Aktovir

General characteristics. Structure:

Active ingredient: 100 mg of an acyclovir, 40 mg of a butaminofen.

Vvspomogatelny substances: liquid paraffin, hard paraffin, vaseline.

Pharmacological properties:

Pharmacodynamics. Aktovir represents the combined drug having antiviral (antiherpetic) effect which is caused by pharmacological properties of the active components entering it: an acyclovir (9-[marked (2-gidroksietooks)] guanine) and a butaminofen (2-гидрокси-3,3-ди-трет-бутил-N-фениланилина). Medicine Aktovir is highly active concerning the Herpes simplex 1 and 2 viruses of types, the virus causing chicken pox and shingles (Varicella Zoster); shows moderate activity concerning Epstein-Barre's virus and a cytomegalovirus.

The molecular and biological mechanism of antiviral activity of an acyclovir is caused by competitive interaction with virus thymidine kinase and phosphorylation with formation of monophosphate, diphosphate, and then triphosphate. By means of enzymes of a host cell triphosphate of an acyclovir is built in instead of a deoxyguanosine DNA of a virus and suppresses replication process. Molecule DNA which part the acyclovir is contacts a DNA polymerase, is irreversible inactivating it. Butaminofen is antioxidant of the phenolic nature and also has antiviral activity. The molecular and biological mechanism of antiviral action of a butaminofen is insufficiently studied. In pilot studies a synergism of antiviral action of combinations of these active components of drug Aktovir is established.

Drug use Aktovir promotes bystry stopping of inflammatory process, reduces an itch, hypostasis, a hyperemia, accelerates formation of crusts, interferes with emergence of fresh rashes. Prevents process progressing, reduces probability of skin dissimination.

Pharmacokinetics. Special researches of parameters of pharmacokinetics of drug Aktovir and his components were not conducted. When drawing on the unimpaired skin and mucous membranes receipt of an acyclovir in a system blood stream is minimum. When drawing on the affected skin (for example, at the surrounding herpes) absorption of an acyclovir moderate; at patients with normal function of kidneys concentration in blood serum makes up to 0,28 mkg/ml, at a renal failure — to 0,78 mkg/ml. It is removed by kidneys (to 9,4% of a daily dose). The second component of drug - бутаминофен when drawing on skin and mucous membranes practically does not come to a system blood stream.

Indications to use:

Aktovir apply at herpetic diseases of skin and mucous membranes: herpes simplex of skin and mucous membranes, genital herpes (primary and recurrent); the localized shingles (auxiliary treatment). Drug first of all is shown to use for patients with the chronic recurrent course of a herpesviridae infection, at resistance to monotherapy by an acyclovir.

Route of administration and doses:

Ointment Aktovir is applied outwardly, applied on affected areas of 4-6 times a day (as soon as possible after the beginning of an infection). The strip of ointment about 1 cm long should be applied on 25 square cm of the affected skin or mucous. It is important to begin treatment of a recurrent infection during a prodromal phase or at the very beginning of display of an infection. Treatment duration - not less than 5 days, at most - 10 days.

Features of use:

Drug Aktovir should not be applied on mucous an eye. Drug Aktovir is effective for prevention of a recurrence of herpes of lips. Efficiency of treatment by that will be higher, than earlier it is begun (at the first symptoms of an infection).

For implementation of medical effect the condition of immune system therefore at patients with reduced immunity (at HIV/AIDS or after transplantation of marrow) against the background of topical administration of ointment Aktovir it is necessary to appoint system administration of antiherpetic drugs, also as in case of a heavy and recurrent current of a herpes infection matters.

Aktovir does not prevent transfer of herpes sexually therefore during treatment it is necessary to abstain from sexual contacts, even in the absence of clinical displays of an infection.

Precautionary measures. For the prevention of autoinoculation of a virus on other sites of skin or mucous it is necessary to use fingerstalls or rubber gloves.

Side effects:

As a rule, undesirable effects at external use of drug Aktovir is not observed. Morbidity, burning, an itch, skin rash, a vulvitis, allergic reactions are in rare instances possible. In the presence of the specified manifestations drug it is necessary to cancel and appoint the corresponding therapy depending on weight of manifestations of undesirable reactions.

Interaction with other medicines:

At patients with the expressed bubble and erosive rashes purpose of ointment Aktovir with zinc oxide ointment is recommended.

If necessary perhaps simultaneous use of drug with benzylpenicillin (in the form of sodium and novocainic salt), klaritromitsiny, metronidazole, Halazolinum, acetylsalicylic acid, an aloe, B1, B6, S vitamins.

Contraindications:

Hypersensitivity to drug components, офтальмогерпес. With care use drug at pregnancy and in the period of a lactation.

Overdose:

Symptoms: headache, neurologic disturbances, short wind, nausea, vomiting, diarrhea, renal failure, lethargy, spasms, coma.

Treatment: gastric lavage, maintenance of the vital functions, hemodialysis.

Storage conditions:

At a temperature not above 25 °C. To store in the place, unavailable to children. Period of validity 2 years. Not to use after the period of validity specified on packaging.
