Heparin

General characteristics. Structure:

Active ingredient: 1000 ME heparins in 1 g of gel.

Pharmacological properties:

Pharmacodynamics. Heparin gel when drawing on skin interferes with formation of blood clots, has moderate antiinflammatory, anti-exudative effect, and has local analgeziruyushchy effect. Blocks formation of thrombin, activity of hyaluronidase oppresses, activates fibrinolitic properties of blood.

The heparin which is gradually released from gel has antitrombotichesky effect and reduces inflammatory process, improves microcirculation and activates fabric exchange, thanks to it accelerates processes of a rassasyvaniye of hematomas and blood clots and reduction of puffiness of fabrics.

Pharmacokinetics. Heparin badly gets through the unimpaired skin and mucous membranes thereof drug practically does not possess systemic resorptive action.

Heparin after absorption biotransformirutsya in a liver (partially by enzyme a hepatic haparinase) and in reticuloendothelial system. Plasma elimination half-life makes 30-60 min.

Indications to use:

Gel of heparin is applied at thrombophlebitis of extremities (preferential superficial), fibrinferment of hemorrhoidal veins, phlebitis after repeated intravenous injections, elephantiasis, a superficial periphlebitis, a lymphangitis, superficial mastitis, the localized infiltrates and hypostases, injuries and bruises (including muscular tissue, sinews, joints), a hypodermic hematoma.

Route of administration and doses:

Thrombophlebitis of extremities, phlebitis after repeated intravenous injections. Gel is applied with a thin layer (0,5-1 g on the site diameter to the 3-5th) on area of defeat and carefully rub gel in skin.

Thrombosis of hemorrhoidal veins. Gel is applied on byazevy or linen laying which is imposed directly on hemorrhoidal nodes and fixed a bandage. With the same purpose it is possible to use the tampon impregnated with heparin gel which is entered into an anus. Course 3-4 days.

Gel is applied by 2-3 times a day daily before disappearance of the inflammatory phenomena, on average from 3 to 14 days. The possibility of carrying out longer course of treatment is defined by the doctor.

Features of use:

Use at pregnancy is possible only according to indications, under medical control. Use at a lactation is possible — according to indications.

Side effects:

Local allergic reactions and dermahemia are in some cases possible.

At use of drug in the recommended doses of toxic effects it is not revealed.

Interaction with other medicines:

Anticoagulative effect of heparin amplifies at simultaneous use of anticoagulants, antiagregant and NPVS. Ergot alkaloids, thyroxine, tetracycline, antihistamines and nicotine reduce effect of heparin.

Contraindications:

Use of gel of heparin is contraindicated at ulcer and necrotic processes in a thrombophlebitis zone, traumatic disturbances of integrity of integuments, and also at the lowered coagulability of blood, thrombocytopenia. Hypersensitivity to gel components.

Overdose:

Due to the small system absorption the overdose is improbable. So far the overdose phenomena when using gel of heparin are not described.

Symptoms. At prolonged use on big surfaces hemorrhagic complications are possible. 

Treatment: drug withdrawal. If necessary apply the antagonist of heparin — protamin sulfate (1% solution).
