Дофамин-Ферейн®

General characteristics. Structure:

Active ingredient: 5 g or 40 g of a dopamine in 1 ml of solution.

Excipients: sodium of pyrosulphite, 0,1 M acid solution hydrochloric, water for injections.

Pharmacological properties:

Pharmacodynamics. Dopamine - cardiotonic, hypertensive means. Stimulates b1 - adrenoceptors, is an agonist of dopamine receptors. Stimulates a-adrenoceptors in high doses and increases resistance of peripheral vessels and systolic arterial pressure, strengthens cordial reductions, increases cordial emission.

Expands mezenterialny and renal vessels, reduces resistance of the last, increases in them a blood stream, stimulates renal filtering, raises a natriuresis. Inhibits synthesis of Aldosteronum.

Pharmacokinetics. After intravenous administration it is completely distributed in an organism, partially passes through a blood-brain barrier. It is metabolized in a liver, kidneys and a blood plasma with the participation of a monoaminooxidase and catechol-0-methyltransferase to inactive metabolites. Plasma elimination half-life of blood - about 2 minutes, from an organism - about 9 minutes.

It is removed by kidneys: about 80% within 24 hours, mainly, in the form of metabolites, a small part of a dose are removed in not changed look.

Indications to use:

Shock of various etiology (cardiogenic, traumatic, postoperative, etc.); for improvement of indicators of a hemodynamics at acute heart and vascular failure, a syndrome of "low cordial emission", connected with surgical intervention, especially with surgeries on heart.

Route of administration and doses:

Enter intravenously kapelno slowly. Ampoule contents (0,5% and 4% dopamine solution) are parted respectively in 125 or 400 ml by 5% of solution of glucose or isotonic solution of sodium chloride from calculation that 1 ml of solution contained respectively 200 or 500 mkg of dopamine.

At contraindications apply more strong solutions where 1 ml contains 800 mkg to introduction of large volumes of liquid. Infusions carry out continuously during of 2-3 h up to 1-4 days and more. Daily dose on average 400-800 mkg. Effect of drug comes quickly and comes to an end in 5-10 minutes after the end of introduction.

Дофамин-Ферейн® enter under cardiomonitor control. Doses and rate of administering define taking into account the level of arterial pressure, heart rate, a heart rhythm, cordial emission and a diuresis. Apply only in the conditions of a hospital.

Features of use:

Pregnancy and lactation. At pregnancy and in the period of a lactation of Dofamin-Fereyn® apply only when the estimated advantage for mother exceeds potential risk for a fruit or the child.

It is necessary to select an optimum dose in each separate case under constant control of indicators of a hemodynamics and ECG.

At hypovolemic shock it is necessary to combine use of Dofamina-Fereyn® with administration of plasma or plasma substituting drugs (or blood).

Side effects:

From cardiovascular system: tachycardia, pains behind a breast, increase in arterial pressure are possible; at use of high doses - arrhythmia.

From digestive tract: nausea, vomiting are possible.

From the central nervous system: the headache, concern, a tremor are possible.

Allergic reactions: the bronchospasm is possible.

Local reactions: at hit of dopamine under skin necroses of skin, hypodermic cellulose are possible.

Interaction with other medicines:

At simultaneous use of Dofamina-Fereyn® and Octadinum strengthening of sympathomimetic action is noted; with monoaminooxidase inhibitors - strengthening of intensity and duration of the cardiostimulating and pressor effect of dopamine is possible.

Introduction of Dofamina-Fereyn® against the background of reception of tricyclic antidepressants leads to strengthening of its effects (possibly development of tachycardia, arrhythmia, heavy arterial hypertension).

Diuretics strengthen diuretic action of Dofamina-Fereyn®.

Contraindications:

Thyrotoxicosis, closed-angle form of glaucoma, benign hyperplasia of a prostate. Pheochromocytoma. It is not necessary to appoint at arrhythmia in combination with monoaminooxidase inhibitors, with cyclopropane and galogensoderzhashchy anesthetics.

Storage conditions:

List B. To store in the place protected from light. To store in the place, unavailable to children. A period of validity - 3 years. Not to use after the term specified on packaging.
