General characteristics. Structure:
Active ingredient: 75 g of potassium of chloride in 1 l of solution.
Excipients: water for injections.
Pharmacological properties:
Pharmacodynamics. The donator of potassium, recovers electrolytic balance. Potassium ions take part in regulation of excitability and conductivity of a myocardium, its automatism and contractility.
At treatment by foxglove drugs decrease in level of potassium in blood serum increases risk of development of arrhythmia, increase of potassium concentration reduces cardiotoxicity of cardiac glycosides and at the same time does not counteract their positive inotropic action. Potassium participates in process of carrying out nervous impulses and their transfers on the innervated bodies, improves reductions of cross-striped muscles at muscular dystrophy and a myasthenia. Introduction to an organism of potassium is followed by increase in maintenance of acetylcholine and excitement of a sympathetic part of the nervous system; at intravenous administration increase in allocation is noted by adrenaline adrenal glands.
Content of potassium in blood serum of the person makes about 5,11 mmol/l. Increase in content of potassium in blood by 4 times leads to a cardiac standstill.
Pharmacokinetics. The potassium ion is allocated generally with urine, in a small amount with a stake, and also with saliva, bilious and pancreatic juice.
Indications to use:
Hypopotassemia (including connected using Hydrochlorthiazidum and other saluretics, with pernicious vomiting, a profuse diarrhea, etc.), intoxication drugs of a foxglove, arrhythmia of various origin (connected generally with electrolytic disturbances and an absolute or relative hypopotassemia), a gipokaliyemichesky form of a paroxysmal mioplegiya, muscular dystrophy, a myasthenia.
Route of administration and doses:
At the hypopotassemia which is followed by disturbances of a rhythm of cordial activity enter intravenously kapelno 20-30 ml of 7,5% of solution into 500 ml of 5% of solution of glucose (to enter slowly! 20-30 drops a minute). After recovery of a cordial rhythm the dose of drug is reduced. At intoxication cardiac glycosides (in mild cases on 2-3 g of potassium of chloride a day, and in heavy - to 5 g).
Features of use:
Apply with care at disturbances of AV of conductivity. In the course of treatment control of level of potassium in blood and an ECG is necessary, and at treatment of states, scarce on potassium, it is necessary to control carefully BRAIDS. At too bystry intravenous administration development of a hyperpotassemia which can potentially lead to a lethal outcome is possible.
In need of use during pregnancy it is necessary to weigh the expected advantage for mother and potential risk for a fruit. In the period of a lactation it is necessary to resolve an issue of the breastfeeding termination.
Side effects:
Nausea, vomiting, diarrhea are possible. Seldom – a hyperpotassemia, confusion of consciousness, feeling of alarm, feeling of numbness or a pricking in hands, ступнях or lips, an asthma or the complicated breath, unusual feeling of fatigue, weakness, heavy feeling or weaknesses in legs.
Interaction with other medicines:
At simultaneous use of salts of potassium about potassium saving diuretics and APF inhibitors the risk of development of a hyperpotassemia increases. Under the influence of drugs of potassium side effect of cardiac glycosides decreases, effect of quinidine on heart, and also undesirable action of Disopyramidum on cardiovascular system amplifies.
Contraindications:
Disturbance of secretory function of kidneys, total block of heart, hyperpotassemia of various etiology. Pregnancy and a lactation Safety of use of solution of potassium of chloride of 7,5% for pregnant women and the feeding women is studied insufficiently. Drug can be used when the estimated advantage for mother exceeds possible risk for a fruit and mother.
Overdose:
Precursory symptom of intoxication potassium are paresthesias. Clinical manifestations of a hyperpotassemia usually appear at potassium concentration in serum from 7 to 8 ¼Ý¬ó/l. Heavier symptoms (including paralysis of muscles and a cardiac standstill) develop at potassium concentration 9-10 ¼Ý¬ó/l. At overdose of drugs of potassium enter chloride sodium solution inside or intravenously; or intravenously 300-500 ml of solution of a dextrose with the maintenance of 10-20 PIECES of insulin in 1000 ml. If necessary carry out a hemodialysis and peritoneal dialysis.
Storage conditions:
