General characteristics. Structure:
Active ingredient: 100 g of a dextrin 40 000. Theoretical osmolarity 310 ¼Äß¼/l.
Excipients: sodium chloride, water for injections.
Pharmacological properties:
Pharmacodynamics. Reopoliglyukin - 40 is drug of a low-molecular dextran. It reduces aggregation of uniform elements of blood, promotes movement of liquid from fabrics in a circulatory bed. In this regard drug increases suspension properties of blood, its viscosity droops, promotes recovery of a blood-groove in small capillaries, has disintoxication effect, warns and reduces aggregation of uniform elements of blood.
Pharmacokinetics. The dextran is removed нз an organism through kidneys (in the first days to 70-50%).
Indications to use:
Reopoliglyukin-40 apply at disturbances of a capillary blood-groove, to prevention and treatment of traumatic, operational and burn shock; disturbances of arterial and venous circulation, for treatment and prevention of thromboses, thrombophlebitises, endoarteritis; at the heart operations performed with use of the cardiopulmonary bypass (for addition to perfused liquid), in vascular and plastic surgery for improvement of local circulation; for desintoxication at burns, peritonitis, pancreatitis, etc. Reopoliglyukin-40 appoint at diseases of a retina and optic nerve, inflammatory processes of a cornea and choroid.
Route of administration and doses:
Reopoliglyukin-40 it is entered only intravenously! The dose is established individually and depends on a clinical situation and a condition of the patient. Because of a possibility of development of anaphylactic reactions, the first 10-20 minutes administer the drug slowly.
For the purpose of the prevention and treatment of the disturbances of a capillary blood-groove connected with traumatic, operational and burn shock apply intravenously kapelno 400 - 1000 ml a day (within 30 — 60 minutes).
At operative measures on heart and vessels enter before operation intravenously kapelno 10 ml/kg, during operation 400 - 500 ml, and within 5-6 days later on 10 ml/kg on introduction.
To children the general dose should not exceed 15 mg/kg/days. At warmly — vascular operations enter to children aged up to 2 — 3 years on 10 ml/kg of 1 times a day (within 60 minutes), up to 8 years — 7 - 10 ml/kg (1 - 2 time a day), up to 13 years — 5 - 7 ml/kg (1 - 2 time a day), 14 years — a dose for adults are more senior. For desintoxication enter 5 — 10 ml/kg within 60 - 90 minutes.
Side effects:
Allergic reactions: reddening of integuments, enanthesis, nausea, fever, acute anaphylaxis.
Interaction with other medicines:
It is necessary to check previously for compatibility a dextrin with drugs which are planned for introduction to infusion solution.
Contraindications:
Drug is contraindicated at thrombocytopenia, diseases of kidneys (with an anury), heart failure and in cases when it is not necessary to enter large amounts of liquid. Deficit of fructose-1,6-diphosphatase, the increased individual sensitivity to drug, a fluid lungs, a hyperpotassemia.
Storage conditions:
To store in the dry place at a temperature not above 25 °C. To store in the place, unavailable to children. Freezing at transportation is allowed. A period of validity - 2 years. Drug should be used before the date specified on packaging.

Solution for infusions of 10% in polymeric containers on 100, 200, 250, 400 and 500 ml or in packages for intravenous infusions on 250 and 500 ml. Each container or a package for intravenous infusions place in the secondary package made of polyethylene or polypropylene films with the put application instruction. Containers or packages for intravenous infusions but secondary packages stack in boxes from corrugated fibreboard.
