Ferrolek-Zdorovye, 50 mg/ml 2 ml of N5

General characteristics. Structure:

International name: Ferric oxide dextran complex;
main physical and chemical properties: viscous liquid of red-brown color; structure: 1 ml of solution contains 50 mg of iron (in the form of dextran iron); excipients: water for injections.

Pharmacological properties:

Pharmacodynamics. Fills shortage of ions of iron in an organism at iron deficiency anemias of various genesis, stimulates an erythrogenesis. At course treatment gradual regression clinical (weakness, fatigue, dizziness, tachycardia, a xeroderma) and laboratory displays of anemia happens drug.

Pharmacokinetics. At linkng with proteins of plasma transferrin which comes to red marrow is formed. Collects in cells of a liver, reticuloendothelial system, muscular tissue.

Indications to use:

Treatment of iron deficiency states in situations when bystry completion of reserves of iron (heavy posthemorrhagic anemia, therapy by erythropoetin) is necessary, and also at inefficiency of therapy by peroral drugs (a syndrome of the broken intestinal absorption, a chronic coloenteritis, nonspecific ulcer colitis, after a resection of a stomach, a small intestine).

Route of administration and doses:

The course dose of the drug Ferrolek-Zdorovye is defined by the hemoglobin level, body weight and a sex of the patient. Values for its definition are given in the table. This dose provides recovery of level of hemoglobin to norm and replenishment of the exhausted iron reserves.
More precisely the course dose (D) can be calculated proceeding from the body weight of the patient (M) and level of hemoglobin (Hb): for women – D - 0,00476 x M x (148-Nb) + 6; for men – D - 0,00476 x M x (148-Nb) + 14; for children up to 15 years – D - 0,00476 x M x (148-Nb), where: D - a course dose of drug in milliliters; M - the body weight, kg; Hb - hemoglobin level at the patient prior to treatment, g/l.

Intramuscular introduction: total quantity of the drug Ferrolek-Zdorovye is determined by the dose table or calculation. To the patients leading moderately active lifestyle, injections are entered daily, alternately into different buttocks. To the patients leading an inactive life or bed patients the frequency of injections is reduced to one or two weekly.
Ferrolek-Zdorovye it is entered deeply into a muscle for reduction of risk of hypodermic complications. To children 4 months are more senior: 0,6 ml on 1 kg of body weight a day (3 mg of iron on 1 kg of body weight a day). To adults and patients of old age: 2-4 ml (100 – 200 mg of iron) depending on hemoglobin level.
Intravenous administration: Ferrolek-Zdorovye enter intravenously struyno with a speed of 1 ml/min., but the volume of drug should not exceed 10 ml for 1 injection (contents of an ampoule to part 0,9% with sodium chloride solution to 10 ml).
Jet intravenous administration has to be applied only in that case when drop introduction is not shown.
To children 4 months are more senior: 0,6 ml on 1 kg of body weight a day (3 mg of iron on 1 kg of body weight a day). To adults and patients of old age: 2 ml (100 mg of iron) depending on hemoglobin level.
Maximum daily dose: for children 4 months – 0,14 ml of drug on 1 kg of body weight are more senior (7 mg of iron on 1 kg of body weight a day); for adults – 4 ml of drug.
At drop introduction necessary amount of drug dissolve 0,9% of solution of sodium chloride in 200–400 ml. The single dose for adults makes 2 ml of drug, for children 4 months – 0,6 ml of drug on 1 kg of body weight are more senior.
If at intravenous use there were any side reactions, introduction needs to be stopped immediately. Initial rate of administering does not exceed 10 drops a minute. In the absence of side reactions speed is increased till 45-60 by drops in a minute.
The maximum daily dose of drug at drop introduction makes 10 ml for adults, for children 4 months – 0,14 ml of drug on 1 kg of body weight are more senior, 1 ampoule (100 mg) dissolve 9% of solution of sodium chloride in 50 ml.

Features of use:

In the beginning to all patients enter intramusculary or intravenously a testodoza – 0,5 ml of drug. At intravenous administration the test dose has to be entered slowly, at least 30 seconds.
Though side effect of drug can be revealed the first several minutes after an injection, it is necessary that passed not less than an hour before introduction of an initial therapeutic dose.
Ferrolek-Zdorovye it can be entered intravenously or intramusculary. Intravenous drop administration has to be carried out only in the conditions of hospital. Intravenous administration does not become the patient with asthma.
With care patients should appoint drug with diseases of a liver and kidneys.
Use of drug at treatment of patients of advanced age does not increase risk of development of side effects.
Influence on ability to manage vehicles and mechanisms. There are no messages that use of drug reduces concentration of attention at control of vehicles and work with potentially dangerous mechanisms.
As well as other iron preparations for parenteral use, Ferrolek-Zdorovye are not appointed along with peroral iron preparations. The interval between parenteral use of drug and the beginning of use of peroral forms of iron has to make not less than 5 days.

Side effects:

Hyperthermia, headache, dizziness, arterial hypotension, arthralgia, inflammation in the place of an injection (erubescence); allergic reactions (skin itch); nausea, vomiting, lock, diarrhea.

Interaction with other medicines:

It is impossible to mix drug with other medicines.

Contraindications:

Hypersensitivity to drug; the anemia which is not caused by deficit of iron; excess of iron in an organism (hemochromatosis, a hemosiderosis); hemoglobin synthesis disturbances (the anemia caused by poisoning with lead, sideroblastny anemia); the expressed disturbances of a hemostasis (hemophilia) because of formation of a hematoma; first trimester of pregnancy; children up to 4 months.

Overdose:

Can lead to the acute overload iron which is shown as a hemosiderosis.
Treatment. In case of a serious poisoning enter Deferoxaminum intravenously slowly: to children – 15 mg/h, adult – 5 mg/kg/h (to 80 mg/kg/days); at slight poisoning to children each 4–6 h, adult – on 50 mg/kg intramusculary enter 1 g (to 4 g/days).

Storage conditions:

To store in protected from light and the place, unavailable to children, at a temperature from 15 °C to 25 °C. Not to allow freezing.
Period of validity - 2 years.
