Adalatum

Adalatum has pronounced hypotensive, vasodilating and anti-anginal action. Causes reduction of peripheric vascular resistance and expansion of coronary arteries.

Form of release and structure

Active agent of Adalatum – the nifedipine reducing the movement of calcium ions through calcium channels in cells.

Drug is produced in the form of solution for infusions and tablets.

In a cardboard box together with the bottle containing solution also special one-time syringe and a connecting tube for infusions which use reduces losses of nifedipine at its introduction is located.

One bottle of Adalatum contains 5 mg of active agent in 50 ml of infusion solution.

Excipients:

• Ethanol of 96% – 7,500 g;
• Macrogoal of 400 - 7,500 g;
• Sodium hydroxide 0,1 N – to pH 4,5-7,5;
• Water for injections – 34,995 g.

One tablet Osmo-Adalat, coated, consists of nifedipine of 30 mg or 60 mg.

Excipients:

• Gipromelloza;
• Magnesium stearate;
• Poly(ethylene oxide);
• Sodium chloride;
• Ferrous oxide red;
• Cellulose acetate;
• Polyethyleneglycol-4000;
• Опадрай OY-S-24914 (ferrous oxide red + titanium dioxide).

Indications to use

In the instruction to Adalatum the following diseases at which its use is reasonable are specified:

• Hypertensive crisis;
• Arterial hypertension;
• Coronary heart disease;
• Printsmetal's stenocardia.

Contraindications

Treatment by Adalatum is impossible in some cases. It:

• Unstable stenocardia;
• Cardiogenic shock;
• The first four weeks after an acute myocardial infarction;
• Age up to 18 years;
• Pregnancy and feeding by a breast;
• Hypersensitivity to nifedipine.

Purpose of drug with simultaneous use of rifampicin is contraindicated. Care is required at therapy of some diseases which are specified in the instruction to Adalatum:

• Subaortal or heavy aortal stenosis;
• Chronic heart failure;
• The expressed bradycardia;
• Disturbances of cerebral circulation in a severe form;
• Liver failure;
• The expressed arterial hypotension.

Advanced age of the patient is also a condition of careful use of Adalatum.

Route of administration and dosage

Adalatum should be entered intravenously within 4-8 hours, at the same time rate of administering should not exceed 125 ml/h (1,25 mg of nifedipine/h).

The greatest possible daily dose has to correspond to 15-30 mg of active ingredient.

Infusional therapy can be carried out within 3 days, then the patient should be transferred to oral administration.

Pill is taken inside, on 30 mg of 1 times a day.

Side effects

Treat the most often arising negative effects:

• The rushes of blood to the person causing a feeling of heat, a lowering of arterial pressure, tachycardia;
• Headache;
• Nausea.

Less often (frequency of development is less than 1%) after use of Adalatum there can be following undesirable effects:

• Aggravation of displays of heart failure, stethalgia;
• Rash on skin, burning and pain in the field of an injection;
• Nervousness, dizziness;
• The increased perspiration.

Special instructions

The amount of alcohol, the corresponding 45 g of ethanol at the rate on a day dose contains Adalatum. The patients having alcoholism or disturbances of metabolism of ethanol, to pregnant women and the feeding women, children and patients with diseases of a liver or epilepsy need to take it into account at purpose of nifedipine.

Ethanol can influence effect of other medicines, break ability to driving of motor transport and to work with the mechanisms requiring special attention.

Terms and storage conditions

To store in the dark place, at air temperature not above 25 °C.

The period of validity makes 2 years and is specified on packaging.

Adalatum is released from drugstores according to the recipe of the doctor.
