Oftamirin-Darnitsa

General characteristics. Structure:

Active ingredient: мирамистин;

1 ml of solution contains a miramistin of 0,1 mg;

excipients: sodium chloride, dinatrium phosphate dodecahydrate, sodium dihydrophosphate a dihydrate, water for injections.

Pharmacological properties:

Miramistin – cationic surfactant with antimicrobic (antiseptic) action.

The direct hydrophobic interaction of a molecule with lipids of membranes of microorganisms leading to their fragmentation and destruction is the cornerstone of action of a miramistin. At the same time a part of a molecule of a miramistin, plunging into the hydrophobic site  of a membrane, destroys a nadmembranny layer, loosens a membrane, increases its permeability for high-molecular substances, changes euzymatic activity of a microbic cell, inhibits fermental systems that leads to oppression of life activity of microorganisms and their cytolysis.

Unlike other antiseptic agents, мирамистин has sharp selectivity of action concerning microorganisms as practically does not affect membranes of cells of the person. This effect is connected with other structure of cellular membranes of the person (considerable the bigger length of the lipidic radicals which are sharply limiting a possibility of hydrophobic interaction of a miramistin with cells).

Miramistin possesses the expressed antimicrobic action concerning gram-positive and gram-negative, aerobic and anaerobic, spore-forming and asporogenic bacteria in the form of monocultures and microbic associations, including hospital strains with polyresistance to antibiotics.

Affects perniciously on causative agents of diseases,   sexually transmitted, – gonokokk, pale treponemas, trichomonads, chlamydias, and also  viruses of herpes, an immunodeficiency of the person, etc. Has antifungal effect on ascomycetes of the sort Aspergillus and the sort Penicillum, barmy (Rhodotorula rubra, Torulopsis gabrata, etc.) and drozhzhevidny (Candida albicans, Candida krusei, etc.) mushrooms, dermatophytes (Trichophyton rubrum, Trichophyton mentagrophytes, Trichopphyton verrucosum, T. schoenleini, T. violaceum, Epidermophyton Kaufman-Wolf, E. floccosum, Microsporum gypseum, Microsporum canis, etc.), and also on other pathogenic fungi (for example Pityrosporum orbiculare (Malassezia furfur)) in the form of monocultures and microbic associations, including fungal microflora with resistance to chemotherapeutic drugs. Under the influence of a miramistin resistance of microorganisms to antibiotics decreases.

Miramistin possesses antiinflammatory and immunoadjuvant action, strengthens local defense reactions, regenerator processes, activates mechanisms of nonspecific protection owing to modulation of a cellular and local humoral immune response, than accelerates recovery.

Miramistin has local effect. Data on possible penetration of drug into a system blood stream are absent.

Indications to use:

In complex treatment of the infectious processes of front department of an eye (blepharites, conjunctivitis, a keratitis, keratouveita) caused by the gram-positive and gram-negative  bacteria, chlamydias, mushrooms and viruses sensitive to a miramistin, eye injuries, burns of eyes (thermal and chemical).

Prevention of an ophthalmia of newborns, including gonococcal and Chlamidia.

Prevention and treatment of pyoinflammatory damages of eyes in before - and the postoperative period.

In complex treatment of acute and chronic outside otitis, a chronic purulent mesotympanitis, otomycoses.

In complex treatment of acute rhinitis.

Route of administration and doses:

Ophthalmology.

For treatment of infectious processes of front department of an eye by the adult and to children        12 years are more senior dig in in a conjunctival sac 2-3 drops, to children up to 12 years – on 1-2 drops of 4-6 times a day. Treatment duration usually does not exceed 2 weeks.

For the purpose of prevention of complications in before - and the postoperative period – on 2-3 drops              3 times a day in 2-3 days prior to operation. After operation – within 3-5 days on 1-2 drops              3 times a day.

At treatment of burns of eyes, after washing of an eye a large amount of water, carry out frequent instillations (each 5-10 minutes) for 1-2 hours. For further treatment drug is used to adults and children 12 years – on 2-3 drops, to children up to 12 years – on          1-2 drops of 4-6 times a day are more senior.

For prevention of an ophthalmia at newborns right after the birth to the child dig in on 1 drop of drug in each eye 3 times with an interval of 2-3 minutes.

 

Otolaryngology.

At treatment of acute and chronic outside otitises, otomycoses

adults to children is more senior 12 letzakapyvat 3-5 drops of 4-6 times a day;

to children up to 12 years – on 2-3 drops of 4-6 times a day.

A day for 10-14 days it is possible to enter the gauze turunda moistened with drug into outside acoustical pass of 4-6 times (instead of an instillation).

At chronic mesotympanites it is applied in complex treatment by means of hardware ultrasonic irrigation or introduction to a drum cavity together with antibiotics.

At treatment of acute rhinitises, infections of a mucous membrane of a nose dig in to adults and children 12 years on 2-3 drops, to children up to 12 years – on 1-2 drops in each nasal course         of 4-6 times a day are more senior. A course of treatment – 7-14 days.

Features of dosing. Exact dosing is reached by pressing a bottle bottom – one pressing = one drop.

Features of use:

Use during pregnancy or feeding by a breast.

Drug can be used during pregnancy or feeding by a breast.

Ability to influence speed of response at control of motor transport or work with other mechanisms.

Drug does not influence speed of response at control of motor transport or the robot with  other mechanisms.

Children. Drug is used in pediatric practice.

Side effects:

At drug use    perhaps short-term  burning sensation  disappearing independently in 15-20 sec. The termination of treatment is not required.

Interaction with other medicines:

At combined use with antibiotics of systemic and local action efficiency of drug increases.

Contraindications:

The increased individual sensitivity to a miramistin or to other components of drug.

Overdose:

Cases of overdose were not observed.

Storage conditions:

Period of validity. 3 years. репарат it is impossible to apply after the termination of the period of validity specified on packaging and in cases when control of "the first opening" is broken.

After opening of a bottle drug is used by no more than 28 days. to wound in original packaging, the place, unavailable to children, at a temperature not above 30 °C. To hold a bottle densely closed during the period between use.
