Atsiklovir-Pharmak (cream)

General characteristics. Structure:

Active ingredient: aciclovir;

1 g of cream contains the acyclovir micronized in terms of 100% dry matter of 50 mg;

excipients: paraffin white, soft; liquid paraffin; cetostearyl alcohol (type A) emulsified; propylene glycol; half-oxameasures; диметикон; the water purified.

Pharmacological properties:

Pharmacodynamics. Selectively influences the Herpes simplex viruses of type 1 and 2, and also Varicella zoster. Competitively interacts with a virus thymidinekinase and it is consistently phosphorylated with education mono - di - and triphosphate; the last inhibits a DNA polymerase, joins instead of a deoxyguanosine in DNA of viruses and suppresses replication.

Prevents formation of new elements of rash, reduces probability of skin dissimination and visceral complications, accelerates formation of crusts.

Pharmacokinetics. At topical administration in the form of cream the acyclovir is practically not soaked up in a system blood stream.
Pharmaceutical characteristics.

Main physical and chemical properties: cream of white or almost white color.

Indications to use:

Infections of lips and the persons called by a herpes simplex virus.

Route of administration and doses:

Drug is used only for external use.

To adults and children 12 years cream are more senior it is necessary to apply on an affected area of 5 times a day each 4 hours, excepting night time. It is important to begin treatment at the first symptoms of an infection – in a prodromal stage.

Course of treatment - 5 days.  If improvement did not occur, it is possible to continue treatment up to 10 days.

If symptoms of a disease do not disappear in 10 days of treatment, the patient needs to see a doctor.

Before putting cream it is necessary to wash up hands; it is important not to pound affected areas and not to touch them with a towel to prevent deterioration in a course of a disease or transmission of infection.

Features of use:

Use during pregnancy and feeding by a breast.

There are no data on an adverse effect of an acyclovir during pregnancy or feeding by a breast as at topical administration the acyclovir practically does not get to a system blood stream and breast milk. However it is possible to use drug only if, according to the doctor, the expected positive effect for mother exceeds potential risk for the fruit/child.
Children.

Children up to 12 years have not enough data on safety of use of drug therefore children are recommended to use drug 12 years are more senior.

Use of cream on mucous membranes because of possibility of irritation is not recommended.

Consultation of the doctor is necessary for patients with especially heavy manifestations of a herpes infection; if necessary along with cream use drugs of an acyclovir of systemic action (a tablet, infusion).

Patients with herpes on lips (especially in the presence of open defeats) need to follow strictly rules of precaution not to allow infection of other people.

Ability to influence speed of response at control of motor transport or work with other mechanisms. Clinical trials on studying of influence of drug on speed of response were not conducted.

Side effects:

Drug is usually well transferred, however in the field of putting cream fast-passing burning, an acute pain can be felt, appear an insignificant peeling (at prolonged use of drug). In isolated cases allergic manifestations in the form of an erythema and an itch are possible, the Quincke's disease is very seldom possible; in these cases drug is cancelled and if necessary take the appropriate measures (the desensibilizing therapy, etc.). It was very seldom reported about developing of contact dermatitis after cream use. When carrying out tests for sensitivity similar cases are connected with components of a cream basis, but not with an acyclovir.

Interaction with other medicines:

It is not necessary to appoint drug together with local cationic antiseptic agents.

Contraindications:

Hypersensitivity to an acyclovir and other components of drug, and also to a valatsiklovir.

Overdose:

Cases of overdose were not observed. As drug is intended only for topical administration, development of symptoms of overdose is improbable.

Storage conditions:

Period of validity. 3 years. Not to use drug after the termination of the period of validity specified on packaging. To store in the place, unavailable to children, at a temperature from 15 °C to 25 °C; not to allow drug freezing.
