General characteristics. Structure:
Active ingredient: 0,4 g of a diatsetofenonilselenid.
Pharmacological properties:
Pharmacodynamics. Selenobel – medicine on the basis of organic compound of selenium – a diatsetofenonilselenid (C16H14O2Se) which molecule contains 25% of an inorganic microelement.
Selenium is one of the essential microelements playing an important role in normal functioning of vitals and systems. Daily requirement of a human body makes about 2 mkg/kg of body weight.
Selenium – the microelement which is a part glutathione peroxidases which comes to light practically in all cells and biological liquids of the person and animals. Enzyme has no strict selectivity of action and represents the first line of protection of an organism against the damaging action of peroxide radicals which are formed in an organism in excess quantities at various morbid conditions, including a hypoxia, stresses, impact of ionizing radiation, etc.
The lack of selenium of an organism causes easing of body resistance to influence of adverse environmental factors (radiation, chemical and biological agents, weather meteoconditions). Long deficit of a microelement in a diet of people, especially at young age, can lead to development of very serious, quite often fatal disease – a dilatatsionny cardiomyopathy (a keshansky disease). Most often this pathology develops at the children and teenagers living in areas with the insufficient content of selenium in the soil. The disease is shown by dizziness, faints, heartbeat, pains and interruptions in cardiac performance, weakness, a shiver and pains of muscles, fatigue, an asthma at exercise stresses, decrease in working capacity.
On an ECG at the diseased disturbances of a rhythm of cordial activity, conductivity and excitability of a myocardium come to light. Characteristic of the specified pathology is its reversibility at timely appointment to patients of drugs of selenium. It is shown that at the person who arrived to the local area and eating products of local production, clinical symptoms of a disease can be shown in 3 months.
It is established that selenium has protective action at acute and chronic intoxications xenobiotics. It has ability to korrigirovat disturbances of immunity at persons who receive less necessary amounts of selenium in a diet.
Selenium is effective antioxidant at the cellular level. It has ability to change functional activity of phagocytes and to modulate an oxidatic cellular stress; to level the shifts of the oxidatic status accompanying endocrine frustration. Selenium increases nonspecific resistance of an organism at the diseases which are followed by exhaustion of the general resistance to extreme influences. It has ability to stimulate an erythrogenesis and a leukopoiesis. Increase in marrow of absolute number of erythroidal cells, reliable increase in absolute number of mature granulocytes and decrease in level of lymphoid elements is experimentally established considerable (by 1,5 times).
The direct damaging influence of selenium on tumor cells of the person is experimentally shown (both on proliferating cells, and on interfazny tumor cells).
Pharmacokinetics. Assimilation of soluble forms of selenium in a digestive tract happens for 80-100%. In experiments with administration of isotope 75Se it is shown that 91-93% of a microelement are soaked up in digestive tract. At adults at intake from 76 to 100% of selenium are absorbed. Organic compounds of selenium are acquired better, than inorganic.
Most intensively absorption of selenium happens in duodenal and in a little smaller degree – in lean and ileal, and also cross and colonic guts. In a stomach this microelement is practically not acquired. Intensity of absorption of selenium depends on character of a diet. Dominance of proteins, small amount of sulfates and salts of heavy metals promotes fuller digestion of selenium.
Transport and deposition of selenium in an organism is carried out as the specific proteins containing селеноцистеин (selenoproteina) and albumine, globulins, and also lipoproteins. The microelement blood is delivered to all bodies and fabrics. It passes through hematoencephalic, ophthalmologic and gematotestikulyarny barriers. At the person the greatest concentration of selenium comes to light in a myocardium, a liver and skeletal muscles. In small amounts it is found in adrenal glands and teeth. Selenium is emitted from an organism in three ways – through kidneys, intestines and lungs. At nursing mothers selenium is emitted with breast milk. In physiological conditions the homeostasis of selenium is regulated generally by excretion of this microelement with urine within 24 hours after its introduction.
Indications to use:
Prevention and treatment of selenic insufficiency; impact on an organism of adverse factors (radiation, chemical, biological) external environment.
Prevention of manifestations of toxic effects of courses of a polikhimoterapiya of malignant tumors.
Route of administration and doses:
For prevention of selenic insufficiency in local areas adults should use daily on 1 tablet of a selenobel. Persons who are in a local zone quickly are recommended to apply selenobet in the above-stated dosage during all term of stay in the region.
At impact on an organism of adverse environmental factors selenobet it is recommended to accept the adult on 1 tablet of 1 times a day.
Drug is accepted after food and washed down with a small amount of liquid.
At treatment of patients with a clinical picture of selenic insufficiency it is recommended to appoint selenobet on 1 tablet 3 times a day. Duration of a course of treatment depends on degree of manifestation of clinical displays of a disease.
For prevention of manifestations of toxic effects of himoterapevtichesky drugs to accept on 1 tablet once a day; during a chemotherapy, and also in intervals between chemotherapy courses - 400 mkg/days. Duration of reception selenobet up to 60 days.
Features of use:
It is not necessary to exceed the recommended drug doses. At emergence of symptoms of overdose use of a selenobel should be stopped.
Drugs of selenium do not influence ability to manage vehicles and moving mechanisms.
Clinical tests of efficiency and safety of a selenobel at children were not carried out. According to literary data selenosoderzhashchy drugs are successfully used for treatment of a local dilatatsionny cardiomyopathy at children and teenagers.
Use at pregnancy and a lactation: selenobet it is not recommended to appoint to pregnant women. In need of administration of drug in the period of a lactation it is necessary to stop breastfeeding for treatment.
Precautionary measures. At use of a selenobel of observance of special precautionary measures it is not required.
Side effects:
Allergic reactions, hyperemia, itch are in rare instances possible. At emergence of allergic reactions treatment by drug should be stopped, in case of sharp reddening and an itch it is necessary to carry out the desensibilizing therapy.
Interaction with other medicines:
Negative manifestations at use of the means containing selenium against the background of therapy by other medicines are not described.
Simultaneous use of a selenobel with the drugs containing selenium is not recommended (including as a part of complexes of vitamins and minerals.)
Contraindications:
Individual intolerance of selenium or other components of drug; polyvalent allergy, pregnancy, feeding by a breast.
Overdose:
Symptoms. At reception of high doses of a selenobel development of dermatitis, skin itch, the increased fragility of nails, a hair loss, irritability, increased fatigue, weakness, emergence of metal smack in a mouth, nausea, vomitings, a garlic smell in expired air and the separated sweat, and also development of peripheral neuropathy is possible.
Symptomatic treatment: gastric lavage, maintenance of the vital functions. There is no specific antidote.
