General characteristics. Structure:

Active agent: атенолол 50/100 mg, Chlortalidonum of 12.75/25.5 mg

Pharmacological properties:

Pharmacodynamics. The combined hypotensive drug of long action. Contains the active agents which are mutually supplementing actions of each other. Atenolol - the selection beta1-adrenoblocker, has no vnutnenny SMA; urezhat ChSS, reduces UOK and IOC (during 24 h); reduces raised by the ABP both in position of the patient "standing", and in situation "lying". Chlortalidonum - diuretic HP of average force of action, promotes removal of Na+, Cl-, K+, Mg2 + and waters, Ca2+ detains; reducing the maintenance of Na + in a vascular wall, reduces its sensitivity to vasoconstrictive influences. Duration of its diuretic action - 24-48 h.

Pharmacokinetics. After intake absorption of an atenolol from a GIT - 50%. TCmax of an atenolol and Chlortalidonum - 2-4 h. Communication with proteins of plasma at an atenolol - 6-16%, at Chlortalidonum - 90% (including with erythrocytes). Atenolol is slightly metabolized in a liver. Atenolol is brought by kidneys, Chlortalidonum - kidneys and through intestines. T1/2 of an atenolol is 6-9 h (at single and at prolonged use).

Indications to use:

Arterial hypertension.

Route of administration and doses:

Inside, on 1 tablet a day (morning).

Features of use:

Systematic control of electrolytic balance is necessary; patients with reduced secretory function of kidneys have a control of KK. At prolonged treatment it is necessary to appoint the drugs K+ in addition. Cancellation is carried out gradually, reducing a dose within 7-10 days. Before surgical intervention using the general anesthesia cancellation has to be complete at least for 48 h before operation. If the patient accepted drug before operation, it should pick up HP for the general anesthesia with the minimum negative inotropic effect. At a bronkhoobstruktivny syndrome purpose of beta2-adrenostimulyator is possible. With care. Side effects:

Bradycardia, AV blockade, development or aggravation of SN, cold snap of extremities, adynamy, nausea, locks or diarrhea, dizziness, sleep disorders, hypopotassemia, hypercalcemia, hyponatremia, hyperuricemia; a skin itch, a dermahemia, an asthma, a bronchospasm, thrombocytopenia, a neutropenia, vision disorders, change of a _olerantnost to glucose, decrease in a potentiality, sweating disturbance, reduction of secretion of the lacrimal liquid.

Interaction with other medicines:

Increases concentration of lidocaine in plasma; Cimetidinum increases bioavailability of an atenolol. The hypotensive effect is weakened by NPVP (Na delay + and blockade of synthesis of Pg) and estrogen (Na+ delay). At a concomitant use with verapamil, diltiazem, digoxin, Amiodaronum, antiaritmika, anesthetics - strengthening of expressiveness negative hrono-, other and dromotropic influences.

Contraindications:

Hypersensitivity, AV blockade of the II-III Art.; HSN (resistant to treatment by cardiac glycosides); bradycardia; cardiogenic shock; liver and/or renal failure; bronchial asthma; pregnancy, lactation period, children's age.

Overdose:

Symptoms: the expressed bradycardia, arterial hypotonia. Treatment: at bradycardia - in/in 1-2 mg of atropine; 10 mg of a glucagon in/in bolyusno. Dobutaminum of 2.5 mkg/min. in/in, 10-25 mkg of an izoprenalin in/in kapelno with a speed of 5 mkg/min.
