General characteristics. Structure:

Active agent: sodium warfarin (in the form of a clathrate) 2,17 mg corresponds to warfarin of sodium of 2 mg in one tablet.

Active agent: sodium warfarin (in the form of a clathrate) 5,40 mg corresponds to warfarin of sodium of 5 mg in one tablet.

Pharmacological properties:

Warfarin is derivative a dikumarola, interfering synthesis vitamin of K-dependent blood-coagulation factors (VII, IX, X, II). After intake warfarin is well soaked up in a GIT, biological availability more than 90%, biological half-life – about 36 hours. Warfarin does not make any direct impact on the formed blood clot, however interferes with a further thrombogenesis, besides, protects from emergence of complications owing to a thromboembolism.

Indications to use:

Prevention and treatment of thromboembolic diseases (TEZ), risk of developing of an embolism at chronic fibrillation of auricles, after an acute myocardial infarction, patients with the artificial valve have hearts and with TEZ in the anamnesis.

Route of administration and doses:

Treatment usually begins with a dose of 10 — 15 mg a day which in 2 — 4 days is adjusted depending on indicators of a prothrombin time. The usual maintenance dose makes 2 — 10 mg a day. Duration of treatment is individual, treatment should be continued until danger of developing of a thrombembolia disappears. The dose decreases gradually, during 3 — 4 weeks, for prevention of a withdrawal. The dosage at children is individual, the usual maintenance dose makes 0,09 — 0,32 mg / 1 kg of weight of the patient in days.

Features of use:

It is necessary to be careful at patients with a medium-weight or moderate form of a renal or liver failure, with injuries which can be complicated by internal bleeding, with a hypertension, a severe form of diabetes, heavy allergic reaction, an active form of tuberculosis, polyarthritis, and also at long catheterization.

Side effects:

Bleeding, urticaria, alopecia, dermatitis, the increased temperature, nausea, vomiting, diarrhea, spasms in a stomach, a phenomenon of "purple toes", the reaction connected with hypersensitivity, the reaction consisting in a hemorrhagic heart attack and a necrosis of skin.

Interaction with other medicines:

Effect of warfarin alcohol, Allopyrinolum, aminosalicylic acids, anabolic steroids, anesthetics, Amiodaronum, Chlorali hydras, chloramphenicol, Chlorproramidum, chymotrypsin, Cimetidinum, Clofibratum, a dextran, diuretics, the phenopro-hair dryer, an ibuprofen, indometacin, inhalation anesthetics, Methyldopum strengthen, MAO inhibitors, Disulfiramum, erythromycin, nifedipine, a methotrexate, phenylbutazone, Phenytoinum, quinidine, quinine, salicylates, sulfonamides, metronidazole, co-trimoxazole, thyroid hormones, hepatotoxic drugs, peroral anti-diabetics, azolovy antimycotics, Tamoxifenum, флувоксамин, norfloxacin, ловастатин and симвастатин. Effect of warfarin adrenokortikoida, antacids, antihistaminic drugs, carbamazepine, barbiturates, a haloperidol, meprobamate, glucocorticoids, oral contraceptives, vitamin C, griseofulvin, rifampicin, a grass reduce a St. John's Wort, and also the food stuffs rich with vitamin K.

Contraindications:

Hypersensitivity to warfarin or excipients, pregnancy, conditions of bleedings (hemophilia, thrombocytopenia), operations on TsNS or in the eyes, operations or injuries with the subsequent big open wounds, the tendencies to bleeding connected with active formation of ulcers or with explicit bleeding from a GIT, urinogenital system and a respiratory organs, a cerebrovascular hemorrhage, cerebral aneurism, the stratifying aortic aneurysm, a pericardis, a subacute bacterial endocarditis, threat of abortion, an eclampsia and a preeclampsia, a lumbar puncture and other diagnostic and therapeutic actions which can be complicated by uncontrollable bleeding, a malignant hypertension, heavy a form of a renal or liver failure, not cooperating patient, and also children up to 3 years.

Overdose:

There are no data on overdose.
