General characteristics. Structure:

Active ingredient: 10 mg of Sulfadiazinum of silver in 1 g of cream.

Excipients: methylparahydroxybenzoate, пропилпарагидроксибензоат, the castor oil which is not refined a makrogolglitserina hydroxystearate, a macrogoal cetostearyl ether, propylene glycol, cetostearyl alcohol, polysorbate 80, the water purified.

Chemotherapeutic means for topical administration which prevents and suppresses development of an infection at burns and other wound processes, reduces expansion of a necrosis in depth and provides healing of the damaged surfaces.




Pharmacological properties:

Pharmacodynamics. The mechanism of antimicrobic effect of cream of Sulfadiazinum of silver differs from the mechanism of effect of other silver salts and streptocides. Silver Sulfadiazinum in a wound moderately dissociates, and thus ions of silver are slowly and evenly released in concentration which are toxic for microorganisms, but are harmless to cells of skin and hypodermic tissue of the person. Ions of silver contact some components of a bacterial cell among most of which reaction, important for suppression of growth and reproduction, is linkng with RNA.

Sulfadiazinum is found in concentration which are much lower than MBK and therefore its direct impact on bacteria is negligible. It is confirmed also by the fact that paraaminobenzoic acid (PABK) does not suppress antimicrobic action of Sulfadiazinum of silver.

Cream Argezin possesses the wide antimicrobic action spectrum covering all species of microorganisms which can be found in burn wounds. Tests of in vitro showed that all strains of Pseudomonas aeruginosa, Proteus mirabilis and Escherichia coli show sensitivity to silver Sulfadiazinum already in concentration of 50 mg/ml. For Staphylococcus aureus, Klebsiella spp., Enterobacter, Candida albicans and some other fungi of MBK makes 100 mg/ml. It is established that Sulfadiazinum of silver is 50 times more effective than Sulfadiazinum, мафенид acetate and many other antimicrobic means. It is explained by its unique mechanism of antimicrobic action.

Pharmacokinetics. Pharmacological active component of cream Argezin gets into nekrotizirovanny fabric and exudate. This effect is especially important because the antibiotics of systemic action which are usually used in clinical practice do not reach microorganisms in a necrosis zone with the broken microcirculation. Silver Sulfadiazinum moderately dissociates in a wound, evenly and slowly releasing silver ions. Thus bystry and massive linkng of ions of silver with chlorine ions, proteins and other components of physiological liquids is prevented, and there are no disturbances of electrolytic balance.

The amount of the absorbed silver very little also does not exceed 1%. Researches using isotopes of silver showed that it cannot be found in bodies. Results of researches on animals show that the most part of the soaked-up silver is allocated with bile.

Sulfadiazinum can be absorbed in quantity to 10%, at the same time concentration in plasma generally does not exceed 2,0 mg of %. In case of drawing a large amount of cream on extensive burn wounds of value of concentration of Sulfadiazinum in plasma can approach therapeutic values (8-12 mg of %) in exceptional cases. Sulfadiazinum is removed with urine.


Indications to use:

Medicine Argezin in the form of cream is intended for topical treatment <the following diseases and morbid conditions:
burns of various depth and area, any nature (thermal, chemical, electric current, etc.);
• for healing of epidermal and superficial thermal burns;
• at deep thermal and subdermalny burns (at impossibility to do without operational treatment) for the purpose of the prevention of development of an infection before operational treatment if operation is not carried out during the first several days after getting injured;
• the infected superficial wounds and burns with weak exudation, decubituses, trophic it is also long not healing ulcers (including wounds of a stump), grazes, skin transplantation.


Route of administration and doses:

After preprocessing of a wound surface apply a layer 2-4 mm thick of Argezin on it. On less extensive wounds cream is applied with a sterile gauze, and on more extensive - a sterile shovel or a glove. Then apply a bandage a layer of cream or leave open (an exposure method at treatment of burns). In the first case it is necessary to make the bandage consisting of four layers of a sterile dry gauze, a vatirovanny gauze or a layer of sterile cotton wool and crepe bandage.

When using a method of exposure follows except the conditions shown to such method of treatment to take into account and the fact that the burn surface has to be always protected. Therefore as required Argezin apply on a burn surface several times in days.

Before each next application it is necessary to eliminate a layer of old cream completely.

At extensive wound surfaces cream is eliminated with a stream of warm water or by means of a bathtub with solution of an antiseptic agent, and at small - by means of the tampon moistened in normal saline solution or solution of an antiseptic agent (for example, Furacilin). In 48 hours after a burn injury it is necessary to determine burn depth precisely. If it is about an epidermal or superficial thermal burn, treatment continues Argezin. Deep thermal or subdermalny burns treat Argezin before transplantation.

At out-patient treatment of bandaging of the struck surfaces do daily during the first several days, and then as required. Treatment in stationary conditions has to be followed by daily bandaging which is recommended to be done in the morning.

Argezin's application is painless, cream does not leave spots on clothes and bed linen.


Features of use:

Patients should appoint medicine with care with hypersensitivity to streptocides owing to possible emergence of allergic reactions, and also patients with inborn deficit have dehydrogenase glyukozo-6-phosphate as putting cream on the big surface of skin can lead to hemolysis.


Side effects:

At long treatment of extensive burn wounds serumal concentration of Sulfadiazinum can reach the same values, as at system therapy (8-12 mg of %). As similar side effects are possible, as well as at the general use of streptocides, it is necessary to check concentration of streptocides in serum. Such researches are necessary also at a heavy renal and liver failure.


Interaction with other medicines:

Data on influence of Sulfadiazinum of silver on action of other medicinal are absent.


Contraindications:

Cream use Argezin is forbidden at premature and newborn children as the risk of developing of a kernicterus of a brain is high. Argezin's use at pregnancy is not recommended and is reasonable only when over 20% of skin or in case health and mother's life in bigger danger, than health of a fruit are burned.

Prescription of medicine is contraindicated at pochechno - a liver failure, a porphyria, deep purulent wounds and burns with the expressed exudation.


Overdose:

Now about overdose cases at topical administration of Sulfadiazinum of silver it was not reported.


Storage conditions:

To store at a temperature not above 25 °C. To store in the place, unavailable to children. Period of validity 2 years.
