Intestines lymphoma

Description:

Lymphoma of a small bowel represent heterogeneous group of diseases. According to modern classification they are subdivided into V-cellular and T-cellular lymphoma. In turn among V-cellular lymphoma there can be localized and diffusion forms.

The localized forms of the V-cellular lymphoma of a small bowel (so-called lymphoma of the western type) widespread it is preferential in countries of Western Europe and the United States of America, proceed with clinical symptoms of intestinal impassability, intestinal bleedings more often, sometimes perforation of a small bowel and seldom shown by a diarrheal syndrome. On the contrary, diffusion forms of the V-cellular lymphoma, and also the T-cellular lymphoma of a small bowel proceed often with heavy diarrhea and Development of a classical sprue.

Intestines Lymphoma symptoms:

Primary diffusion V-cellular lymphoma of a small bowel

Primary diffusion V-cellular lymphoma of a small bowel (synonym: the immunoproliferative disease of a small bowel) is characterized diffusion lymphoid плазмоклеточной by infiltration of proximal departments of a small bowel and mezenterialny lymph nodes. At early stages of the development the diffusion V-cellular lymphoma is characterized by infiltration of a mucous membrane of a small bowel and mezenterialny lymph nodes mature plasmocytes. Further, however, the disease can progress and pass into a malignant (immunoblast) form with the advent of unripe cells, distribution of infiltration to a submucosal layer and destruction of structure of lymph nodes.

As the main symptom in a clinical picture of a diffusion V-cellular lymphoma serves the diarrhea leading to a sprue (sometimes to an exudative enteropathy) and loss of body weight. The mechanism of development of diarrhea in patients is connected with disturbance of processes of absorption because of diffusion damage of a mucous membrane and obstruction of absorbent vessels of a wall of a gut, accession of a giardiac infection (at every third patient) and a syndrome of excess reproduction of bacteria. Fever, changes of fingers of hands in the form of "drum sticks", gepato-and a splenomegaly can be other symptoms of a disease.

The T-cellular lymphoma of a small bowel associated with an enteropathy

The T-cellular lymphoma associated with an enteropathy (the old name "malignant histiocytosis") usually develops at patients with a mucosal atrophy of a small bowel (most often at the patients suffering local to a spr). The peak of incidence falls on the age period from 50 to 80 years. The risk of development of the T-cellular lymphoma increases at patients with the diagnosis local to a spr, made already at advanced age. Most often at the T-cellular lymphoma the jejunum is surprised.

The clinical picture of the T-cellular lymphoma of a small bowel (diarrhea, abdominal pains, decrease in body weight, etc.) often does not differ from that at preceding local to a spr. Emergence of a lymphoma suspect of such cases at a refrakternost of clinical symptoms a spra to a classical aglyutenovy diet.

Intestines Lymphoma reasons:

In blood serum find pathological immunoglobulin in most of patients (heavy and - IgA chain). This fact explains the old name of a disease - "a disease heavy and - chains". The diffusion V-cellular lymphoma of a small bowel meets preferential in the countries of the Mediterranean Basin (from here one more name - "the Mediterranean lymphoma"), the Central and South Africa, South America. The disease is more often noted at persons with the low social and economic standard of living, especially at having intestinal parasitic infections.

Treatment of the Lymphoma of intestines:

Treatment of primary diffusion V-cellular lymphoma of a small bowel. In initial stages of a disease (existence lymphoid плазмоклеточной infiltrations without formation of a lymphoma) the long therapy by tetracycline giving long clinical and histologic remission is effective. In the developed stage of process the combined chemotherapy in combination with radiation therapy is applied. Life expectancy of such patients fluctuates from several months to several years, averaging 20 months.
Treatment of the T-cellular lymphoma of the small bowel associated with an enteropathy. At the localized form of a tumor radical operation - an enterectomy with the subsequent radiation and aggressive chemotherapy is possible. The forecast is usually adverse though cases of long postoperative survival of patients are known.
