General characteristics. Structure:

Main physical and chemical properties: transparent liquid of red-brown color, sweet taste, with a characteristic smell of raspberry;
Active agent: gland of ammonium citrate of 40 mg (8,2 mg of elementary iron are equivalent), folic acid of 0,3 mg, cyanocobalamine of 0,01 mg; excipients: ethanol, sorbite, methylparahydroxybenzoate (methylparaben), пропилпарагидроксибензоат (propylparaben), saccharin of sodium, sodium chloride, dinatrium эдетат (Trilonum of B), fragrance crimson, the water purified.

Pharmacological properties:

Pharmacodynamics. Gemoferon — the balanced complex of bivalent iron, _{B12 vitamin} and folic acid, necessary for normal process of a hemopoiesis. Iron is a part of hemoglobin, a myoglobin, various enzymes, reversibly connects oxygen and provides its transport in fabric, participates in a number of redoxreactions, stimulates an erythrogenesis. Folic acid together with cyanocobalamine stimulates an erythrogenesis, participates in synthesis of amino acids, nucleotides, nucleic acids, sincaline exchange. During pregnancy it is necessary for normal development of nerve fibrils of a fruit, protects it from influence of teratogens, plays an important role in reactions of cellular immunity. Cyanocobalamine participates in synthesis of nucleotides, is an important factor of normal growth and development of epithelial cells, is necessary for metabolism of folic acid and synthesis of a myelin. Cyanocobalamine and folic acid also interfere with development of megaloblastny anemia and neurologic disturbances.

Pharmacokinetics. At use inside the ion of bivalent iron which is a part of drug is almost completely absorbed from digestive tract in a system blood stream. The maximum concentration of iron in blood serum is reached in 2-4 hours after administration of drug. Iron almost completely contacts transferrins of plasma and is involved in process of formation of hemoglobin, a myoglobin, cytochrome oxydase, a catalase and peroxidase or deposited in RES cells. Route of administration and doses:

Drug contains alcohol. Admissible concentration of alcohol in drugs for children of the first five years of life makes 0,5 %. In this regard the daily dose of drug for children of this age category needs to be divided into two receptions and to mix with a water tablespoon. Features of use:

It is not necessary to exceed the recommended drug doses. With care appoint to patients with an abnormal liver function, digestive tract, to the persons suffering from hemoglobinopathies, iron exchange disturbance. It is not necessary to combine oral administration of iron preparations with their parenteral administration, and also with repeated hemotransfusions.

Side effects:

Drug is well transferred by patients. In isolated cases at individual hypersensitivity abdominal pains, nausea, vomiting, a diarrhea or a lock are possible, however these manifestations have temporality and disappear upon transition to Gemoferon's use after meal, a dose decline or drug withdrawal. In isolated cases emergence of allergic reactions (an itch, skin rashes) which demand drug withdrawal and performing the desensibilizing therapy is possible.

Interaction with other medicines:

Calcium drugs, antacids, Cimetidinum, холестирамин, the ftorkhinolona, medicines and foodstuff containing hydrocarbonates, carbonates, oxalates, phosphates, tea coffee reduce absorption of iron in intestines. Drug is not recommended to be combined with a methotrexate, Disulfiramum, Pyrimethaminum, Triamterenum and Trimethoprimum. Iron preparations reduce absorption of tetracycline and a therapeutic effectiveness of Penicillaminum.

Contraindications:

Excess accumulation of iron in an organism (hemochromatosis, hemosiderosis), an urokoproporfiriya, disturbance of digestion of iron (hypo - aplastic and hemolitic anemias), a round ulcer of a stomach and duodenum in an active phase, acute coloenterites, hypersensitivity to drug components.

Overdose:

Development of nausea, vomiting, pain in epigastric area is possible, diarrheas which quickly disappear after drug withdrawal and do not demand holding special events. In cases of considerable overdose of drug it should be cancelled, carried out a gastric lavage by hydrosodium carbonate solution, in case of substantial increase of concentration of serumal iron it is necessary to apply a specific antidote of iron — Deferoxaminum. Carry out symptomatic therapy.
