Rivodaron

General characteristics. Structure:

Active agent: 200 mg of Amiodaronum of a hydrochloride.

Pharmacological properties:

Amiodaronum on classification of Vaughan Williams belongs to class medicines III. Blocks ion channels, mainly potassium and to a lesser extent calcic, the alpha ‑ and betaadrenergichesky inhibitor is non-competitive. Slows down sinuatrial, atrialny and nodal conductivity, does not exert impact on intra ventricular conductivity. Increases the refractory period and reduces excitability of a myocardium of an auricle, ventricle and Avuzla.

Indications to use:

Disturbances of a rhythm of auricles (fibrillation of auricles or a ciliary arrhythmia, maintenance of a sinoatrial rate after cardioversion), Avuzlovye arrhythmias — tachycardia, ventricular tachycardia (life-threatening premature reduction of ventricles, ventricular tachycardia volleys, prevention of attacks of ventricular tachycardia and fibrillation of ventricles), the disturbances of a rhythm connected with Wpwsindromom. Amiodaronum is appointed, first of all, in cases when above the given arrhythmias are followed by other heart disease (arrhythmias against the background of coronary insufficiency or HSN). Medicine is intended for therapy of adults.

Route of administration and doses:

The initial daily dose of 600 mg divided into three receptions for 8 — 10 days. The daily dose can be raised to 1 200 mg. The dose of a maintenance therapy is established individually. Usually — 100 — 400 mg a day. A pill is taken entirely, without chewing, regardless of meal.

Features of use:

During treatment it is necessary to abstain sunshine.

Side effects:

Pro-arhythmic actions meet seldom, depending on a dosage symptomatic bradycardia can develop. Amiodaronum collects in a cornea epithelium, inhibits transformation of thyroxine to triiodothyronine (owing to presence of iodine at a molecule), causes a photosensitivity, peripheral neuropathy, intersticial pneumonia, headaches, sleep disorders, nausea, a lock and other disorders of a GIT.

Interaction with other medicines:

Simultaneous use of Amiodaronum with quinidine, sotaloly, the erythromycin entered intravenously or with the drugs causing a hypopotassemia (diuretic means, system kortikoida, Amphotericinum In) can cause serious violations of a rhythm, including arrhythmia like "pirouette" (torsades de pointes). Bradycardia and disturbances of transfer can arise in a combination with beta adrenoblockers, some blockers of calcium channels (verapamil, diltiazem) and the general anesthetics. Amiodaronum increases efficiency of peroral anticoagulants, increases concentration of digoxin, Phenytoinum, cyclosporine.

Contraindications:

Sinus bradycardia, Sablokada. A sick sinus syndrome and Avblokada without use of a pacemaker (risk of a sinus stop). Thyroid dysfunction. Hypersensitivity to iodine, or to the substances which are contained in drug. A combination therapy with the drugs capable to cause arrhythmia like "pirouette" (torsades de pointes). Feeding by a breast. Considering the content of iodine, to use drug during pregnancy only in inevitable cases, very short course.

Overdose:

There are no data on overdose.

Storage conditions:

To store the place protected from light and moisture, at a temperature not over 25 ºС. To store in the place, unavailable to children.
