General characteristics. Structure:

Deytvuyushchy substance: 300 mg of Acetylcysteinum in 1 ampoule.

Excipients: dinatrium эдетат, sodium a hydroxide, water for injections.

Pharmacological properties:

Pharmacodynamics. Acetylcysteinum has the effect liquefying a phlegm. Existence in structure of Acetylcysteinum of sulphhydryl groups promotes a rupture of disulfide bridges of acid mucopolysaccharides of a phlegm that leads kdepolyarization of mukoproteid and to reduction of viscosity of slime. Drug keeps activity in the presence of a purulent phlegm.

Acetylcysteinum has the antioxidant effect caused by existence of nucleophilic thiol SH group which easily gives hydrogen, neutralizing oxidizing radicals.

At preventive use of Acetylcysteinum reduction of frequency and weight of aggravations at patients with chronic bronchitis and a mucoviscidosis is noted. The protective mechanism of Acetylcysteinum is based on ability of its reactive sulphhydryl groups to connect chemical radicals.

Acetylcysteinum easily gets in a cell, is deacetylated to L-cysteine from which intracellular glutathione is synthesized. Glutathione - high-reactive tripeptide, powerful antioxidant, the cytoprotector catching endogenous and exogenous free radicals and toxins. Acetylcysteinum warns exhaustion and promotes increase in synthesis of the intracellular glutathione participating in oxidation-reduction processes of cells thus promoting a detoxication of harmful substances. Action of Acetylcysteinum as an antidote at poisoning with paracetamol is explained by it.

Pharmacokinetics. At intravenous administration of 600 mg of Acetylcysteinum the maximum concentration in plasma makes 300 mmol/l, plasma elimination half-life - 2 hours. The general clearance is equal to 0,21 l/h/kg, and distribution volume at the plateau is equal to 0,34 l/kg.

Acetylcysteinum gets into intercellular space, is preferential distributed in a liver, kidneys, lungs, a bronchial secret. Acetylcysteinum and its metabolites are removed, generally by kidneys.

Indications to use:

Флуимуцил® apply at diseases of the respiratory organs which is followed by formation of a phlegm of the increased viscosity: acute and chronic bronchitis, pneumonia, bronchietasia, mucoviscidosis, bronchial asthma.

Route of administration and doses:

Parenteral administration: the adult enter 1 ampoule (300 mg / 3 ml) deeply intramusculary or intravenously 1-2 times a day if the doctor it is not offered differently. To children of 6-14 years enter 1/2 doses of adults. The daily dose for children is younger than 6 years - 10 mg/kg of weight. Duration of treatment has to be established by results of change of a condition of the patient. High local and general tolerance to drug allows to conduct long courses of treatment.

Inhalations: on 1 ampoule 1-2 times a day if the doctor it is not offered differently, within 5-10 days or longer courses. Owing to high safety of drug the frequency of acceptance and relative size of a dose can be changed by the doctor in acceptance limits, depending on a condition of sick and therapeutic influence. There is no need of change of an adult dose in case of use of drug for treatment of children.

Endobronchial introduction: to enter as appropriate (constant tubes, the bronchoscope, etc.) 1-2 ampoules or more in day depending on clinical indications.

The ampoule is opened before the use. The opened ampoule mozhet to be stored in the refrigerator within 24 hours. Use of drug from earlier opened ampoule for injections is forbidden. Solution should not adjoin to rubber and metal surfaces.

Features of use:

Acetylcysteinum should be applied with care at the patients inclined to pulmonary bleedings, a pneumorrhagia, and also at persons with diseases of a liver, kidneys, dysfunction of adrenal glands. Patients with bronchial asthma and obstructive bronchitis should appoint Acetylcysteinum with care under systematic control of bronchial passability.

Pregnancy and lactation. At pregnancy and a lactation use of Acetylcysteinum is possible only if the estimated advantage for mother exceeds potential risk for a fruit and the baby.

Side effects:

At parenteral administration of Fluimutsila® perhaps easy burning in an injection site, skin rash, a small tortoiseshell.

At inhalation introduction - reflex cough, local irritation of respiratory tracts, stomatitis, rhinitis, it is rare - a bronchospasm (in this case appoint bronkhodilyatator).

Interaction with other medicines:

The combined use of Acetylcysteinum with antibechics can increase stagnation of a phlegm because of suppression of a tussive reflex.

At simultaneous use with such antibiotics as tetracyclines (excepting doxycycline), ampicillin, Amphotericinum In, their interaction with thiol group of Acetylcysteinum is possible that leads to decrease of the activity of both drugs. Therefore the interval between reception of these drugs has to make not less than 2 hours.

The concomitant use of Acetylcysteinum and nitroglycerine can lead to strengthening of vasodilating and dezagregantny action of the last.

Acetylcysteinum eliminates toxic effects of paracetamol.

Contraindications:

Hypersensitivity to Acetylcysteinum, a peptic ulcer of a stomach of a duodenum in an aggravation stage.

Overdose:

Acetylcysteinum at reception in doses of 500 mg/kg/day does not cause signs and symptoms of overdose.

Storage conditions:

To store at the room temperature from + 15 °C to + 25 °C. To store in the places unavailable to children! A period of validity - 5 years. Not to use after the date specified on packaging.
