General characteristics. Structure:

Active agent: 5 mg or 10 mg of a bisoprolol of a fumarat.

Pharmacological properties:

Bisoprolol is the expressed cardioselective blocker a beta of adrenoceptors without sympathomimetic activity (VSA). Has anti-hypertensive effect, negative hrono-and inotropic action. Bisoprolol after intake is completely soaked up in a GIT, about 50% are metabolized in a liver, about 50% are removed through kidneys in an invariable look. The elimination half-life makes 10 — 12 hours.

Indications to use:

Arterial hypertension; Ischemic heart disease: angina of exertion, rest stenocardia, unstable stenocardia; subacute stage of a myocardial infarction; postinfarction period; in a combination with nitrates or antagonists of calcium channels it is possible to apply also at alternative stenocardia; disturbances of a cordial rhythm, first of all at a tachyarrhythmia (supraventricular and ventricular).

Route of administration and doses:

Therapy is begun with 5 — 10 mg once a day, as a rule, in the morning. If necessary it is possible to increase a dosage to 20 mg once a day. At the expressed abnormal liver functions or kidneys (clearance of creatinine less than 20 ml/min.) it is not recommended to exceed the maximum daily dose of 10 mg. A pill is taken in the morning on an empty stomach or during meal, washing down with enough liquid.

Features of use:

At a diabetes mellitus бисопролол can mask hypoglycemia signs (tachycardia, a tremor, excessive sweating). Before operational intervention it is necessary to inform the anesthesiologist on use of a bisoprolol. Therapy by bisoprolol should be ended, gradually reducing a dose.

Side effects:

Dizziness, headaches, cryesthesia or nonsensitivities in the lower extremities, paresthesia, osrtostatichesky hypotension, a lock, a diarrhea, nausea, abdominal pains, a depression, sleep disorders.

Interaction with other medicines:

Combined use of hypotensive drugs with bisoprololy can lead to excessive decrease in the ABP. The combined reception of antagonists of calcium like verapamil or some antiaritmik increases risk of development of bradycardia and decrease in a kontraktilnost. The combined reception it is central the operating anti-hypertensive means (a clonidine, a metyldop, моксонидин, рилменидин) increases risk of decrease in ChSS and minute volume of heart. The combined reception of antagonists of calcium of dihydropyridinic type (for example, nifedipine) increases risk of development of hypotension. Glycosides of a foxglove can lead to delay of a cordial impulse, and those to decrease in ChSS.

Contraindications:

Hypersensitivity to a bisoprolol or any of drug excipients, an acute heart failure, cardiogenic shock, an atrioventricular block of II and III degrees, a sick sinus syndrome, sinuatrial blockade, bradycardia, hypotension, heavy bronchial asthma, HOBL of heavy degree, a pheochromocytoma (except the previous therapy by bokator of alpha receptors), Reynaud's syndrome of heavy degree, a severe form of HIBNK, Prinzmetal's stenocardia (in pure forms and as monotherapy), a metabolic acidosis. During pregnancy and a lactation administration of drug is not recommended.
