Metilergobrevin

General characteristics. Structure:

Active agent: methylergometrine maleate of 200 mkg

Excipients: maleic acid, glycine, sodium chloride, thiocarbamide, water for and.

Pharmacological properties:

Semi-synthetic derivative natural alkaloid of an ergot (ergometrine). Raises a tone and sokratitelny activity of a myometrium. Has weak effect of a pas peripheral vessels, practically does not raise the ABP.

Pharmacokinetics. Therapeutic action in/in introductions is shown later in 30-60 sec., after introduction in oil - in 2-5 min. Effect of drug continues 3 and more than a h after use in oil and to 2 h later in/in uses. Communication of drug with proteins of plasma makes 35%. Vd makes 39.1-73.1 l, clearance – 9.9-18.9 l/h. Drug is quickly distributed on body tissues - the semi-distribution period in/in uses makes later 1-3 min. Gets into breast milk. It is metabolized in a liver, removed preferential with a stake, partially - kidneys.

Indications to use:

— for the purpose of reduction of the second period of childbirth after emergence of a front part of a shoulder of a fruit;
— childbirth (the II period, after emergence of a front part of a shoulder);
— metrorrhagia (including atonic);
— hypotonic bleedings in an early puerperal period (prevention and treatment), post-operating rooms (Cesarean section, removal of myoma), post-abortion.

Route of administration and doses:

Metilergobrevin's doses are defined according to indications.

Atonic uterine bleedings: appoint 0.1 mg (0.5 ml) in/in or 0.2 mg (1 ml) in oil, in need of an injection it is possible to repeat at an interval of 2 h.

Metrorrhagias: appoint 0.1 – 0.2 mg in oil or 0.1 mg in / century.

Cesarean section: the drug is administered after extraction of a fruit: in/in on 0.05-0.1 mg or in oil on 0.2 mg.

Abortion: appoint after expansion of the channel of a neck of uterus in/in 0.1-0.2 mg. At spontaneous abortions it is shown in/in introduction of 0.05-0.1 mg.

Childbirth: at attentive control of the obstetrician for reduction of the second period of childbirth enter into 0.1-0.2 mg of Metilergobrevin only after emergence of the head or a front part of a shoulder of a fruit. When conducting childbirth in the conditions of the general anesthesia introduction of 0.2 mg of Metilergobrevin is recommended. If it is impossible in/in drug use, then it is entered by in oil 0.2-0.4 mg in a dose.

Features of use:

Use of drug should be made only in the conditions of a specialized hospital, at strict medical control of the ABP, ChSS, sokratitelny uterine activity. At use directly after the child's birth (if the placenta is in a cavity of the uterus) can interfere with department of an afterbirth.

Side effects:

Abdominal pain, nausea, vomiting, the increased sweating, dizziness, headache, sonitus, bradycardia, tranzitorny increase in the ABP, peripheral vasospasm, thorax pain, диспноэ, a stroke, a myocardial infarction, a fluid lungs, allergic reactions in the form of skin rash; extremely seldom - an acute anaphylaxis, decrease in secretion of milk.

Interaction with other medicines:

At treatment and prevention of atonic uterine bleedings Metilergobrevin's combination with oxytocin is effective.
Metilergobrevin can strengthen angiotonic effects of sympathomimetics and ergotamine.
The combination to a dopamine is contraindicated since the gangrenosis of extremities is possible.
Ftorotanum reduces sensitivity of a uterus and reduces activity of methylergometrine.

Contraindications:

— pregnancy;
— first period of childbirth;
— the second period before emergence of the head of a fruit;
— severe forms of arterial hypertension;
— occlusal diseases of peripheral vessels;
— hypersensitivity to ergot drugs;
— sepsis.

With care: renal or liver failure, cardiovascular diseases, need of manual department of a placenta.

Use of drug METILERGOBREVIN at pregnancy and feeding by a breast
Use is contraindicated during pregnancy, during the 1st period of childbirth, during the 2nd period of childbirth till the fruit head birth. It is not recommended to apply in the period of a lactation.

Overdose:

Symptoms of overdose are: nausea, vomiting, abdominal pain, feeling of numbness and a pricking in extremities, the arterial hypertension accompanied in hard cases with respiratory depression, a hypothermia, spasms and a coma.

At overdose carry out a symptomatic treatment with careful observation of functions of cardiovascular and respiratory systems.

Storage conditions:

Drug should be stored at a temperature from 8 °C to 15 °C in protected from light and the place, unavailable to children.
