General characteristics. Structure:

Active agent: 15 mg / 30 mg chloride trospiya.

Pharmacological properties:

Trospy chloride — quarternary ammonium derivative a nortropana with parasympatolytic action (антихолинергик). Is a competitive antagonist of acetylcholine on postsynaptic muskarinovy receptors. Has high affinity to receptors of M1 and M3, low affinity to Sq.m receptors, and as a result of it has the weakening effect on smooth muscles of a bladder (detruzor). Increase in volume of a bladder, suppression of feeling of a desire to an urination, and, as a result, to considerable reduction of frequency of urinations is a clinical consequence of this effect. Trospiya chloride oppresses a kontraktilnost of smooth muscles not only urogenital system, but also a GIT. Reduces secretion of a bronchial secret, saliva, sweat and also accommodation. The trospiya of chloride is advantage the minimum penetration through a blood-brain barrier that reduces risk of development of the central parasympathetic side effects, and the minimum development of medicinal interactions.

Indications to use:

Symptomatic therapy of a pollakiuria and/or urgentnost (imperative desires to an urination) and/or an urgentny inkontinention at the patients suffering from a syndrome of a hyperactive bladder, or the idiopathic or neurogenic hyperactivity of a detruzor urodynamic confirmed.

Route of administration and doses:

The recommended daily dose — 45 mg a chloride trospiya. After assessment of individual efficiency and portability it is possible to lower a daily dose to 30 mg, or to raise to 90 mg after a four-week course of therapy. At patients with a heavy renal failure (clearance of creatinine — 10 — 30 ml/min. / 1,73 sq.m) the recommended dose — 15 mg a chloride 1 — 2 trospiya р / days or 1 — 2 time each two days. A pill is taken inside, entirely, during food, washing down with a glass of water.

Features of use:

Because Spazmed can break accommodation, ability to manage vehicles and service mechanisms can be reduced. Special care at patients with intestinal impassability, impassability of urinary tract with risk of development of an ischuria, autonomous neuropathy, hernia of an esophageal opening of a diaphragm and a reflux esophagitis, a heavy abnormal liver function or kidneys, and also at those patients to whom increase in heart rate is undesirable (a hyperthyroidism, an ischemic heart disease, heart failure) is necessary. Prior to therapy the organic reasons of a polakisuriya and an urgentnost have to be excluded (diseases heart, kidneys, a polydipsia, inflammations and malignant new growths of uric ways). Spazmed's use during pregnancy and feeding by a breast perhaps only in urgent cases.

Side effects:

Most often can appear: dryness in a mouth, dyspepsia, a lock, stomach aches, nausea; less often: accommodation disturbance, tachycardia, disturbance of bladder emptying.

Interaction with other medicines:

Drug can strengthen anticholinergic action of an amantadin, tricyclic antidepressants, quinidine, anti-histamine drugs, Disopyramidum; can strengthen tachycardic action a beta? sympathomimetics; to reduce action of prokinetics (for example, Metoclopramidum, цизаприд). Guar, холестирамин, холестипол can reduce absorption a chloride trospiya.

Contraindications:

Hypersensitivity to active ingredient, an urine retention, closed-angle glaucoma, a tachyarrhythmia, a myasthenia, a serious chronic inflammatory illness of intestines (ultserozny colitis or a disease Krone), toxic megacolon, the renal failure needing a hemodialysis. Drug is not intended for children aged up to 12 years.

Overdose:

There are no data on overdose.

Storage conditions:

Drug should be stored in dry, protected from light, the place, unavailable to children, at a temperature from 15 ˚ C to 25C.
