General characteristics. Structure:

Active agent: 35 mg of a propafenon of a hydrochloride.

Pharmacological properties:

Propafenon treats antiarrhytmic means of Ic of a class (on Vaughan-Williams classification). Is a blocker of natrium channels and a non-selective blocker of beta adrenoceptors. Action of a propafenon, its side effects and interactions depend on a biotransformational phenotype of the patient. The elimination half-life considerably differs at persons with bystry biotransformation (t½ 2 — 10 hours) and at persons with slow biotransformation (t½ 12 — 32 hours).

Indications to use:

Symptomatic conditions of the takhikardialny supraventricular arrhythmia demanding therapy i.e. atrioventricular nodal tachycardia, supraventricular tachycardia at a WPW-syndrome (WPW) or paroxysmal fibrillation of auricles. A heavy symptomatic ventrikulyarny tachyarrhythmia if the doctor considers a condition of the patient life-threatening.

Route of administration and doses:

The dosage has to be picked up individually on the basis of monitoring of an ECG and arterial pressure. In case of use of infusion careful control of cordial activities for an ECG (the QRS complex, intervals of PR and QTC) and blood circulatory system indicators is necessary. A single dose — 1 — 2 mg/kg. Intravenous injections have to be entered slowly within 3 — 5 minutes, the interval between separate injections should not be shorter than 90 — 120 minutes. If the QRS complex extends or QT interval which is depending on change of ChSS will be extended it is more, than for 20%, it is necessary to stop intravenous administration of drug immediately. Short-term infusion: within 1 — 3 hours of 0,5 — 1 mg/min. A slow intravenous infuziya:maksimalny daily dose of 560 mg (there correspond 160 ml of drug Propanorm of 35 mg / 10 ml solution for injections). For preparation of infusion it is necessary to use 5% glucose solution.

Features of use:

Drug can cause dizziness, thereby it is adverse to influence the activity demanding the increased concentration of attention. Indications and a dose of drug need especially to be considered at patients with a pacemaker. It is necessary to conduct careful clinical and laboratory observation of the patients accepting it is long therapy by anticoagulants. If during therapy there are a SA blockade, AV blockade of the highest degree, repeated polymorphic premature ventricular contraction, treatment needs to be suspended.

Side effects:

Cardial: bradycardia, AV blockade, blockade of legs of ventriculonectors, hypotension.

Not cardial: nausea, vomiting, dryness in a mouth, bitterness in a mouth, a lock, an abdominal pain, increased fatigue, dizziness, a syncope. Seldom: headache, cholestatic jaundice or allergic skin reaction; in exceptional cases: leukopenia, thrombocytopenia.

Interaction with other medicines:

At a concomitant use of a propafenon and a metoprolol, warfarin, cyclosporine, theophylline and digoxin the level of these medicines in a blood plasma increases. Considerable strengthening of effect of a propafenon arises in a combination with local anesthetics (anesthesia in stomatology!). Rifampicin and phenobarbital reduce effect of a propafenon. The combination with Amiodaronum increases risk of side effects from heart, the combination with tricyclic antidepressants and ritonaviry is also not recommended.

Contraindications:

Hypersensitivity to a propafenon or excipients. At the expressed structural changes of a myocardium (uncontrollable congestive heart failure with fraction of emission of a left ventricle <35%; cardiogenic shock except for the shock caused by arrhythmia); heavy symptomatic bradycardia; a sick sinus syndrome, sinuatrial blockade, an atrioventricular block of the II—III degree, blockade of legs of ventriculonectors or an intraventricular block in the absence of a pacemaker; the expressed hypotension; manifestation of an imbalance of electrolytes (for example, potassium metabolism disturbance); serious obstructive bronchopulmonary illness. During pregnancy and feeding a breast to appoint only if the expected effect exceeds possible risk for a fruit.

Overdose:

There are no data on overdose.
