General characteristics. Structure:

One sparkling tablet contains active agent: Acetylcysteinum of 200 mg or 600 mg
excipients: citric acid anhydrous 843,03 / 648,99 mg, Natrii hydrocarbonas 695,64 / 548,72 mg, lemon fragrance 100,00 / 100,00 of mg, adipic acid 100,00 / 41,82 of mg, adipic acid finely dispersed 20,00 / 20,00 mg, mg povidone 21,33 / 20,47, mg aspartame 20,00 / 20,00.

Description: Round flat tablets, white or white with a yellowish shade, with a lemon smell. On one party of a tablet of risk.

Pharmacological properties:

Pharmacodynamics. Acetylcysteinum shows sekretolitichesky and sekretomotorny actions in the field of a bronchial path. Liquefies a phlegm, increasing its volume, facilitates its department. Keeps the activity and in the presence of a purulent phlegm.
The mechanism of action is based on ability of sulphhydryl groups of Acetylcysteinum to terminate disulfide bridges of acid mucopolysaccharides of a phlegm that leads to depolarization of mucoproteins and reduction of viscosity of slime.
Promotes increase in synthesis of glutathione which is an important antioxidant factor of intracellular protection and provides maintenance of functional activity and morphological integrity of a cell that, in particular, explains its efficiency as antidote at poisonings with paracetamol.
Thanks to ability of sulphhydryl group to neutralize electrophilic oxidizing toxins, Acetylcysteinum has antioxidant effect.
Possesses also some antiinflammatory action (due to suppression of formation of the free radicals and active oxygen-containing agents responsible for development of an inflammation in pulmonary fabric).

Pharmacokinetics. At intake Acetylcysteinum is well soaked up from digestive tract. Substantially is exposed to effect of "the first passing" through a liver, it is metabolized with formation of cysteine, diacetylcysteinum, cystine and various disulfides that reduces bioavailability to 10%. The maximum concentration of Acetylcysteinum in a blood plasma is reached in 1-3 hours, at patients with an abnormal liver function - in 8 hours.
Therapeutic action is observed in 30-90 min. and remains within 2-4 hours.
It is removed generally by kidneys in the form of inactive metabolites, an insignificant part is allocated in not changed view with fecal masses.
Gets through a placental barrier, collects in amniotic liquid.

Indications to use:

Diseases of a respiratory organs and states which are followed by formation of the viscous hardly separated mucopurulent phlegm:
• acute and chronic bronchitis;
• tracheitises owing to a bacterial and/or viral infection;
• bronchiolitis;
• pneumonia;
• bronchial asthma;
• bronchoectatic disease;
• atelectases owing to obstruction of bronchial tubes a mucous stopper;
• a mucoviscidosis (as a part of a combination therapy);
• removal of a viscous secret from respiratory tracts at posttraumatic and postoperative states;
• catarral and purulent otitis, antritis, sinusitis (simplification of an otkhozhdeniye of a secret).
Drug is used at overdose treatment by paracetamol.

Route of administration and doses:

Inside, after food, previously having dissolved tablets sparkling in glass with water. A pill sparkling should be taken right after dissolution.
The following dosages are usually recommended:
Tablets sparkling 600 mg
adults and teenagers are more senior than 14 years: The 7th tablets sparkling 2 once a day or 1 tablet sparkling once a day (600 mg of Acetylcysteinum a day).
Tablets sparkling 200 mg

• adults and teenagers are more senior than 14 years: 1 tablet sparkling 2-3 once a day (400-600 mg of Acetylcysteinum a day);
• children aged from 6 up to 14 years: 1 tablet sparkling 2 once a day (400 mg of Acetylcysteinum a day);
• children aged from 2 up to 6 years: The 7th tablets sparkling 2-3 once a day (200-300 mg of Acetylcysteinum a day).
Treatment of a mucoviscidosis: - children are more senior than 6 years: 1 tablet sparkling 3 once a day (600 mg of Acetylcysteinum a day); - children aged from 2 up to 6 years: The 7th tablets sparkling 4 once a day (400 mg of Acetylcysteinum a day).
Duration (continuity) of use depends on features of a disease. At therapy of chronic bronchitis and a mucoviscidosis treatment can be long (up to several months).

Features of use:

At use of drug for patients with bronchial asthma it is necessary to provide a phlegm drainage, to combine with reception of bronchial spasmolytics.
During drug use by the patient the plentiful use of liquid is recommended that supports sekretolitichesky action of drug.
At use of drug it is necessary to use glasswares, to avoid contact of drug with metal, rubber, oxygen, the easily oxidized substances.
Each tablet sparkling contains 20 mg of aspartame (it is equivalent to the content of 11,2 mg of phenylalanine), owing to what drug cannot be used sick with a fenilketonuriya.

Side effects:

From the alimentary system: seldom - heartburn, nausea, vomiting, diarrhea, feeling of overflow of a stomach; the bleeding which is partially associated with hypersensitivity reaction.
Allergic reactions: very seldom - skin rash, an itch, a small tortoiseshell, tachycardia, a lowering of arterial pressure, a bronchospasm (it is preferential at patients with hyperreactivity of bronchial tubes).
Others: seldom - a headache, nasal bleedings, a rhinorrhea, a sonitus, drowsiness, stomatitis, fever.
At emergence of undesirable side effects it is necessary to see a doctor.

Interaction with other medicines:

At simultaneous use of Acetylcysteinum and antibechics stagnation of a phlegm because of suppression of a tussive reflex therefore the similar combined treatment has to be carried out only under straight medical vision can amplify.
There are data that the thiol group of Acetylcysteinum can neutralize activity of some antibiotics (Amphotericinum In, ampicillin, tetracyclines, excepting doxycycline, semi-synthetic penicillin, cephalosporins, aminoglycosides). Therefore oral administration of these antibiotics in 2 hours after reception of Acetylcysteinum is reasonable.
It is established also that such antibiotics as amoxicillin, doxycycline, erythromycin, тиамфеникол, tsefuroksy do not interact with Acetylcysteinum.
There are messages that the concomitant use of Acetylcysteinum and nitroglycerine can lead to strengthening of vasodilating action of the last and reduction of aggregation of thrombocytes.
Acetylcysteinum reduces a hepatotoxic action of paracetamol.

Contraindications:

• the known hypersensitivity to Acetylcysteinum or other ingredients of a ready dosage form;
• pregnancy, lactation period;
• a peptic ulcer of a stomach and duodenum in an aggravation phase;
• fenilketonuriya;
• children up to 14 years (for tablets of 600 mg);
• children up to 2 years (for tablets of 200 mg).
With care
With extra care apply Acetylcysteinum at patients with bronchial asthma, diseases of a liver, kidneys, dysfunction of adrenal glands, a gullet varicosity, at persons inclined to pulmonary bleedings, a pneumorrhagia, fenilketonuriy, arterial hypotension.

Overdose:

So far overdose cases are not described by Acetylcysteinum drugs at oral administration. In a dose of 500 mg/kg Acetylcysteinum did not cause poisoning symptoms. Are theoretically possible: diarrhea, heartburn, nausea, vomiting, stomach ache.
Treatment: symptomatic.

Storage conditions:

Period of validity 2 years. Not to apply after the expiration specified on packaging! In the dry place, at a temperature not above 25 °C. To store in the place, unavailable to children!
