Amoksil

General characteristics. Structure:

1 tablet contains trihydrate amoxicillin in terms of amoxicillin – 0,25 g or 0,5 g;
excipients: potato starch, polyvinylpirrolidone low-molecular medical, calcium stearate.

Pharmacological properties:

Pharmacodynamics. Amoxicillin – an antibiotic of a broad spectrum of activity from group of aminopenicillin. Possesses the bactericidal mechanism of action which consists in suppression of synthesis of a cell wall of bacteria, sensitive to drug. It is active concerning such microorganisms: gram-positive bacteria of Staphylococcus spp. (except for the strains producing a penicillinase), Streptococcus spp., (including Streptococcus pneumoniae), Bacillus anthracis, Corynebacterium diphtheriae (the majority of strains), Erysipelothrix rhusiopathiae, Listeria monocytogenes; gram-negative bacteria: Borrelia burgdorferi, Escherichia coli, Haemophilus influenzae, Klebsiella spp., Leptospira spp., Moraxella (Branhamella catarrhalis) (majority of strains), Neisseria gonorrhoeae, Neisseria meningitidis; Proteus mirabilis, Salmonella spp., Shigella spp., Treponema pallidum, Treponema pertenue; anaerobic bacteria: Eubacterium spp., Clostridium perfringens, Clostridium tetani, Peptostreptococcus spp., Peptococcus spp., Streptococcus faecalis.
In a combination with metronidazole shows activity in relation to Helicobacter pylori.
The microorganisms producing a penicillinase are steady against amoxicillin. It is inactive concerning mycobacteria, mycoplasmas, rickettsiae, mushrooms, amoebas, a plasmodium, viruses, and also Pseudomonas aeruginosa and Proteus spp. (except for P. mirabilis).

Pharmacokinetics. After intake amoxicillin is well soaked up, reaching the maximum concentration in blood serum in 2 hours after reception. Elimination half-life 1,5 hours. It is brought out of an organism in not changed look through kidneys (60 – 70%).

Indications to use:

Амоксил® appoint for treatment of the infections caused by microorganisms, sensitive to drug, including:
upper respiratory tract infections (average otitis, tonsillitis, sinusitis);
lower respiratory tract infections (bronchitis, pneumonia);
infections of biliary tract and digestive tract;
infections of urinary tract (cystitis, pyelonephritis, a symptomless bacteriuria at pregnant women);
acute uncomplicated gonorrhea;
infections of skin and soft tissues.

In a combination with metronidazole or klaritromitsiny apply to treatment of the diseases of a digestive tract associated with Helicobacter pylori (chronic gastritis, a peptic ulcer of a stomach and a 12-perstny gut).

Route of administration and doses:

Drug is accepted on doctor's orders. Амоксил® appoint inside (irrespective of meal). Drug dosing mode individual. The single dose for adults and children is more senior than 10 years (with body weight more than 40 kg) makes 250 - 500 mg, at heavy disease - 1 g. For children at the age of 5? 10 years the single dose is equal to 250 mg; aged from 2 up to 5 years - 125 mg; to children under 2 years drug appoint 30 mg/kg of body weight in a daily dose. The maximum dose – 60 mg/kg a day.
For adults and children the interval between receptions makes 8 hours.

At patients with a renal failure at clearance of creatinine of 15 - 40 ml/min. the interval between receptions should be increased till 12 o'clock; at an anury the daily dose should not exceed 2 g. In a combination with metronidazole (1,5 g/days) the daily dose of the tablets Amoksila® makes 2,25 g (on 3 receptions). To patients with the expressed renal failure (clearance of creatinine less than 30 ml/min.) and/or a liver appoint a day 1 g of metronidazole and 1,5 g of amoxicillin (in 2 receptions). For an eradikation of Helicobacter pylori appoint amoxicillin on 1 g 2 times a day with metronidazole on 0,5 g (or кларитромицин on 0,5 g 2 times a day) and лансопразол (омепразол) in a standard dose of 7 days.

Features of use:

With care it is necessary to appoint during pregnancy. It is allocated with breast milk that can lead to the child's sensitization.
At a renal failure the dose should be established taking into account clearance of creatinine.

Side effects:

At use of Amoksila® are possible:
allergic reactions (small tortoiseshell, erythema, Quincke's edema, rhinitis, conjunctivitis; in isolated cases? fever, joint pains, eosinophilia; very seldom? acute anaphylaxis);
from a digestive tract (nausea, vomiting, diarrhea), development of superinfections.

At use in a combination with metronidazole are possible:
nausea, vomiting, anorexia, diarrhea, lock, pain in epigastric area, a glossitis, stomatitis, in isolated cases hepatitis, pseudomembranous colitis, intersticial nephrite, disturbances of a hemogenesis;
at prolonged use in high doses? dizziness, ataxy, loss of consciousness, depression, periodic neuropathy, spasm.

Interaction with other medicines:

Amoxicillin reduces effect of action of estrogensoderzhashchy contraceptives. Strengthens digoxin absorption. Reduces clearance and increases toxicity of a methotrexate. Removal from an organism through kidneys is slowed down by acetylsalicylic acid, indometacin, Sulfinpyrazonum, oksifenbutazony, phenylbutazone.

Contraindications:

Use of Amoksila® is contraindicated in case of hypersensitivity to penicillin, cephalosporins, and also at an infectious mononucleosis. Use of drug in a combination with metronidazole is contraindicated at diseases of a nervous system, hemopoiesis disturbances, a lymphoid leukosis, infectious mononucleosis, pregnancy, a lactation, hypersensitivity to nitroimidazole derivatives. Амоксил® in a combination with metronidazole it is not recommended to patients to appoint younger than 18 years, at diseases of a digestive tract with long diarrhea or vomiting, at heavy pathology of a liver. Against the background of a combination therapy with metronidazole it is impossible to take alcohol.

Overdose:

Symptoms: nausea, vomiting, diarrhea, disturbance of water and electrolytic balance.
Treatment: the gastric lavage, use of enterosorbents (absorbent carbon), salt laxatives, correction of water and electrolytic balance, if necessary apply a hemodialysis.

Storage conditions:

To store in the dry, protected from light place at a temperature from 15 ºС to 25 ºС. Period of validity 2 years. To store in the place, unavailable to children.
