Ftalazolum

General characteristics. Structure:

1 tablet contains phthalylsulfathiazole (Ftalazolum) in terms of 100% substance of 0,5 g; excipients: potato starch, talc, calcium stearate.

Pharmacological properties:

Pharmacodynamics. Sulfanamide drug of a broad spectrum of activity, is active rather gram-positive and gram-negative microorganisms – causative agents of intestinal infections. Renders the bacteriostatic effect caused by disturbance of formation of growth factors of microorganisms – the folic and degidrofoliyevy acids necessary for synthesis of purines and pyrimidines.

Pharmacokinetics. Drug is practically not soaked up from digestive tract, bioavailability makes it 5–10%. The main quantity it is late in intestines where an active sulfanamide part of a molecule – sulphathiazole is chipped off. A part of the adsorbed drug is acetylated in a liver and removed with urine. High concentration of sulphathiazole in intestines taking into account specific bacteriostatic activity of drug concerning an indestinal flora cause high performance of Ftalazola-KMP at intestinal infections.

Indications to use:

Shigellosis, colitis (including ulcer), coloenterites, an amebiasis, intestines operations (for prevention of complications).

Route of administration and doses:

At a shigellosis to adults and children 12 years are more senior appoint: in the first 2 days of treatment – on 6 g a day (on 1 g each 4 hours), for the 3-4th day – on 4 g a day (on 1 g each 6 hours), for the 5-6th day – on 3 g a day (on 1 g each 8 hours). The course dose makes 25–30 g. In 5–6 days after the first course of treatment conduct the second year: the first 2 days – on 1 g in 4 hours (at night – in 8 hours), only 5 g a day; the 3-4th day – on 1 g in 4 hours (do not appoint at night), only 3 g a day. The course dose makes 21 g (at easy disease the dose can be reduced to 18 g). The highest doses for adults inside: one-time – 2 g, daily – 7 g. Children up to 3 years are recommended to appoint drug in other dosage form. To children from 3 to 7 years appoint 0,4 g to reception, from 7 to 12 years – 0,75 g on reception 4 times a day. Course of treatment
– 7 days. At treatment of other diseases Ftalazolum is appointed by the adult in the first 2–3 days on 1–2 g by each 4–6 hours, in the next 2–3 days – half doses.

Features of use:

Drug gets through a placenta, and in researches on animals its adverse action on a fruit therefore use of drug at pregnancy is not recommended is revealed. Ftalazolum gets into breast milk and can cause a kernicterus in the children who are on breastfeeding and also hemolitic anemia in children with deficit glyukozo-6-fosfatdegidrogenazy. Drug with care is appointed at diseases of kidneys.

Side effects:

At individual hypersensitivity to streptocides allergic reactions are possible.

Interaction with other medicines:

Depending on the nature of a disease Ftalazolum can be applied in combination with antibiotics. Along with drug it is reasonable to appoint streptocides which are well soaked up (Sulfadimezinum, Etazolum, Etazolum sodium, etc.). It is incompatible with salicylates, PASK, dipheninum (strengthening of toxicity of Ftalazolum), Oxacillinum (decrease of the activity of Oxacillinum), nitrofurans (increase in risk of development of anemia and a methemoglobinemia), drugs men's and female sex hormones (oppression of function of gonads), Calcii chloridum and vitamin K (decrease in a blood coagulation).

Contraindications:

Individual sensitivity to drug, a bazedova a disease, blood diseases, acute hepatitises, pregnancy, feeding of a breast. Drug is contraindicated to patients with an allergy to furosemide, thiazide diuretics, inhibitors of a karboangidraza and derivatives of sulphonylurea. Ftalazolum is contraindicated to children up to 2 months. The exception makes an inborn toxoplasmosis at which streptocides are applied according to vital indications.

Overdose:

In case of overdose emergence of a macrocytosis and pancytopenia is possible that is caused by deficit of folic acid. It it is possible to warn purpose of folic acid or calcium of a folinat.

Storage conditions:

In the dry, protected from light place at a temperature from 15 ºС to 25 ºС. A period of validity – 5 years. To store in the place, unavailable to children!
