Cerucal

General characteristics. Structure:

2 ml of solution contain 10,54 mg of Metoclopramidum of a hydrochloride in quality of active agent that corresponds to 10 mg of anhydrous Metoclopramidum of a hydrochloride.

Excipients:
sodium sulfite, sodium chloride, disodium salt 2HO of ethylene diamine tetraacetic acid, carbon dioxide, water for injections.

Pharmacological properties:

Metoclopramidum is a specific blocker of dopamine (D2) and serotoninovy receptors. Has antiemetic effect owing to impact on chemoceptors of a trigger zone of a myelencephalon.

Drug exerts the regulating and normalizing impact on activity of digestive tract. Lowers a physical activity of a gullet, raises a tone of the lower sphincter of a gullet, accelerates gastric emptying, and also accelerates advance of food on a small bowel, without causing diarrhea. Stimulates prolactin secretion.

Metoclopramidum passes through placental and hematoencephalic barriers and gets into maternal milk.

Indications to use:

Vomiting, nausea of various genesis (it can be in certain cases effective at the nausea and vomiting caused by use of tsitostatik). Atony and hypotonia of a stomach and intestines (in particular, postoperative); dyskinesia of biliary tract; reflux esophagitis; functional pyloric stenosis. It is applied to strengthening of a vermicular movement when carrying out X-ray contrast researches of digestive tract, and also as the means facilitating duodenal sounding.

Route of administration and doses:

Solution of Cerucal is entered intramusculary or intravenously. The adult appoint 10 mg 3–4 times a day. The maximum single dose makes 20 mg, daily — 60 mg. For children from 2 to 14 years the single dose makes 0,1 mg/kg of body weight, the highest daily dose &#!51; 0,5 mg/kg of body weight. Frequency of introduction is 1–3 times a day.

Duration of a course of treatment is defined by the doctor and makes usually 4–6 weeks. In some cases treatment can be continued up to 6 months. At a renal failure the dose of drug has to be reduced (at clearance of creatinine to 10 ml/min. — 10 mg of 1 times a day; from 11 to 60 ml/min. — 15 mg the day divided into 2 introductions).

Prevention and treatment of nausea and vomiting, caused by use of tsitostatik. Depending on the nature of chemotherapeutic treatment the daily dose makes from 2 to 4 mg/kg of body weight of the patient. Injection solution of cerucal can be dissolved with isotonic solution of sodium chloride or 5% glucose solution. It is impossible to mix with alkaline infusion solutions!

Features of use:

Due to the content of sodium sulfite it is not necessary to appoint injection solution of Cerucal the patient with bronchial asthma, with hypersensitivity to sulfites.

With care it is necessary to appoint the patient with arterial hypertension, Parkinson's disease, with an abnormal liver function and kidneys, to the aged patient, and also children as at them the risk of emergence of a diskinetichesky syndrome is much higher.

Use of Cerucal in the period of a lactation is not recommended, in need of use of drug it is necessary to resolve an issue of the breastfeeding termination.

During pregnancy (the 2 and 3 trimesters) and to children aged from 2 up to 14 years drug is appointed according to vital indications.

At use of drug it is necessary to avoid potentially dangerous types of activity requiring special attention, bystry mental and motor reaction (driving of vehicles, etc.).

During treatment by drug alcohol intake is not recommended.

Side effects:

In an initiation of treatment the lock, diarrhea, dryness in a mouth, feeling of fatigue, drowsiness, a headache, dizziness, a depression, excitement, irritability, sensation of fear, concern, allergic reactions are possible. In some cases, mainly at children, the diskinetichesky syndrome can develop. At prolonged use at patients of advanced age development of the phenomena of parkinsonism, late dyskinesia is possible. In rare instances at long administration of drug in high doses — a methemoglobinemia, hypostases, a galactorrhoea, a gynecomastia, disturbances of a menstrual cycle.

Interaction with other medicines:

Antikholinesterazny means can weaken action of Metoclopramidum. Drug strengthens absorption of tetracycline, ampicillin, paracetamol, acetylsalicylic acid, a levodopa; reduces absorption of digoxin and Cimetidinum. Strengthens effect of alcohol and medicines, the oppressing TsNS. It is not necessary to appoint along with neuroleptics since the risk of emergence of extrapyramidal reactions increases.

Contraindications:

Bleedings from digestive tract, a pyloric stenosis, mechanical intestinal impassability, perforation of a stomach or intestines, a pheochromocytoma, extrapyramidal disturbances, epilepsy, prolaktinozavisimy tumors, pregnancy, children's age up to 2 years, individual intolerance of Metoclopramidum.

Overdose:

Can be noted a hypersomnia. disorientation and extrapyramidal frustration. As a rule, the symptomatology disappears after the termination of administration of drug within 24 hours. If necessary treatment is carried out by holinoblokator and protivoparkinsonichesky means.

Storage conditions:

In the cool, protected from light place unavailable to children. Ready infusion solution can be stored — at the room temperature within 24 hours. Period of validity of 5 years. Not to apply after expiry date.
