the expressed abnormal liver functions. At drug use perhaps manifestation of estrogenopodobny action of Silymarinum to patients with hormonal disturbances (endometriosis, a hysteromyoma, a carcinoma of a mammary gland, ovaries, a uterus, a prostate) drug is appointed with care. For the period of treatment by drug it is necessary to stop breastfeeding.

Influence on ability to driving and potentially dangerous mechanisms: does not render.

Side effects:

From digestive tract: often - the dispepsichesky phenomena (nausea, heartburn, a meteorism, an eructation bitterness or oil, smack of oil or bitterness in a mouth) not demanding special intervention or drug withdrawal; seldom - ease of a chair (diarrhea), a lock, pains in right hypochondrium, exacerbations of gout (as a rule, pass after drug withdrawal).

Allergic reactions: very seldom - skin rash, an itch.

From a nervous system: very seldom - strengthening of the existing vestibular disturbances (vomiting, dizziness).

Interaction with other medicines:

At combined use with peroral contraceptive drugs and drugs which are used at hormonal replacement therapy reduction of their effect Silymarinum is possible. Silymarinum which is Biyenosilim's part suppresses system of P450 cytochrome and can strengthen effects of such drugs as diazepam, to alprazola, кетоконазол, ловастатин, vinblastine.

Contraindications:

Hypersensitivity to drug components, the burdened аллергоанамнез, acute pancreatitis, pregnancy, breastfeeding, children's age up to 18 years (efficiency and safety are not established).

With care: arthropathy, chronic pancreatitis, diarrhea, gout, endometriosis, hysteromyoma, carcinoma of a mammary gland, ovaries, uterus, prostate.

Overdose:

The overdose phenomena at Biyenosilim's use are not described. In case of deliberate reception of high doses of drug strengthening of expressiveness of undesirable reactions is possible: diarrhea, nausea, heartburn, a meteorism, an eructation bitterness or oil, smack of bitterness or oil in a mouth.

Treatment: symptomatic (there is no specific antidote). It is necessary to cause vomiting, to carry out a gastric lavage using absorbent carbon.

Storage conditions:

In the place protected from moisture and light at a temperature not above 25 °C. To store in the place, unavailable to children. Period of validity 3 years. Not to use after the period of validity specified on packaging.
