Cordiaminum 2,0ml N10

General characteristics. Structure:

International and chemical names: nikethamide; niacin diethyl amide; 
main physical and chemical properties: the transparent, colourless or slightly painted liquid, with a characteristic smell;
structure: 1 ml of solution contains Nikethamidum 0,25 g; 
excipients: water for injections.

Pharmacological properties:

Pharmacodynamics. Cordiaminum concerns to group of analeptical means. Has a promoting effect on TsNS, and especially on the respiratory and vasomotor centers (at their reduced tone). The mechanism of action consists of two components - central and peripheral. Central it is connected with the direct influence on a vasomotor center of a myelencephalon leading to its excitement and the mediated increase in the system ABP (especially at initial oppression of this center). The peripheral component of the mechanism of action is connected with excitement of chemoceptors of a carotid sine that leads to increase in frequency and depth of respiratory movements. Does not exert direct impact on cardiovascular system.

Pharmacokinetics. Absorption – good, does not depend on a way of introduction. Is exposed to bystry biotransformation in a liver with formation of inactive metabolites. It is excreted, generally by kidneys.

Indications to use:

Collapse, asphyxia, depressed case at operative measures and during the postoperative period, acute and chronic disorder of blood circulation, decrease in a tone of vessels, respiratory depression at infectious diseases and in the period of reconvalescence. 

Route of administration and doses:

Cordiaminum is entered subcutaneously, intramusculary, intravenously. Subcutaneously, intramusculary and intravenously adult enter in a dose 1-2 ml, 1-3 times a day. To children subcutaneously, depending on age, enter: till 1 year – 0,1 ml; from 1 year to 4 years – 0,15-0,25 ml; 5-6 years – 0,3 ml; 7-9 years – 0,5 ml; 10-14 years – 0,8 ml. Intravenous administration is carried out slowly.
At intoxication drugs, analgetika, somnolent the adult intravenously slowly enter 3-5 ml of drug (it is possible to enter also under skin and into muscles).
The highest doses for adults subcutaneously: one-time – 2 ml, daily – 6 ml. 
The highest single dose for hypodermic and intravenous administration at poisoning with drugs makes 5 ml.

Features of use:

As subcutaneous and intramuscular injections of Cordiaminum are painful, for reduction of pains, depending on a situation, into the place of an injection previously enter novocaine (to adult – 1 ml of 0,5% of solution, children – according to age).
As there are no data on safety of use for treatment of pregnant women and the nursing women, care at purpose of Cordiaminum is necessary for these categories of patients. 

Side effects:

The concern, muscular twitchings (beginning with circular muscles of a mouth), dermahemia of the person, itch of integuments, vomiting, arrhythmias; infiltration and morbidity in an injection site, allergic reactions.

Interaction with other medicines:

Strengthens effects of psychostimulants, antidepressants. Reduces action of narcotic analgetik, somnolent, antipsychotic means, anxiolytics, anticonvulsants. The effect decreases PASK, derivatives of a fenotiazin.

Contraindications:

Hypersensitivity, predisposition to spasms, epilepsy, tonikoklonichesky spasms (in the anamnesis), a porphyria, a hyperthermia at children.

Overdose:

The overdose is followed by strengthening of side effect of drug, development of toniko-clonic spasms is possible. 

Storage conditions:

To store in protected from light and the place, unavailable to children, at a temperature not above 25 °C. 
Lines of storage - 5 years.
