General characteristics. Structure:
International and chemical names: Theophylline; main physical and chemical properties: tablets of color, white or white with a yellowish shade, with scarcely noticeable impregnations, inodorous; structure: 1 tablet contains theophylline 0,3 g; excipients: composition polymeric carrier, calcium stearate.
Pharmacological properties:
Pharmacodynamics. Teopek as active ingredient contains theophylline and has pharmacological properties, characteristic of it: stimulates TsNS, strengthens sokratitelny activity of a cardiac muscle, moderately expands coronary, and also renal and other peripheral vessels, stimulates a respiratory center, moderately strengthens a diuresis, oppresses aggregation of thrombocytes and a liberation of mediators of an allergy. The most important property of drug – bronchodilatory action.
The mechanism of action is connected with inhibition of phosphodiesterase and increase in maintenance of cyclic AMF in smooth muscle cells. Drug influences intracellular distribution of calcium, reduces release of a histamine and leukotrienes from mastocytes, inhibits synthesis and activity of prostaglandins, increases release of endogenous catecholamines. Due to increase in a physical activity of the ciliary epithelium of a trachea and bronchial tubes stimulating influences on a respiratory center and bronkhodilatiruyushchy action drug improves alveolar ventilation.
Pharmacokinetics. Thanks to a theophylline combination to the composition polymeric carrier which is a part of tablets drug possesses the prolonged action that provides therapeutic concentration of theophylline in blood serum at double administration of drug in days.
The theophylline combination to the polymeric carrier promotes the dosed release of alkaloid in a digestive tract. The maximum of concentration of theophylline in a blood plasma is noted in 3-6 hours after Teopek's reception inside. The general duration of stay of theophylline in plasma is equal to 12-24 hours. Therapeutic concentration of theophylline is equal in a blood plasma approximately 15 mkg/ml. Drug in a liver is metabolized. Values of an elimination half-life are very variable. 10% of the accepted dose are removed in not changed view with urine. Release of theophylline at Teopek's reception is increased at congestive heart failure, cirrhosis, at smokers.
Indications to use:
Teopek appoint the adult as broncholitic means at different forms of a bronchospasm, in particular at bronchial asthma, chronic obstructive bronchitis, emphysema of lungs.
Route of administration and doses:
Teopek accept inside, after food, washing down with water. The tablet can be halved, but it is impossible to make small, including to chew or dissolve in water.
In the first 1-2 days the adult drug is appointed on 0,15 g (1/2 tablets), by 1-2 times a day, with an interval between receptions of 12-24 hours. In the next days the single dose is increased to 0,3 g, by 2 times a day.
Treatment duration - 2-3 months. However it is necessary to consider that optimum doses and duration of administration of drug need to be selected individually, being guided by character of a course of a disease, the accompanying pathological processes, the nature of complex therapy.
Features of use:
It is necessary to be careful at purpose of drug to patients with the ischemic bolezyyu of heart expressed by arterial hypertension, a renal failure and/or a liver.
Safety and efficiency of use of drug during pregnancy and in the period of a lactation are not established.
Clinical data on Teopek's use of the prolonged action for treatment of children are absent.
Side effects:
At Teopek's use dizziness, a headache, tachycardia, concern, sleep disorders, anorexia, nausea, vomiting, pains in a stomach are possible. In such cases the dose is reduced or drug is cancelled. Development of side effects is more probable at coronary insufficiency, tendency to tachycardia, at epileptic seizures, chronic gastritis and colitis, a liver failure.
Interaction with other medicines:
Teopek is incompatible with anticonvulsants and barbiturates, ephedrine, methylxanthines, contraceptive hormonal drugs, antibiotics macroleads, furosemide, propranolol, verapamil. The majority of the specified drugs owing to increase in concentration of theophylline in blood can strengthen side effect of the last, and barbiturates reduce its content in blood.
Contraindications:
Drug is contraindicated at individual hypersensitivity, hyperfunction of a thyroid gland, an acute myocardial infarction, a subaortal stenosis, premature ventricular contraction, epilepsy and other convulsive states, pregnancy and in the period of a lactation. With care it is necessary to appoint at a peptic ulcer of a stomach and duodenum.
Drug is contraindicated to children up to 14 years.
Overdose:
So far about cases of overdose of Teopek it was not reported.
Storage conditions:
To store in dry, protected from light and the place, unavailable to children, at a temperature not above 25ºС.
Period of storage - 4 years.
