General characteristics. Structure:
Active agent: Acetylcysteinum of 20 mg/ml
Other ingredients: methylparahydroxybenzoate, пропилпарагидроксибензоат, sorbitol, sodium citrate, fragrance.
Pharmacological properties:
AZZ Children's — mucolytic, the expectorant applied to fluidifying of a phlegm at the diseases of respiratory system which are followed by formation of dense slime. Acetylcysteinum is derivative amino acids cysteine. The drug AZZ Children's works sekretolitichesk and increases motility of a respiratory path. The mucolytic effect of AZZ Children's has the chemical nature. Due to existence of free sulphhydryl group Acetylcysteinum terminates disulfide bridges of acid mucopolysaccharides that leads to a depolymerization of mukoproteid of a purulent phlegm. As a result of it the phlegm becomes less viscous.
Drug has also antioxidant pneumotyre-tread properties that is caused by binding of chemical radicals by its sulphhydryl groups and, thus, their neutralization. Besides, AZZ Children's promotes increase in synthesis of glutathione — an important factor of a chemical detoxication. This feature of Acetylcysteinum provides its efficiency at acute poisonings with paracetamol and other toxics (aldehydes, phenols).
After oral administration Acetylcysteinum quickly and completely is soaked up and exposed to metabolism in a liver with formation of cysteine, pharmacological an active metabolite, and also diacetylcysteinum, cystine, and further — the mixed disulfides. Bioavailability very low — about 10%. Linkng with proteins of a blood plasma — about 50%. Acetylcysteinum is allocated with kidneys in the form of inactive metabolites (inorganic sulfates, diacetylcysteinum).
T½ is defined, mainly, bystry biotransformation in a liver and makes about 1 h.
Indications to use:
The acute and chronic diseases of bronchopulmonary system which are followed by the increased formation of a phlegm with deterioration in expectoration; acute and chronic bronchitis; bronchietasia; chronic obstructive bronchitis.
Route of administration and doses:
To adults and children 14 years are aged more senior appoint 2 measured spoons (10 ml) of solution 2–3 times a day (there correspond 400–600 mg of Acetylcysteinum a day).
To children at the age of 6–14 years appoint on 1 measured spoon (5 ml) of solution 3–4 times a day (there correspond 300–400 mg of Acetylcysteinum a day).
To children at the age of 2–5 years appoint on 1 measured spoon (5 ml) 2–3 times a day (there correspond 200–300 mg of Acetylcysteinum a day).
To children under 2 years appoint ½ measured spoons (2,5 ml) of solution 2–3 times a day (there correspond 100–150 mg of Acetylcysteinum a day).
Drug should be accepted only in the form of the prepared solution after food.
Solution preparation
To open a bottle, having pressed a cap and having at the same time turned it to the left. To pour cold water to a tag (deepening in glass). To close a bottle. To stir up strongly. Again to add waters to a tag and to stir up. To repeat this procedure until the volume of solution is not brought to a tag.
Duration of use of drug is defined by the doctor and should not exceed 4–5 days.
Features of use:
At patients with stomach ulcer or a duodenum Acetylcysteinum should be applied with care.
Drug should be used with care at patients with OH. This results from the fact that in preparation time of solution there can be a reflex bronchospasm as powder at preparation of solution can get to the inhaled air therefore the mucous membrane of a nose is irritated.
During treatment Acetylcysteinum recommends to use enough liquid.
Patients with rare hereditary intolerance of fructose should not accept drug.
The reference for patients with a diabetes mellitus and patients with inborn hypersensitivity to fructose
10 ml (2 measured spoons) of solution, ready to use, contain 3,7 g of sorbitol (a source of 0,93 g of fructose) that there correspond 0,31 grain units.
Sorbitol can have easy laxative effect.
Use during pregnancy or feeding by a breast
For today there are no sufficient data concerning use of drug during pregnancy and feeding by a breast therefore it is possible to appoint drug in case the expected advantage for mother exceeds potential risk for a fruit or the child.
Ability to influence speed of response at control of vehicles or other mechanisms
There are no data on negative influence on ability to manage vehicles or to work with difficult mechanisms.
Children
Children aged till 1 year should appoint Acetylcysteinum only according to vital indications; to carry out treatment under strict observation of the doctor. Children under 2 years can appoint Acetylcysteinum only under observation of the doctor.
Side effects:
Diarrhea, nausea, heartburn, vomiting are possible.
Sometimes noted a headache, an inflammation of a mucous membrane of a mouth, a sonitus.
Separate cases of allergic reactions, spasm of bronchial tubes (at patients with hypersensitivity of bronchial system are recorded, for example, at OH), skin rash which quickly passes of an itch, tachycardias, and also decrease in the ABP.
Metilparagidroksibenzoat and пропилпарагидроксибензоат can become the reason of separate hypersensitivity reactions.
At the first signs of hypersensitivity to drug its use needs to be stopped.
Interaction with other medicines:
Antibiotics of a tetracycline row (except for doxycycline) are not recommended to be applied along with Acetylcysteinum.
Cases of an inactivation of antibiotics of other groups Acetylcysteinum noted only during in vitro experiments at direct mixing of the last. But for safety of the patient the interval between reception of antibiotics and Acetylcysteinum has to make not less than 2 h.
At simultaneous use of drug with antibechics in connection with decrease in a tussive reflex dangerous stagnation of slime is possible.
Simultaneous use of nitroglycerine with Acetylcysteinum can lead to strengthening of vazodilatatorny effect of nitroglycerine.
Incompatibility with other medicines
In vitro incompatibility is revealed with some semi-synthetic penicillin, tetracyclines, cephalosporins, and also aminoglycosides. There are no data on incompatibility with such antibiotics as amoxicillin, erythromycin, tsefuroksy.
Contraindications:
Hypersensitivity to Acetylcysteinum or other components of drug. Hepatitis, a renal failure (in order to avoid increase in nitrogen-containing substances in an organism).
Overdose:
Still cases of development of heavy and life-threatening side effects, even are noted at considerable overdose. Nausea, vomiting and diarrhea are in some cases possible. For children of chest age there is a risk of hypersecretion.
Symptomatic treatment.
Storage conditions:
At a temperature not above 30 °C. To store ready solution at a temperature of 2-8 °C no more than 12 days.
