Diklofenak-Zdorovye forte, gel of 3%, 50 g

General characteristics. Structure:

International and chemical names: diclofenac; sodium salt 2-[(2,6 dichlorophenyl) - amine] phenylacetic acid;
main physical and chemical properties: gel of white color, a homogeneous consistence, with a specific smell;
structure: 1 g of gel contains sodium diclofenac in terms of 100% substance – 30 mg; 
excipients: methylparahydroxybenzoate (нипагин), mint oil (oil mint radio), carbomer, glycerin or глицерол, a macrogoal 400, ethanol (96%), a dimethyl sulfoxide, solution of ammonia of 15%, the water purified.

Pharmacological properties:

Pharmacodynamics. Anti-inflammatory, analgetichesky, antirheumatic drug. The mechanism of action is caused by oppression of biosynthesis of prostaglandins by cyclooxygenase inhibition, reduction of formation of kinin and other mediators of an inflammation and pain, the stabilizing influence on lizosomalny membranes. At rheumatic diseases reduces hypostasis, a hyperemia and a rest pain and at the movement, morning constraint and a swelling of joints, improving their functional capacity. At postoperative and traumatic damages quickly reduces pain and the phenomena of an inflammation in the field of putting drug, promotes a rassasyvaniye of a posttraumatic hematoma. The dimethyl sulfoxide and oil mint increase antiinflammatory and analgetichesky effect of drug.

Pharmacokinetics. Diclofenac at cutaneous putting drug well gets through a skin barrier, preferential concentrating in the center of an inflammation and synovial fluid. Linkng with proteins of plasma makes 99%. It is metabolized in a liver. It is removed by kidneys (about 65%) and with bile (about 35%) in the form of inactive connections with glucuronic and sulfuric acids; less than 1% are excreted in not changed form. 

Indications to use:

The pseudorheumatism ankylosing a spondylarthritis, acute gouty, infectious, psoriasis and traumatic arthritises, osteoarthroses, a bursitis, a tendovaginitis, radiculitises, thrombophlebitises, neuralgia, mialgiya, a posttraumatic inflammation of muscles, joints, sinews and sheaves (stretchings, bruises), a postoperative edematous pain syndrome and also as symptomatic therapy of other diseases which are followed by an inflammation and pain.

Route of administration and doses:

Gel in number of 2–3 g (1–3 cm) is applied with a thin layer on skin in the field of an inflammation (sore joints or other body parts with an inflammation and pain) 2–4 times a day, easily rubbing in skin. The average daily dose for adults and children is more senior than 14 years makes 6 g of gel.
Duration of a course of treatment is defined individually depending on efficiency and sensitivity to therapy and, as a rule, does not exceed 14 days. 

Features of use:

Drug should be applied only on the unimpaired sites of skin, avoiding hit on open wounds, in eyes and on mucous membranes. After application of drug it is necessary to wash up hands if this area is not exposed to treatment.
Use of gel can be combined with reception of diclofenac in tablets which daily dose should not exceed 75 mg.
Before use to patients with a peptic ulcer of a stomach and duodenum, bronchial asthma, rhinitis, polyps of a mucous membrane of a nose, abnormal liver functions, kidneys or system of a hemopoiesis, and also at simultaneous use of other non-steroidal anti-inflammatory drugs it is necessary to consult to the doctor.
With care appoint the patient who has in the anamnesis data on allergic reactions to non-steroidal anti-inflammatory drugs and a dimethyl sulfoxide, a peptic ulcer of a stomach.

Side effects:

Phenomena of local irritation of skin: itch, hyperemia, burning, rash. 

Interaction with other medicines:

At cutaneous use of drug its interaction with other medicines is not established.
However it is necessary to consider that diclofenac of sodium can exponentiate effects and ulcerogenic action of corticosteroids, increase concentration of digoxin and lithium in blood, to weaken effect of diuretics and saluretics of group of a tiazid and furosemide. The therapeutic effectiveness at use with salicylates decreases, at the same time ulcerogenic action on a mucous membrane of a stomach amplifies. At use along with anticoagulants it is necessary to control a condition of system of a blood coagulation.

Contraindications:

Digestive tract erosive cankers, bronchial asthma, hemopoiesis disturbances, individual sensitivity to drug components, pregnancy, the feeding period a breast, children's age up to 14 years.

Overdose:

When putting gel on big sites of skin or at exceeding of the recommended doses the system side effects characteristic of diclofenac of sodium and other non-steroidal anti-inflammatory drugs are possible: dyspepsia, nausea, pain in epigastric area, a headache, dizziness, drowsiness, a bronchospasm. Treatment: dose decline or cancellation of administration of drug, symptomatic therapy.

Storage conditions:

To store in the place, unavailable to children, at a temperature from 15 °C to 25 °C.

Period of validity – 2 years.
